Metformin-Associated Lactic Acidosis Following Acute Overdose: Clinical Course and Conservative Management.

Metformin is an antidiabetic medication that belongs to the class of oral hypoglycemic agents: Biguanides. Although its mechanism of action remains elusive, it is well established that metformin enhances insulin sensitivity and decreases hepatic gluconeogenesis. Metformin usually does not provoke hypoglycemia and is well tolerated. Despite this fact, metformin-associated lactic acidosis (MALA) remains one of the concerning side effects especially in the context of tissue hypoxia and pre-existing renal failure. Management usually involves maintenance of hemodynamic stability, correction of acid

Usefulness of Therapeutic Drug Monitoring and Pharmacogenetics for a Patient Treated with Olanzapine, Buspirone, and Fluvoxamine: A Case Study.

BACKGROUND: A patient, with a mental disorder caused by an intracranial infection, treated with olanzapine, fluvoxamine, and buspirone. The plasma exposure of olanzapine was too high at standard doses, with evidence indicating that it was caused by drug-drug interactions. METHODS: Using pharmacogenomics and therapeutic drug monitoring to guide drug dose adjustment for a patient in clinical practice. RESULTS: The patient underwent pharmacogenetic testing in addition to therapeutic drug monitoring as part of a pharmacist-led comprehensive evaluation of medication therapy management in a clinic…

Recurrent headaches with visual disturbance.

Headache, particularly migraine, is the commonest neurological problem with which patients present to general practitioners and neurologists. Episodic migraine affects up to 18% of women and 6% of men. Acute migraine attacks can be severely disabling and chronic migraine is even more disabling. Of the mental and neurological disorders, migraine ranks eighth worldwide in terms of disability. Migraine is one of the primary headaches and may occur with or without aura. Differentiation from other severe primary headaches, such as cluster headache, is important for management. The vast majority of

Parent-professional agreement on diagnosis and recommendations for children with developmental disorders.

Twenty-eight children with developmental disorders underwent an interdisciplinary assessment for diagnosis and recommendations about management. Following this assessment, the degree of agreement on diagnosis and recommendations between parents and the team was determined. There was substantial agreement for diagnosis of clearly identifiable conditions such as genetic disorders, autism, and mental retardation but less agreement for milder disorders such as learning disabilities, attention deficit disorder, and emotional disorders. Agreement on management varied, being higher for speech therapy

Enhanced Recovery After Surgery Protocol for Primary Cleft Palate Repair: Improving Transition of Care.

BACKGROUND: Enhanced recovery after surgery (ERAS) protocols have been adopted for many types of surgery. Postoperative pain following palatoplasty may cause feeding and swallowing difficulty. Our study evaluated the use of ERAS protocols to improve the management of pain following primary palatoplasty as well as in the transition of care from inpatient to outpatient. METHODS: An Institutional Review Board approved retrospective analysis was performed for patients who previously underwent primary palatoplasty before ERAS implementation. Separately, an Institutional Review Board approved prosp

Medical Management of Aortic Disease: If They Don't Need Surgery, What Do They Need?

Management of aortic disease has evolved significantly over the past few decades. A preemptive diagnostic approach with a multidisciplinary team and shared decision-making has led to improved clinical outcomes. Surgery is the cornerstone of management for most aortic conditions; however, medical therapy is now an important adjunctive therapy in most if not all patients. Herein, we review the role and evidence behind medical management of patients with aortic disease.

Update on Restless Legs Syndrome Management during Pregnancy.

Restless legs syndrome (RLS) affects about 1 in 4 pregnant women, typically emerging in the second or third trimester and resolving after delivery. Iron deficiency, family history, and multiparity are consistent risk factors, with transient RLS conferring increased long-term risk of chronic disease. Management prioritizes nonpharmacologic measures and iron repletion, reserving medication for severe, refractory cases. Oral or intravenous iron remains the safest and most effective therapy, while dopamine agonists, gabapentinoids, opioids, and benzodiazepines are last-line options. Collaborative

Drop and Swell: Unanticipated Corneal Edema From Netarsudil Therapy.

Neovascular glaucoma (NVG) poses significant treatment challenges, often necessitating complex medication regimens to manage intraocular pressure (IOP). This report details a 65-year-old male with NVG secondary to central retinal artery occlusion (CRAO), who developed corneal epithelial bullae as a rare side effect of topical netarsudil therapy. Despite this complication, a continued treatment led to the gradual resolution of corneal lesions while maintaining controlled IOP. This case underscores the importance of vigilant monitoring for adverse effects and highlights the need for further rese

Australia and New Zealand Transplant and Cellular Therapies (ANZTCT) position statement: COVID-19 management in patients with haemopoietic stem cell transplant and chimeric antigen receptor T cell.

