Extranodal nasal-type NK/T-cell lymphoma of the palate and paranasal sinuses.

BACKGROUND: Extranodal nasal-type natural killer (NK)/T-cell lymphoma represents a rare entity, typically originating in the nasal cavity, palate or midfacial region. Signs and symptoms include non-specific rhinitis and/or sinusitis, nasal obstruction, epistaxis, facial swelling and development of deep necrotic ulceration in the midline of the palate, causing an oronasal defect. Differential diagnosis includes fungal infections, Wegener's granulomatosis, tertiary syphilis, other non-Hodgkin's lymphomas and malignant epithelial midline tumors. CASE REPORT: We present a case of a 40-year-old ma

Malignant transformation of a maxillary follicular ameloblastoma to squamous cell carcinoma: A case report and review of literature.

INTRODUCTION: Ameloblastoma is a benign but locally infiltrative epithelial odontogenic neoplasm of the jawbones. Ameloblastoma and squamous cell carcinoma commonly affect the mandible and maxilla. However, malignant transformation of ameloblastoma into squamous cell carcinoma is extremely rare. CASE PRESENTATION: A 72-year-old man presented with a rapidly enlarging mass in the maxilla. Diagnostic imaging revealed a large mass with lytic changes involving the anterior and left aspects of the maxilla, and extending superiorly into the maxillary sinus. The patient underwent curettage treatment

Coronoid-temporalis pedicled rotation flap for middle skull base reconstruction.

We report on a patient who underwent maxillectomy and subsequent coronoid-temporalis flap reconstruction of the surgical defect to treat a maxillary sinus carcinoma invading the middle skull base. The patient underwent successful reconstruction of the middle skull base and displayed acceptable functional result. Middle skull base defects can be successfully repaired using a local coronoid-temporalis pedicled flap. The coronoid process of the mandible is advantageous as a local flap because it is close to the skull base defects and can easily be dissected from adjacent tissues. The use of a cor

Reconstruction of midface bone defects with vitallium micromesh.

Complex midface defects following trauma and tumor resection present a challenging problem for maxillofacial surgeons. Regardless of the cause of these defects, restoration of accurate contour is essential to achieve both proper function and cosmetically acceptable appearance. Traditionally, bone grafts have been the preferred method of reconstruction. We present the first report of the use of Vitallium micromesh in reconstruction of midface bone and cartilaginous defects in 3 patients. In 1 patient, bone loss resulted from trauma; in the other 2, loss was due to tumor resection. In all 3 pati

[Fronto-naso-ethmoido-sphenoido-maxillo-orbital mucocele with ophthalmologic presentation].

PURPOSE: We present a case of a large mucocele pressing the orbit forward and compared our findings with those reported by others. CASE REPORT: A 23-year-old woman was examined for an inflammatory tumor of the internal canthus. She had a 6/10 vision loss of the left eye, diplopia, and non-axial exophthalmos. CT scan and magnetic resonance imaging evidenced a huge sinus mucocele behind the orbit. Surgery using the paralateronasal approach was performed. Pathology confirmed the diagnosis of mucocele. DISCUSSION: Ophthalmic complications of mucoceles result from tumor growth leading to compress

Analysis of DNA methylation and microRNA expression in NUT (nuclear protein in testis) midline carcinoma of the sinonasal tract: a clinicopathological, immunohistochemical and molecular genetic study.

The aim of this study was a detailed clinicopathological investigation of sinonasal NUT midline carcinoma (NMC), including analysis of DNA methylation and microRNA (miRNA) expression. Three (5%) cases of NMC were detected among 56 sinonasal carcinomas using immunohistochemical screening and confirmed by fluorescence in situ hybridization. The series comprised 2 males and 1 female, aged 46, 60, and 65 years. Two tumors arose in the nasal cavity and one in the maxillary sinus. The neoplasms were staged pT1, pT3, and pT4a (all cN0M0). All patients were treated by radical resection with adjuvant r

Spatially resolved ex vivo drug response profiling in SMARCB1-deficient sinonasal carcinoma.

SMARCB1-deficient sinonasal carcinoma (SDSC) is a rare, highly aggressive malignancy with limited therapeutic options and no established preclinical models. Here, single-nucleus RNA sequencing (snRNAseq), spatial transcriptomics, and ex vivo patient-derived tissue slice culture (TSC) were combined to resolve intratumoral heterogeneity, niche organization, and treatment vulnerabilities in an index SDSC. snRNAseq identified three malignant subpopulations, including two specialized states marked by ALDH1A1 and NTN4. Spatial profiling mapped these states to distinct niches. The ALDH1A1+ compartmen

Interpretable clinical-radiomics fusion model for noninvasive prediction of INI1 deficiency and prognosis in sinonasal undifferentiated carcinoma (SNUC): a two-center study.

