Trends in childrens' ability to consent to a dental examination and the potential impact on reported caries indices.

UNLABELLED: New guidance on consent for England and Wales suggests that children aged over 11 should be asked to consent to the NHS child dental survey examinations. If they are "Gillick competent" then they can provide consent. Whether they are "Gillick competent" is a matter of clinical judgment of the examining dentist. This paper explores the level of understanding expressed after the examination by children apparently "Gillick competent". It considers issues how a dentist judges a child competent to make a decision to participate in a dental survey. OBJECTIVE: The objective of this inves

Severe self-neglect: an epidemiological and historical perspective.

Older adults with severe self-neglect have multiple deficits in various social, functional and physical domains, and often live insqualor. These individuals often present with poor personal hygiene, domestic squalor and hoarding which results in a threat to their own health as well as personal and public safety. Severe self-neglect occurs along a continuum with older adults often having cognitive and affective disorders compared with younger individuals presenting with psychiatric illnesses. In cases of severe self-neglect with hoarding, evidence has shown this behavior occurs in diverse socia

[Advance refusal of treatment in case of loss of autonomy due to persistent brain disease].

A short overview is given of the current debate on ethics and legal clarification of the range and binding force of so-called living wills demanding interruption of treatment in case of loss of autonomy due to persistent or progressive brain disease. Using the examples of dementia and persistent vegetative states - conditions with growing significance for psychiatrists - the binding force of living wills is examined for cases in which the irreversibility and extent of consciousness loss cannot be predicted with certainty. The range of living wills' authority appears also unclear. Legal proposa

Comparison of written examinations required for dental licensure in Japan and the United States: contents, cognitive levels, and cultural implications.

The goal of this article is to describe differences between the written examinations required for dental licensure in Japan and in the United States. Candidate guides, old exams, and other published data pertaining to the Japan National Examination for Dentists (NED) and the U.S. National Board Dental Examination (NBDE) were used. Dental education in Japan consists of a six-year program following high school graduation. The U.S. counterpart is usually a four-year program following a bachelor's degree. The NED, which is made up of 330 questions, is a single, all-important gatekeeper test that i

The capacity of people with a 'mental disability' to make a health care decision.

BACKGROUND: Based on the developing clinical and legal literature, and using the framework adopted in draft legislation, capacity to make a valid decision about a clinically required blood test was investigated in three groups of people with a 'mental disability' (i.e. mental illness (chronic schizophrenia), 'learning disability' ('mental retardation', or intellectual or developmental disability), or, dementia) and a fourth, comparison group. METHODS: The three 'mental disability' groups (N = 20 in the 'learning disability' group, N = 21 in each of the other two groups) were recruited through

Timing of exposure to household poverty and adolescent mental health problems.

AIMS: Mental health problems in adolescence are increasingly prevalent and have tremendous impacts on life-long health and mortality. Although household poverty is a known risk factor for adolescent mental health, evidence of the timing hypothesis is scarce. We aimed to examine the longitudinal associations of poverty across childhood with mental health in adolescence, focusing on the timing of exposure. METHODS: We used the data of 5,671 children from a Japanese population-based longitudinal cohort, which recruited the first graders (aged 6-7 years) and followed biannually until eighth grade

How different domains of quality of life are associated with latent dimensions of mental health measured by GHQ-12.

BACKGROUND AND OBJECTIVES: A short form of the General Health Questionnaire (GHQ-12) is a useful screening instrument for assessing mental health. Furthermore, Quality of life (QoL) is a critical treatment outcome in many clinical and health care research settings. This study aimed to reassess the dimensionality of GHQ-12 using Multidimensional Graded Response Model (MGRM) and evaluate how its extracted dimensions are associated with the QoL's domains. METHODS: Isfahan Cohort Study 2 (ICS2) is a population-based, ongoing prospective cohort study among adults aged 35 years and older who were f

Sense of coherence and burnout in nursing home workers during the COVID-19 pandemic in Spain.

