The ulnar artery free flap: the first 7 years.

This paper reviews 7 years of experience with the ulnar artery free flap at the Wellington Regional Plastic Surgery Unit. It has become the free flap we use most commonly to repair defects after resection of intraoral cancers, other head and neck defects, and soft-tissue defects of the lower limbs. Of 56 attempted cases, 52 have been successful. Two procedures were abandoned intraoperatively because of abnormalities in vascular pattern, and 2 failed in the lower limb because of anastomotic failure. These 4 cases occurred early in the development of the technique; all cases in the last 4 years

Spontaneous Regression of Parapharyngeal Arteriovenous Malformation.

Arteriovenous malformations (AVMs) are fast-flow vascular malformations that mostly occur in the head and neck region. They are typically progressive and their spontaneous regression is almost never seen. We present a case with pulsatile tinnitus and a parapharyngeal AVM. It resolved completely after diagnostic catheter-based angiography alone.

Transplantation of vascularized submandibular gland in dogs.

PURPOSE: Presently, treatments for xerostomia only target symptoms, as an active therapy method has not been established. Herein, we discuss the possibility of using a submandibular gland allograft technique for the disease. MATERIALS AND METHODS: Using a vascularized submandibular gland transplantation method, we extracted portions of the submandibular gland, including the duct and chorda tympani branches, from beagle dogs and placed them into the submental region of age- and weight-matched dogs. We then measured the amount of saliva secretion and examined the grafted glands histologically.

Anatomical Evaluation of the Pterygomaxillary Complex Using Cone Beam Computed Tomography.

BACKGROUND: The pterygomaxillary region is a complex anatomical area formed by the junction of the maxillary, palatine, and sphenoid bones and contains critical neurovascular structures. Accurate assessment of this region during Le Fort I osteotomy is essential, particularly to prevent hemorrhage and nerve injury that may occur during the pterygomaxillary separation phase. This study aims to investigate the morphometric characteristics of the pterygomaxillary region using cone-beam computed tomography (CBCT) and to evaluate the effects of age, sex, and laterality on these anatomical parameters

MICROVASCULAR DECOMPRESSION FOR TRIGEMINAL NEURALGIA.

BACKGROUND: Trigeminal Neuralgia (TGN) is the most frequently diagnosed type of facial pain. In idiopathic type of TGN it is caused by the neuro-vascular conflict involving trigeminal nerve. Microvascular decompression (MVD) aims at addressing this basic pathology in the idiopathic type of TGN. This study was conducted to determine the outcome and complications of patients with idiopathic TGN undergoing MVD. METHODS: In a descriptive case series patients with idiopathic TGN undergoing MVD were included in consecutive manner. Patients were diagnosed on the basis of detailed history and clinica

Rhino-orbital-cerebral mucormycosis.

This review focuses on sinus, sino-orbital, and rhinocerebral infection caused by the Mucorales. As the traditional term of "rhinocerebral" mucormycosis omits the critical involvement of the eye, the more comprehensive term as rhino-orbital-cerebral mucormycosis (ROCM) is used. The most common underlying illnesses of ROCM are diabetes mellitus, hematological malignancies, hematopoietic stem cell transplantation, and solid organ transplantation. Sporangiospores are deposited in the nasal turbinates and paranasal sinuses in immunocompromised patients. Qualitative and quantitative abnormalities o

Pathological evaluation of radiotherapy and concomitant intraarterial cisplatin for maxillary sinus cancer.

OBJECTIVE: Since 2010, we have mainly performed surgical treatment following radiotherapy and concomitant intraarterial cisplatin (RADPLAT) for locally advanced maxillary sinus cancer (MSC). The present study investigated treatment results and pathological evaluations following RADPLAT for MSC. METHODS: Pathological response to RADPLAT was evaluated using surgical specimens. Pathological response was graded in accordance with the classification method that Shimosato reported in 1964, as grade V (no tumor cells remain in any of section), grade IV, III, II, I, and 0. [Sinonasal cavities: CT imaging features of anatomical variants and surgical risk].

Anatomical variants of the sinonasal cavities are common. About 15 major variants are described (nasal septal deviation is present in up to 62% of the population). Because the may lead to complications at the time of endovascular procedures or endoscopic sinonasal surgery (vascular, nervous, or osseous injury), there detection has medicolegal implications. Knowledge of anatomical variants by radiologists and ENT surgeons is thus required. We will describe these variants, their imaging features, frequency, implications and associated risk of potential complication.

