BACKGROUND: Early-onset neonatal sepsis (EOS) is a rare condition but an important cause of severe morbidity and mortality in neonates.
METHODS: This is a prospective observational study in neonates born at ≥34 weeks of gestation (WG). The primary endpoint was EOS, defined by isolation of pathogenic species from blood culture and/or cerebrospinal fluid culture within 72 hours after birth. Data on EOS were collected exhaustively from all maternity wards in Paris area (April 2019-March 2021).
RESULTS: 108 EOS were recorded (annual incidence, 0.32 per 1000 live births; 95% CI 0.26 to 0.38). In …

Chorioamnionitis is an acute inflammation of the membranes and chorion of the placenta typically due to ascending polymicrobial infection in the setting of membrane rupture. It is a common complication of pregnancy associated with significant maternal, perinatal, and long-term adverse outcomes. We present a case of placental infection leading to preterm delivery, severe neonatal sepsis, maternal wound infection, postnatal readmission, and prolonged hospital stay. This virulent infection was caused by multidrug-resistant extended-spectrum beta-lactamase (ESBL)-producing Escherichia Coli (E. Col…

We report 4 cases of neonatal sepsis caused by Streptococcus gallolyticus. The clinical course was quite similar to early-and late-onset group B streptococcus disease. None of the mothers had group B streptococcus (GBS) colonization on prenatal screening nor received intrapartum antibiotics. We proposed the sporadic distribution of S. gallolyticus sepsis among neonates was partly due to relatively low colonization rate in adults compared with GBS. Species determination of S. gallolyticus may not be available using conventional microbiological methods and may contribute to underestimation or mi…

BACKGROUND: Neonatal sepsis and the associated myocardial dysfunction remain a leading cause of infant mortality. Extracellular cold-inducible RNA-binding protein (eCIRP) acts as a ligand of triggering receptor expressed on myeloid cells-1 (TREM-1). M3 is a small CIRP-derived peptide that inhibits the eCIRP/TREM-1 interaction. We hypothesize that the eCIRP/TREM-1 interaction in cardiomyocytes contributes to sepsis-induced cardiac dysfunction in neonatal sepsis, while M3 is cardioprotective.
METHODS: Serum was collected from neonates in the Neonatal Intensive Care Unit (NICU). 5-7-day old C57B…

INTRODUCTION: To identify the risks and outcomes for multidrug-resistant Gram-negative bacilli (MDRGNB) sepsis in neonates.
METHODOLOGY: This was a retrospective case-case-control study between 1991 and 2016. The control group was selected from the same source records of all neonates with clinical or suspected sepsis but not culture-proven.
RESULTS: The numbers of patients in the MDRGNB sepsis, non-MDRGNB sepsis, and control groups were 157, 88, and 218, respectively. MDRGNB sepsis was significantly associated with outborn infants [adjusted odds ratio (aOR) 2.08; p = 0.003] and infants who h

Molecular epidemiology of Staphylococcus aureus strains causing bacteremia in neonates during 2002 to 2005 revealed seven clones, with four MSSA clones responsible for 80% of the cases. Some clones persisted or reappeared throughout the study. Three bacteremic clones were found colonizing health care workers (HCWs), particularly clone C, which was harbored by at least 15% of HCWs.

Many questions remain unanswered (and probably many more remain unasked) about neonatal sepsis. In addition, many mediators of inflammation have been implicated in neonatal sepsis at one time or another. The goal of this article is to put together what the authors view as some of the more important outstanding questions with what is known about some of the more important mediators. We address four questions specifically: (1) Why is neonatal sepsis different from all other (adult) sepsis? (2) 41Is control of pulmonary hypertension the white whale or red herring in neonatal septic shock? (3) Is …

BACKGROUND: Neonatal sepsis is the significant cause of morbidity and mortality in low- and middle-income countries (LMICs) such as Tanzania, where 99% of global neonatal deaths occur. Epidemiological data regarding the causative agents and their resistance profiles can guide clinical practice and prevention strategies. A systematic review and meta-analysis was conducted to determine the prevalence, pathogens and the outcome of neonatal sepsis in Tanzania. METHODS: We conducted a systematic literature search using online databases such as PubMed/ Medline, Embase, Cochrane, Google Scholar, and …

OBJECTIVES: Neonatal sepsis in one twin poses a dilemma in managing the healthy-appearing sibling. Shared exposures and genetic factors increase risk, and early signs of sepsis can be subtle with rapid deterioration. However, the magnitude of this risk for the healthy twin is unclear, and management strategies vary considerably.
METHODS: We systematically searched PubMed and EMBASE on June 10th 2025 for multiples in which at least one infant (0-89 days) developed culture-confirmed Group B Streptococcus or Escherichia coli early-onset or late-onset sepsis or meningitis.
RESULTS: Of 151 sets o…

INTRODUCTION: Neonatal sepsis causes significant morbidity and mortality worldwide but is difficult to diagnose clinically. Clinical prediction models (CPMs) could improve diagnostic accuracy, facilitating earlier treatment for cases and avoiding antibiotic overuse. Neonates in low-income and middle-income countries (LMICs) are disproportionately affected by sepsis, yet no review has comprehensively synthesised evidence for CPMs validated in this setting.
METHODS: We performed a scoping review of CPMs to diagnose neonatal sepsis using Ovid MEDLINE, Ovid Embase, Scopus, Web of Science, Global …

BACKGROUND: In some cases, neonates with pneumonia can develop sepsis. The timely and accurate diagnosis of sepsis due to neonatal pneumonia poses a challenge for many clinicians. Therefore, this study aimed to evaluate the potential of the platelet-to-neutrophil ratio (PNR), as an early biomarker for clinically differentiating neonatal sepsis among pneumonia cases.
METHODS: We performed a cross-sectional analysis of clinical and laboratory data collected from 1,103 patients with neonatal pneumonia [773 with pneumonia and 330 with pneumonia and sepsis (PS)].
RESULTS: The PS group had a lower…