Care staff in nursing homes work in a challenging environment, and the COVID-19 pandemic has exacerbated those challenges in an unprecedented way. On the other hand, the sense of coherence (SOC) is a competence that could help these professionals perceive the situation as understandable, manageable and meaningful. This study aims to analyse the extent to which potential risk and protective factors against burnout have affected nursing home workers during the COVID-19 pandemic and to assess the contribution of these factors to their burnout. Three hundred forty professionals who worked in nursi

Barriers, attitudes, confidence, and knowledge of nurses regarding metabolic health screening and intervention in people with mental illness: a pilot study from Uganda.

BACKGROUND: People with mental illness are at an increased risk for developing cardio-metabolic disorders. Routine screening following pharmacotherapy is however unacceptably low in sub-Saharan African countries with less than 1% adequately screened. It is unknown whether this is due to a lack of adequate competences. OBJECTIVES: The aim of this pilot study was to assess the barriers, attitudes, confidence, and knowledge of nurses regarding metabolic health, prevention and treatment in Uganda. METHODS: Twenty-eight nurses (39% female, 30.9±6.9 years) completed the Metabolic - Barriers, Confi

Improving access to psychological therapies in voice disorders: a cognitive behavioural therapy model.

PURPOSE OF REVIEW: The improving access to psychological therapies initiative has highlighted the importance of managing mental health problems effectively, and research has shown excellent outcomes from cognitive behavioural therapy (CBT) interventions. Patients presenting with functional dysphonia will often also describe psychological distress including anxiety, depression and reduced general well-being, and it is felt that effective voice therapy needs to include the management of psychological well-being. The evidence for the use of CBT enhanced voice therapy is limited to date. Recent re

The acceptability and feasibility of task-sharing for mental healthcare in low and middle income countries: a systematic review.

Task-sharing has frequently been proposed as a strategy to overcome human resource shortages in order to scale up mental health care. Although evidence suggests this approach is effective, to date no review has been conducted to assess its acceptability and feasibility among service users and health care practitioners. This review summarises current findings and provides evidence-based recommendations to improve the success and sustainability of task-sharing approaches. All study designs were included and both qualitative and quantitative data were extracted and reviewed using a comparative th

The self-directed patient in guided internet-delivered CBT: a qualitative study of patient role expectations.

BACKGROUND: Therapist-guided internet-delivered cognitive behavioral therapy (guided iCBT) is an effective and scalable treatment for anxiety and depression. Central to this format is the expectation that patients work independently with therapeutic content. However, limited attention has been paid to how healthcare professionals and patients within guided iCBT understand the patient role. OBJECTIVE: This study explores how healthcare professionals and patients understand the patient role in guided iCBT, using self-determination theory to examine how autonomy, competence, and relatedness shap

Pre-operative evaluation of the volume of bone graft in sinus lifts by means of CompuDent.

OBJECTIVES: Adolescent-onset type 2 diabetes (T2D) is increasing, and depression symptoms amplify T2D risk, highlighting the need for interventions addressing mental and metabolic health. This multisite pilot RCT assessed whether facilitators could be trained consistently to deliver group MBI, cognitive behavioral therapy (CBT), and a matched health education condition (HE) to adolescents at risk for T2D with elevated depression symptoms. It was hypothesized that facilitators would achieve ≥ 80% on knowledge, competence, and adherence following standardized training. METHOD: Facilitators at f

The role of psychological factors and food in improving athletic performance: an analytical study of a specific sports organization's society.

BACKGROUND: Athletic performance reflects the interplay of physical, psychological, and nutritional factors. Although the individual roles of mental and dietary factors are increasingly recognized, their combined relationship with performance within a defined sports organization has rarely been examined. OBJECTIVE: This cross-sectional study examined the associations of psychological factors (motivation, anxiety, mental resilience, self-confidence) and self-reported nutritional practices with self-perceived athletic performance among athletes affiliated with a sports organization in Rizhao, S

Review of the Characteristics of School-Based Integrated Mental Health and Substance Use Prevention Programs for Adolescents Evaluated Through Clinical Trials Between 2016 and 2026.

BACKGROUND/OBJECTIVES: Adolescent substance use and mental health problems frequently co-occur and share common risk and protective factors. Some school-based prevention programs address substance use and mental health within an integrated approach. However, the characteristics and intervention components of these integrated approaches remain insufficiently understood. This review aimed to describe the program characteristics of school-based integrated mental health and substance use prevention programs for adolescents that were evaluated in randomized or cluster randomized controlled trials (