Slab maximum intensity projection images of rotational angiography are useful for evaluation of the perforating branch in a case of basilar artery incarceration secondary to clival fracture.

BACKGROUND: In the event of clivus exhibiting longitudinal fractures, the basilar artery (BA) may become entrapped at the fracture site, although this occurrence is infrequent. Occlusion of bilateral BA perforators, accompanied by entrapment of the BA, can result in brainstem ischemia, which often leads to unfavorable outcomes. We report here the usefulness of slab maximum intensity projection (MIP) images of rotational angiography in evaluating the BA perforators. CASE DESCRIPTION: A 73-year-old man fell from a two-meter stepladder and was transferred to the emergency department. His Glasgow

Methemoglobinemia caused by a low dose of prilocaine during general anesthesia.

Methemoglobinemia is a blood disorder in which an abnormal amount of methemoglobin is produced, and prilocaine is one of the drugs that can cause this disorder. The maximum recommended dose of prilocaine is 8 mg/kg. We report a case of methemoglobinemia caused by the administration of 4.2 mg/kg of prilocaine without other methemoglobinemia-inducing drugs during general anesthesia. A 17-year-old girl with hyperthyroidism and anemia was scheduled to undergo maxillary sinus floor elevation and tooth extraction. The patient's peripheral oxygen saturation (SpO2) decreased from 100% at arrival to 95

The Whole Lateral Type of the Sphenoethmoidal Cell and Its Relevance to Endoscopic Sinus Surgery.

BACKGROUND: The sphenoethmoidal cell and the sphenoid sinus (SS) show great similarity in endoscopy and imaging. Hence, it is important to accurately identify the sphenoethmoidal cell preoperatively to prevent injury of the nerve and artery during endoscopic surgery. The aim of the present study was to investigate a special type of sphenoethmoidal cell. METHODS: A total of 365 inpatients whose paranasal sinus computed tomography (CT) was collected and reviewed from May 2018 to September 2019 were included. The anatomical imaging characteristics of the sphenoethmoidal cell were observed. RESU

Intraoral vertico-sagittal ramus osteotomy: modification of the L-shaped osteotomy.

The sagittal split ramus osteotomy and intraoral vertical ramus osteotomy carry the potential risk of postoperative nerve paralysis, bleeding, and fracture and dislocation of the condyle. In 1992, Choung first described the intraoral vertico-sagittal ramus osteotomy for the purpose of avoiding postoperative dislocation of the condyle. However, there is still potential for damaging the inferior alveolar nerve and maxillary artery with this technique. The authors have developed a modified technique to minimize these risks. An evaluation of surgical experience and patient outcomes with the use of…

Technique for reconstruction of large clival defects through an endoscopic-assisted tunneled retropharyngeal approach.

BACKGROUND: Reconstruction of the clivus and posterior cranial base defects following endoscopic skull-base surgery can be particularly challenging. Commonly, defects in this region are repaired with pedicled mucoperichondrial flaps from the sinonasal cavity. Complex and large defects often require regional or free flaps, particularly when intranasal flaps have been exhausted. While there are no primary barriers to routing flaps into the clivus or nasopharynx in large open approaches, secondary surgical corridors are necessary during endoscopic cases for routing of free flaps. Routing of free-

The trigeminocardiac reflex: Does the activation pathway of its efferent arc affect the intensity of the hemodynamic drop during the management of maxillofacial fractures?

The study aimed to correlate between the stimulated nerve, intensity of trigeminovagal reflex (TVR), and neuropathophysiological pathway by which the efferent arc is activated. Material and methods: A retrospective study included patients who developed TVR during the surgical management of mandibular, midface, and orbital fractures. The reflex was divided into type I, II, III, and IV-TVR according to the following nerves: ophthalmic, maxillary, mandibular, and non-trigeminal nerves, respectively. The magnitude of hemodynamic drops was identified at the intraoperative baseline, during reflex, a

[Endoscopic Surgery for Trigeminal Schwannoma].

Minimally invasive surgical techniques, including the endoscopic endonasal transmaxillary-pterygoid approach (EETMPA), the endoscopic transorbital approach (ETOA), and the endoscopic keyhole approach (EKA), have recently been applied to the surgical treatment of trigeminal schwannomas. However, criteria for patient selection and technical nuances remain sparsely reported; therefore, we describe these aspects based on our previous work. For patient selection, a sufficient surgical corridor to the tumor from the paranasal sinus must be identified on magnetic resonance imaging to perform EETMPA.