PURPOSE: INI1 (Integrase interactor 1) deficiency (defined as loss of nuclear INI1 expression on immunohistochemistry) identifies an aggressive subtype of sinonasal undifferentiated carcinoma (SNUC) associated with poor prognosis. This study aimed to develop and validate an interpretable clinical-radiomics fusion model for noninvasively prediction of INI1 deficiency and assess its prognostic value. METHODS: In this two-center retrospective study, 183 SNUC patients (training: n = 140; test: n = 43) were enrolled. Clinical variables were analyzed via univariate and multivariate analyses to iden

Prognostic value of the pretreatment albumin:globulin ratio combined with adult comorbidity evaluation 27 and TMN staging in patients with squamous cell carcinoma of the maxillary sinus.

The albumin:globulin (A:G) ratio, adult comorbidity evaluation 27 (ACE-27), and TMN staging have been shown to be strong predictive indicators of the survival of patients with many types of tumours. We have investigated the prognostic value of pretreatment based on the A:G ratio combined with TMN staging and ACE-27 in patients with squamous cell carcinoma (SCC) of the maxillary sinus. We studied 196 patients, and the prognostic value was explored by univariate and multivariate Cox's hazards analysis. Multivariate analyses suggested that pretreatment A:G ratio was independently associated with

Genetic profiling of poorly differentiated sinonasal tumours.

The sinonasal cavities harbour a variety of rare tumour types. Many carry a poor prognosis while therapeutic options are limited. Histopathological classification can be difficult, especially for poorly differentiated tumours such as olfactory neuroblastoma (ONB), sinonasal neuroendocrine carcinoma (SNEC) and sinonasal undifferentiated carcinoma (SNUC). We analysed Affymetrix OncoScan genome-wide copy number profiles of these three tumour types, both as originally diagnosed and as regrouped by their cytokeratin (Ck) and neuroendocrine (Ne) expression pattern, aiming to find a relation between

Nodal metastasis and elective nodal level treatment in sinonasal small-cell and sinonasal undifferentiated carcinoma: a surveillance, epidemiology and end results analysis.

OBJECTIVE: Risk of nodal involvement in patients with sinonasal small-cell carcinoma and sinonasal undifferentiated carcinoma (SNUC) has not been well defined because of their rarity. We describe a population-based assessment of specific nodal level involvement in this group of rare neuroectodermal tumours. METHODS: The Surveillance, Epidemiology and End Results (SEER) database from 2004 to 2011 identified patients with SNUC and sinonasal small-cell carcinoma. Overall neck involvement and individual nodal level involvement at presentation were assessed, and comparison was made with a contempo

Management of spindle cell carcinoma of the maxillary sinus: a case report and literature review.

BACKGROUND: Carcinosarcomas, also known as spindle cell carcinomas, are rare and highly aggressive tumors characterized by dual histologic differentiation of squamous cell and mesenchymal cell tumors. Occurrence of carcinosarcoma in maxillary sinus is very rare, with only 11 cases reported since 1957. The small number of reported cases creates an obstacle to the increased understanding of the behavior, prognosis, and therapeutic management of this tumor. CASE REPORT: A 52-year-old man presented with a 2-month history of right nasal obstruction. Computed tomography (CT) and magnetic resonance

Herniation of the antral membrane through an extraction site. Report of a case.

A case of herniation of the antral membrane through a recent extraction site is presented. The lesion was initially diagnosed as a tumor by the referring dentist. Oroantral fistulas are not an uncommon finding after dental extractions. It is rare, however, to see herniation of the antral membrane with large polyps extending through the fistula into the oral cavity. A search of the literature revealed only two other such cases. Surgical removal of the polyps and closure of the oroantral fistula by means of a buccal sliding flap technique is discussed.

Maxillary sinus pyogenic granuloma with orbital extension: a rare pediatric presentation mimicking malignancy.

We report the case of an 11-year-old girl who presented with 2 weeks of nonspecific facial pressure and pain and who was ultimately found to have a right maxillary sinus mass with orbital invasion. Initial imaging and clinical features raised concern for malignancy. However, following biopsy and surgical resection, final pathology revealed a pyogenic granuloma. This benign vascular lesion has rarely been described in the maxillary sinus and, to our knowledge, has not previously been reported to involve the orbit. This case underscores the importance of maintaining a broad differential diagnosi
