Nontuberculous mycobacterial myositis in dermatomyositis with long-term use of immunosuppressant: a case report.

Inflammatory myopathies, such as polymyositis and dermatomyositis, are systemic inflammatory disorders that affect skeletal muscles and inte … e of immunosuppressants and developed a disseminated soft tissue infection in both thighs caused by Mycobacterium abscessus. Multimodal images were obtained using magnetic resonance imaging, ultrasonography, and computed tomography. A strong suspicion of possible combined opportunistic infections and appropriate staining is essential in diagnosing nontuberculous mycobact …

Bactericidal, fungicidal, and antiviral activity of highly purified hypochlorous acid water.

This study demonstrates that high-purity hypochlorous acid water (HP-HAW) produced via reverse osmosis (RO) membrane filtration exhibits rapid, broad-spectrum antimicrobial and antiviral activity, comparable to that of hypochlorous acid water (HAW) prepared without RO membrane treatment. Common vegetative bacteria, dermatophytes, and Candida albicans were completely inactivated within 10 s, indicating excellent performance for routine disinfection. HP-HAW also showed moderate efficacy against Mycobacterium abscessus but limited activity against M. avium, M. kansasii, and spore-forming bacteria…

Cell senescence emerges as a hallmark and therapeutic target of chronic intracellular infection.

Intracellular pathogens are ideal candidates for modelling the pathophysiology of chronic infection, as they hide within host cells and avoid immune clearance by reshaping cellular fate. This study unveils how the intracellular pathogen Mycobacterium abscessus (Mab) reprograms alveolar macrophages towards a senescent state -a multifaced phenotype marked by proliferative arrest, distinctive morphological shifts, activation of DNA damage signalling cascade, and secretion of senescence-associated secretory phenotype (SASP) factors. Mab-induced DNA damage emerged as the primary driver of senescenc…

Th2 bias and T-cell exhaustion characterize the immunopathology of non-tuberculous mycobacterial pulmonary disease.

Non-tuberculous mycobacterial pulmonary disease (NTM-PD) is an escalating global health concern with poorly defined immunological mechanisms, necessitating comprehensive profiling to guide therapeutic advances. We analyzed peripheral blood from 28 treatment-naïve NTM-PD patients (19 Mycobacterium avium complex, 9 Mycobacterium abscessus) and 27 matched controls using 42-marker mass cytometry (CyTOF) and Luminex multiplex assays. A random forest model identified predictive markers, while an in vitro murine macrophage model evaluated chemokine production. NTM-PD patients displayed significant im…

BCG Vaccination Induces M. avium and M. abscessus Cross-Protective Immunity.

Pulmonary non-tuberculous mycobacterial (NTM) infections particularly caused by Mycobacterium avium complex (MAC) and Mycobacterium abscessus (MAB) are becoming major health problems in the U.S. New therapies or vaccines which will help prevent the disease, shorten treatment duration and/or increase treatment success rates are urgently needed. This study was conducted with the objective of testing the hypothesis that Bacillus Calmette Guerin (BCG), a vaccine used for prevention of serious forms of tuberculosis (TB) in children and adolescents in tuberculosis hyperendemic countries, induces cro…

β-Lactam Combinations That Exhibit Synergy against Mycobacteroides abscessus Clinical Isolates.

Mycobacteroides abscessus (Mab) is an opportunistic environmental pathogen that can cause chronic pulmonary disease in the setting of structural lung conditions such as bronchiectasis, chronic obstructive pulmonary disease, and cystic fibrosis. These infections are often incurable and associated with rapid lung function decline. Mab is naturally resistant to most of the antibiotics available today, and current treatment guidelines require at least 1 year of daily multidrug therapy, which is often ineffective and is associated with significant toxicities. β-Lactams are the most widely used clas

Mycobacterium salmoniphilum and M. chelonae in Captive Populations of Chinook Salmon.

Chinook Salmon Oncorhynchus tshawytscha is a keystone fish species in the Pacific Northwest. In 2019, unusual mortalities occurred in two different populations of cultured fingerlings from the same facility in California, USA. The systems consist of outdoor, enclosed, flow-through freshwater tanks that are maintained at 18 ± 1°C. Clinical signs and gross findings were only observed in one population and included abnormal swimming, inappetence, lethargy, skin discoloration, and the presence of multifocal nodular and ulcerative skin lesions. Microscopic lesions were infrequent and consisted of s

In vitro and in vivo antibacterial activity of pravibismane against Mycobacteroides abscessus.

The increasing prevalence of non-tuberculous mycobacterial lung infection represents a global challenge. Infections caused by Mycobacteroides abscessus commonly occur in individuals with underlying predisposing lung diseases such as cystic fibrosis (CF), non-CF-bronchiectasis, chronic obstructive pulmonary disease (COPD), and tuberculosis (TB). The unmet need in the treatment of M. abscessus pulmonary infection is driven by multidrug-resistance, ability to form biofilms, lengthy treatment, and drug-related side effects. Pravibismane was evaluated for its potential to reduce M. abscessus burden

Evaluation of clinical usefulness of a commercial multiplex PCR for diagnosing mycobacterial infections: a multicenter study.

UNLABELLED: Rapid and accurate differentiation between Mycobacterium tuberculosis complex (MTBC) and clinically relevant nontuberculous mycobacteria (NTM) is essential for appropriate patient management but remains challenging with conventional diagnostic methods. We evaluated the clinical performance of a commercial multiplex real-time PCR assay for the direct detection of MTBC, Mycobacterium avium complex (MAC), and Mycobacterium abscessus complex (MABSC) in respiratory specimens. In this prospective multicenter study, 845 respiratory samples from patients with suspected mycobacterial infect…

Nocardia cyriacigeorgica Identified by Bronchial Washing During Evaluation of Radiologic Progression in a Patient With Prior Nontuberculous Mycobacterial Diseases.

Pulmonary nocardiosis may occur in immunocompetent patients with structural lung disease and may resemble the progression of nontuberculous mycobacterial pulmonary disease. We report an 81-year-old immunocompetent woman with chronic bronchiectasis and prior Mycobacterium kansasii, M. intracellulare, and M. abscessus infections. Despite persistently negative sputum mycobacterial cultures, chest computed tomography (CT) revealed progressive nodular bronchiectatic changes. Bronchial washing from the right B5 bronchus yielded Nocardia cyriacigeorgica; mycobacterial studies were negative. Trimethop

Epidemiology of Nontuberculous Mycobacterial Infection in Tuberculosis Suspects.

Nontuberculosis mycobacteria (NTM) are opportunistic pathogens that cause a wide range of illnesses. Here, the species distribution and prevalence of NTM infections in tuberculosis suspects was analyzed. A total of 7,073 specimens from pulmonary and extrapulmonary sites were analyzed, and 709 (10%) were found to be culture positive for mycobacteria. Of these, 85.2% were identified as Mycobacterium tuberculosis complex and 14.8% as NTM (65.7% rapid growers and 34.3% slow growers). Speciation of the NTM isolates (n = 69) identified 19 NTM species.
