INTRODUCTION: Cocktail therapy consists of extracorporeal shock wave treatment (ESWT), hyperbaric oxygen therapy (HBO) and alendronate. We hypothesized that this combined therapy may produce better results than ESWT alone when treating hip necrosis.

OBJECTIVE: To analyze the feature of cortical laminar necrosis (CLN) and to decrease the incidence of misdiagnosis and improve the treatment of CLN.

We report a case report of extensive eyelid necrosis caused by an insect bite. A 39-year-old man had suffered ocular trauma from a hymenopteran insect in Tunisia. The patient presented with eyelid necrosis. Eyelid necrosis mechanisms following insect bites are discussed and other causes of eyelid necrosis are presented. Eyelid necrosis, often caused by vasoconstrictive medications or animal venoms, notably hymenopteran insect bites, can result in substantial eyelid injury.

Subcutaneous fat necrosis of the newborn (SFN) is an uncommon, self-limited condition of neonates and infants. However, hypercalcemia may be a potentially life-threatening complication of the otherwise self-healing process. Rare complications such as thrombocytopenia, hypertriglyceridemia, hypoglycemia, calcium deposits in the liver, inferior vena cava, atrial septum of the heart, and nephrocalcinosis have been reported. To our knowledge, this is the first description of a patient in whom lactic acidosis was encountered in an infant along with the other known complications of SFN.

We reported a 67-year-old woman with unilateral laminar necrosis caused by hypoxic encephalopathy. She presented with sudden onset of dyspnea, unconsciousness and left hemiplegia. Subcutaneous fat necrosis, though a rare phenomenon, can occur in patients of all ages. In newborns, it is an uncommon, self-limited panniculitis of traumatic origin, while in adults it is most commonly encountered among women presenting to breast clinics following a trivial or unnoticed injury sustained long ago. An acute presentation of fat necrosis as a large mass, however, is quite unusual and that may confuse the observer. History of trauma gives a clue to the diagnosis. We report an interesting case of traumatic subcutaneous fat necrosis because of massage therapy in a 66-year-old lady.

PURPOSE OF THE STUDY: Core decompression of the femoral head is a conservative surgical treatment with controversial efficacy. We studied retrospectively a series of 32 cases of femoral head osteonecrosis treated by core decompression between 1988 and 2000 in 25 patients. Corticosteroids are an important part of the pharmacological armamentarium against a wide spectrum of diseases. They are powerful drugs that effect all aspects of human metabolism and, although often life-saving, they have a plethora of important side-effects and a narrow therapeutic window. Most side-effects are well known to physicians but we would like to highlight the problem of avascular necrosis associated with cyclical steroid therapy of short duration using moderate doses for an unusual indication.

Although infrequent, recovery of pituitary function after necrosis of a pituitary adenoma is not an exceptional event. We report the case of a 32-year-old woman with previous surgery for an adrenal mass and signs of hypercortisolism which failed to revert postoperatively.
