The incidence of extrahepatic gastrointestinal metastases from breast cancer is reported in the literature only as necroscopy studies (6-18 percent); they usually originate from lobular or a mixed ductal-lobular subtype. Nonspecific presenting symptoms, death of the patients caused by other more frequent metastases, and variable radiographic features mimicking primary neoplasms cause a clinical underestimation of this pathology. We report here a case of rectal metastasis from a lobular carcinoma eight years after mastectomy.

The vast majority of malignant melanomas in the small intestine are metastasis of cutaneous tumors. Few cases have been published on primary melanomas in this location, some authors consider that they are always metastatic and that the primary tumor has regressed. In this letter, we present the case of a 77-year-old woman with a history of cutaneous melanoma excision 38 years ago who was diagnosed with ileal melanoma in the absence of other lesions during the study of iron deficiency anemia, and we discuss about the origin of this type of neoplasms.

Carotid body tumors, although representing about 65% of head and neck paragangliomas, are rare neoplasms. Doppler ultrasonography, contrast-enhanced computed tomography (CECT), and MRI have been routinely used in the diagnosis of carotid body tumor. We hereby represent a case of 56-year-old male suspected to have lymph node metastasis from carcinoma pyriform sinus detected as carotid body tumor in fluorodeoxyglucose positron-emission tomography-CECT imaging and subsequently confirmed by histopathology.

Adenoid cystic carcinoma (ACC) is a relatively rare epithelial tumor of the salivary glands that accounts for approximately 5 to 10% of all salivary gland neoplasms. The typical clinical and pathological findings of this tumor include slow indolent growth, common local recurrence, and late distant metastasis to lung, brain, bone, liver, thyroid, and spleen. Paratesticularleiomyosarcomas are rare neoplasms. Radical orchidectomy and high ligation of cord followed by surveillance is the treatment of choice. Here we report a 73-year-old man who presented with a 2-year history of painless lump in the left hemiscrotum. A left radical orchiectomy with high ligation of the spermatic cord was performed. The histo-pathological and immunohistochemistry evaluation revealed a moderately differentiated leiomyosarcoma of paratesticular region. The patient is still alive with no evidence of local recurrence and distant metastasis after seven years of follow-up.

Altered biodistribution of Tc-99m methylene diphosphonate is a common occurrence on bone scintigraphy and mucin-producing neoplasms of the gastrointestinal tract are associated with dystrophic uptake. In addition to the uptake in the primary tumor, activity has been reported in metastasis within lymph nodes and liver. We present a case of Tc-99m methylene diphosphonate uptake in peritoneal carcinomatosis and metastatic Krukenberg tumors from a primary mucinous adenocarcinoma of the descending colon.
