Redefining Neck Rejuvenation: The Novel Hyoid-to-Mastoid Crevasse Neo-Ligament and OnderKaak Angle in Deep Plane Neck Lift.

BACKGROUND: Neck rejuvenation has evolved from superficial skin tightening to deeper structural modifications targeting the platysma, submental and subplatysmal fat, and the submandibular gland (SMG). Significant controversy surrounds the surgical maneuvers necessary to achieve the ideal neck. OBJECTIVES: The objective is to introduce the Hyoid-to-Mastoid Crevasse (HMC) neo-ligament for neck rejuvenation that elevates the SMG via a suture secured from the hyoid periosteum to the mastoid crevasse. Moreover, we introduce the OnderKaak (OK) angle, representing the region overlying the submandibu…

Temporal bone mucocele with extension to mastoid in a 60-year-old male: a case report and literature review.

INTRODUCTION: Mucoceles, characterized by cystic lesions with sterile mucus accumulation, mostly occur in the paranasal but rarely affect the temporal-mastoid bone. The pathogenesis involves obstruction of natural drainage or cyst enlargement. Here, the authors present a rare case of temporal-mastoid mucocele with temporal hypertrophy. CASE PRESENTATION: A 60-year-old male presented with progressive painless right temporal region swelling associated with diminished hearing, headache, fever and ear symptoms. Audiometry indicated a profound hearing loss in the right ear. Computed tomography (CT…

Preoperative predictive criteria for mastoid extension in pars flaccida cholesteatoma in assessments using temporal bone high-resolution computed tomography.

OBJECTIVES: We aimed to clarify the usefulness of high-resolution computed tomography (HRCT) and establish HRCT criteria for presurgical assessment of the mastoid extension in pars flaccida cholesteatomas of the middle ear. METHODS: Retrospective observational study. Patients who underwent primary surgery for pars flaccida cholesteatoma and those who underwent temporal bone HRCT for pretreatment evaluation were reviewed. The distance in the anterior-most portion of the mastoid sinus on HRCT was measured, and the presence of surgically verified mastoid extension of cholesteatoma was evaluated.…

Does Mastoid Pneumatization Affect Facial Canal Dimensions and Distances of Facial Tympanic Segment-Scutum and Lateral Semicircular Canal-Scutum?

OBJECTIVES: In the present study, we investigated whether mastoid pneumatization affects facial canal dimensions and distances of facial tympanic segment and scutum, and lateral semicircular (LSS) canal and scutum. METHODS: One hundred sixty-one temporal multidetector computed tomography scans were reviewed. Patients with unilateral sclerotic mastoid pneumatization (no aeration) (group 1, n = 81) and unilateral total mastoid pneumatization (100.0% aeration) (group 2, n = 80) were included. Facial canal dimensions at the labrythine, tympanic, and mastoid segments; facial canal dehiscence and l…

Left Chronic Otitis Media with Squamous Cell Carcinoma of the Middle Ear and Postauricular Mastoid Fistula: A Case Report.

Though squamous cell carcinoma is the commonest tumor, it rarely presents in the middle ear and follows a history of chronic ear discharge. Postauricular mastoid fistula is also a rare complication of chronic otitis media. These two pathological changes occurring simultaneously are even rarer. We report a rare case of a 37 years old male with a history of left-sided chronic ear discharge and conductive hearing loss. Squamous cell carcinoma along with postauricular mastoid fistula was diagnosed based on high-resolution computed tomography scan and histopathology. Left modified radical mastoidec…

Further application of the bilobed flap: the split bilobed flap for reconstruction of composite posterior auricular and mastoid defects.

In this article a modified bilobed flap from mastoid and lateral neck skin for reconstruction of complex defects of the posteromedial surface of the auricle and mastoid skin, with the preservation of the retroauricular sulcus, is described. Reconstruction of the postero-medial auricular surface has almost never been a concern for reconstructive surgeons. It is in fact a shaded area with little aesthetic relevance and direct closure, skin grafting and even secondary healing are used for skin cancer defects repair. Also mastoid skin defects can be repaired with simple techniques such as skin gra…