Patients with post-haemopoietic stem cell transplant or chimeric antigen receptor T -cell (CAR-T) therapy face a significant risk of morbidity and mortality from coronavirus disease 2019 because of their immunosuppressed state. As case numbers in Australia and New Zealand continue to rise, guidance on management in this high-risk population is needed. Whilst we have learned much from international colleagues who faced high infection rates early in the pandemic, guidance relevant to local health system structures, medication availability and emerging therapies is essential to equip physicians t

Alopecia areata and isotretinoin; coincidence or causal relation.

Isotretinoin is a safe and useful medication for acne management. Adverse effects are mostly related to cutaneous and mucous membranes. We present a case of a 24-year-old female patient who was on treatment with isotretinoin for acne vulgaris Grade 4. After 6 months of treatment, the patient presented with localized patch of hair loss. Beginning of alopecia areata in patients with acne vulgaris could be regarded as a side effect of retinoids due to anti-acne therapy. The exact mechanism by which retinoids cause hair loss is not known.

[Surgical and non-surgical management of primary hyperparathyroidism:How do calcimimetics work?].

Primary hyperparathyroidism is a common endocrine disease. The first line therapy for the disease is surgical removal of affected parathyroid gland(s). Other therapeutic options with medication are needed to be established, because many of patients with primary hyperparathyroidism have few or no symptoms and are expected to have a long life expectancy without surgery. Cinacalcet as a calcimimetic, bisphosphonates and denosumab are promising candidates for medical management of the disease. Effectiveness and efficiency of these drugs for patients with primary hyperparathyroidism is to be evalua

Cervical spine pain in the competitive athlete.

Cervical pain is a common complaint in both the well-conditioned athlete and the weekend warrior. Some injuries are mild in nature, responding to conservative treatment, including rest, medication, physical therapy, and time. However, more serious injuries, especially those involving the cervical spine, can have devastating consequences. Having a comprehensive understanding of the evaluation and management of cervical pain and cervical spine emergencies is crucial for physicians providing coverage for organized athletic events or for those who serve as team physicians. This article reviews the

Patterns of care for dysphagic patients with degenerative neurological diseases.

Several of the most common neurodegenerative conditions associated with dysphagia are Parkinsons's disease, progressive supranuclear palsy, postpolio syndrome, and amyotrophic lateral sclerosis. The best clinical practices for treating dysphagic patients with these common conditions are discussed in relation to medication, surgery, diagnosis, and clinical management. Best practice patterns are explained as they relate to the following concepts of care, including comprehension and cognition; interaction with caregivers; dependence-independence during therapy; matching treatment plans to changes

Surgical Considerations in the Treatment of Small Bowel Crohn's Disease.

Surgery remains a cornerstone of the management of Crohn's disease (CD). Despite the rise of biologic therapy, most CD patients require surgery for penetrating, obstructing, or malignant complications. Optimal surgical therapy requires sophisticated operative judgment and medical optimization. Intraoperatively, surgeons must balance treatment of CD complications against bowel preservation and functional outcome. This demands mastery of multiple techniques for anastomosis and strictureplasty, accurate assessment of bowel integrity for margin minimization, and a comprehensive skillset for naviga

Optimizing Kidney Disease Management during Ramadan: Evidence-Based Risk Stratification and Tailored Strategies.

Fasting during Ramadan poses distinct challenges for individuals with kidney disease, requiring a careful balance between spiritual commitments and health maintenance. This article combines insights from recent research and guidelines to offer a thorough framework for risk assessment. Patients are classified into low, moderate, and high-risk categories based on individual, environmental, and health-related factors. Management approaches are organized into four main areas: spiritual support, non-drug interventions, medication adjustments, and modifications to kidney replacement therapy (KRT). E

Medication-related osteonecrosis of the jaw (MRONJ): a review of pathogenesis hypothesis and therapy strategies.

Medication-related osteonecrosis of the jaw (MRONJ), a severe side effect caused by antiresorptive antiangiogenic medication, particularly bisphosphonates (BPs), has become a challenging disease with serious and profound effects on the physical and mental health of patients. Although it occurs with high frequency and is harmful, the exact mechanism of MRONJ remains unknown, and systematic and targeted approaches are still lacking. Maxillofacial surgeons focus on the etiology of osteonecrosis in the mandible and maxilla as well as the appropriate oral interventions for high-risk patients. Adequ

[Current Interventional Management of Acute Upper Gastrointestinal Bleeding].

Upper gastrointestinal bleeding (UGIB), a common medical emergency, causes significant morbidity and mortality. Endoscopic evaluation and treatment remain the standard care in patients who can be hemodynamically stabilized. However, severe bleeding despite conservative medication treatment or medication combined with endoscopic intervention occurs in 5%-10% of patients, requiring interventional or surgical treatment. Endovascular embolization has emerged as an alternative to emergency operative intervention for high-risk patients with non-variceal UGIB and is now commonly considered the first-
