OBJECTIVES: To examine the incidence and nature of complicating factors in surgery for chronic otitis media (COM) in a metropolitan public h … nts had cholesteatoma involvement. Most patients (73%) were managed by an extended modified radical mastoidectomy. An additional 31 procedures were required in these 33 patients for control of complications. There was no iatrogenic sensorineural hearing loss or facial paralysis.
CONCLUSIONS: The incidence of CCOM in this large, U.S. metropolitan public hospita …

The aim of this retrospective study was to evaluate the functional results of surgical lowering of the high jugular bulb in the treatment of patients with Ménière's disease and pulsatile tinnitus. Fifteen patients with disabling Ménière's disease associated with pulsatile tinnitus and a high and medial jugular bulb were included in this study. As treatment a complete mastoidectomy was performed, after which the jugular bulb was freed by an infralabyrinthine and subfacial approach. The bulb was then displaced downwards with surgical wax. Functional results of surgery were assessed by a question…

A 12-year-old girl had a 1-week history of left otalgia with hearing loss in May, 1989. Audiometry showed a conductive hearing loss of 60 dB. The conventional medical treatment for acute otitis media was initiated, but the symptoms and signs failed to respond this treatment. Four weeks after initial presentation, a diagnostic mastoidectomy was performed. The histological findings of the granulomatous tissue in the mastoid cavity showed nonspecific granulomatous inflammation, infiltrated with multinucleated giant cells. Initial laboratory studies revealed elevated ESR, positive CRP and RA, inve…

The authors report on 5 observations of necrotising angiitis with a specifically otological onset, namely 4 Wegener's syndromes and one periarteritis nodosa. In all 5 cases, the initial symptomatology consisted of otological signs in a feverish context, producing pictures of serous or sero-purulent otitis. Its resistance to usual forms of therapy led in 3 cases to the installation of transtympanic aerators, and in 2 cases to the performance of an antro-attico-mastoidectomy. One of the observations led to the discovery in the middle ear of a histological aspect specific to the disease, from whi…

Pyoderma gangrenosum (PG) is an immune-mediated skin condition that is commonly associated with inflammatory bowel diseases (IBD). Its diagnosis and management are often challenging due to the heterogeneity of the clinical presentation and the need for an individualized therapeutic approach. We present the case of a 19-year-old Caucasian male with Crohn's disease, who received therapy with a biological agent and developed severe PG in the retroauricular region following mastoidectomy for mastoiditis, which was complicated by cerebral venous sinus thrombosis. The diagnosis of PG was made with t…

Canal wall down (CWD) mastoidectomy is a surgical procedure that is essential to eradicate chronic ear diseases but results into bone defects due to the resulting mastoid cavity. This study investigated the ability of human tonsil-derived mesenchymal stem cells (hTMSCs) combined with a hydroxyapatite (HAp)-chitosan patch to promote osteogenesis in the treatment of postoperative temporal bone defects using an animal model. At 12 weeks postsurgery, the obliteration ratio and bone formation in the tympanic bulla of rats that were grafted with hTMSCs and a hydroxyapatite (HAp)-chitosan patch or no…

OBJECTIVE: To report a novel tympanoplasty modification for anterior tympanic membrane perforation closure.
MATERIALS AND METHODS: A prospective study on 13 patients who underwent inferior tympanoplasty between December 2008 and May 2011 was carried out. In our technique, an inferior rather than a posterior flap is raised and the graft is laid from the inferior direction to obtain better access to the anterior part of the tympanic membrane perforation and provide better support.
RESULTS: A total of 13 patients underwent the novel inferior tympanoplasty technique with a mean age of 33 years.

CONCLUSION: Matrix removal by a non-suction technique with intraoperative dexamethasone injection is a safe and effective management modality, regardless of fistula size.
OBJECTIVE: Our goal was to evaluate the outcome of hearing treated by non-suction technique with intraoperative dexamethasone injection.
METHODS: This was a retrospective chart review of 720 mastoidectomy cases for cholesteatoma, performed at our tertiary otolaryngologic care centers between 2005 and 2012. A total of 17 patients with a unilateral labyrinthine fistula were encountered.
RESULTS: There was no recurrent choles…

The surgical procedures applied were: myringotomy with drainage, cortical mastoidectomy and revision of zygomatic area and treatment with antibiotics (ceftriaxon). Enterococcus faecalis and Streptococcus viridans were found in the culture of middle ear and mastoid effusion. After half a year of follow-up the child had a normal hearing. …

The work was undertaken to investigate the spectrum of bacteria responsible for acute mastoiditis and to find out whether it is affected by intake of antibiotics prior to surgical treatment. The records were reviewed of 22 children with acute mastoiditis in whom mastoidectomy had been performed and bacterial cultures obtained. Eleven of the patients had had antibiotic treatment prior to admission (9 penicillin V and 2 erythromycin). Streptococcus pneumoniae was found in 8 of the purulent discharges: Haemophilus influenzae, Streptococcus beta-hemolyticus and Staphylococcus aureus in 2 each; Pro…

There are actually two principal approaches to the surgical treatment of middle ear cholesteatoma, a subject very much debated. First, the cholesteatoma can be exteriorized and treated with an open technique (classic or modified radical mastoidectomy, open tympanoplasty); and second, the radical removal of the cholesteatoma is also possible in the majority of cases by closed tympanoplasty. In this article we examine 283 patients (forty-four children and 239 adults) operated on by the closed technique to review and discuss the postsurgical cholesteatoma problem (residual and recurrent). Total i…

Chronic otitis media (COM) is treated medically and/or surgically. Ossiculoplasty for COM with severe-to-profound hearing loss has not been recommended because the severity of hearing loss before surgery is negatively associated with postoperative hearing improvement. Our study assessed the effectiveness of ossiculoplasty for severe-to-profound hearing loss and identified prognostic factors associated with favorable outcomes. We retrospectively reviewed the medical records of 182 patients with severe-to-profound hearing loss who underwent ossiculoplasty with tympanoplasty and mastoidectomy for…

Temporal bone reconstruction is a persisting problem following middle ear cholesteatoma surgery. Seeking to advance the clinical transfer of stem cell therapy we attempted the reconstruction of temporal bone using a composite bioartificial graft based on a hydroxyapatite bone scaffold combined with human bone marrow-derived mesenchymal stromal cells (hBM-MSCs). The aim of this study was to evaluate the effect of the combined biomaterial on the healing of postoperative temporal bone defects and the preservation of physiological hearing functions in a guinea pig model. PURPOSE: The location of the sigmoid sinus may change according to the ventilation status of the mastoid bone, in which case the relationship of the sigmoid sinus to the facial nerve and semicircular canals is predicted to change. The purpose of this study was to evaluate the importance of sigmoid sinus distances to the facial nerve and semicircular canals concerning mastoid surgery.
METHODS: The relationship of the sigmoid sinus and the facial nerve with the semicircular canals was analyzed from the cone beam computed tomography images of 75 male and 95 female subjects, aged 18-65 years. Fir
