Automated classification of cells in sub-epithelial connective tissue of oral sub-mucous fibrosis-an SVM based approach.

Quantitative evaluation of histopathological features is not only vital for precise characterization of any precancerous condition but also crucial in developing automated computer aided diagnostic system. In this study segmentation and classification of sub-epithelial connective tissue (SECT) cells except endothelial cells in oral mucosa of normal and OSF conditions has been reported. Segmentation has been carried out using multi-level thresholding and subsequently the cell population has been classified using support vector machine (SVM) based classifier. Moreover, the geometric features use

Connective tissue changes and mast cell variations in benign and malignant lesions of the uterine cervix.

We have studied mast cell variations and stromal changes in 100 benign and 100 malignant lesions of the uterine cervix. Mast cells were found in close proximity to the cervical glands or young proliferating fibroblasts. Comparison of mast cell densities in benign and malignant conditions revealed an increase in inflammatory processes, while in cancers there is diminution in number or total absence of mast cells. There is no conclusive correlation between the age of the patients and the density of these cells. An inverse relationship existed between the mast cell population and degree of anapla

Radiofrequency ablation in tumour-induced osteomalacia: a therapeutic challenge.

Tumour-induced osteomalacia (TIO) is an uncommon, debilitating disorder often characterised by non-specific clinical manifestations, posing a significant diagnostic challenge. The tumours causing TIO can be minuscule and occur in unusual areas, further complicating diagnosis. This report details the case of a woman in her early 30s presenting with chronic pain who subsequently developed fragility fractures. The evaluation revealed hypophosphataemia with evidence of renal phosphate wasting demonstrated by a low ratio of tubular maximum reabsorption of phosphate to the glomerular filtration rate

Primary Renal Mesenchymal Neoplasms in Nephrectomy Specimens: A 16-Year Survey of Primary and Incidental Cases From a Single Institution.

Context. Primary renal mesenchymal neoplasms are rare but diverse. Several neoplasms of mesenchymal derivation can arise as a primary renal mass; however, incidental tumors are identified in nephrectomies performed for end-stage renal disease or other malignant epithelial renal neoplasms. In this article, we document their frequencies with demographic information in nephrectomy specimens. Design. We retrospectively searched our database for nephrectomy specimens from the year 2000 to 2016. Results. A total of 224 cases were identified with an incidence rate of 4.4% (224/5128). One hundred nine

Mesenchymal stem cell-derived CCN2 promotes the proliferation, migration and invasion of human tongue squamous cell carcinoma cells.

Recent studies have demonstrated that mesenchymal stem cells (MSC) exhibit a tropism to tumors and form the tumor stroma. In addition, we found that MSC can secrete different types of factors. However, the involvement of MSC-derived factors in human tongue squamous cell carcinoma (TSCC) growth has not been clearly addressed. The CCN family includes multifunctional signaling molecules that affect the initiation and development events of various tumors. In our study, we report that CCN2/connective tissue growth factor (CTGF) was the most highly induced among the CCN family members in MSC that we

Coexistence of tumor-induced osteomalacia and primary hyperparathyroidism.

OBJECTIVE: To present an unusual case of coexisting tumor-induced osteomalacia (TIO) and primary hyperparathyroidism (PHPT). METHODS: We report the clinical features, imaging studies, and the results of laboratory investigations before and after surgical resection of both a soft-tissue tumor and a parathyroid adenoma. RESULTS: A 44-year-old woman was referred to the endocrinology department with a diagnosis of PHPT accompanied by unusually severe hypophosphatemia, despite having received treatment with cinacalcet. Debilitating muscle weakness and bone pain, severe phosphaturia and hypophosph

Tumor-induced osteomalacia with elevated fibroblast growth factor 23: a case of phosphaturic mesenchymal tumor mixed with connective tissue variants and review of the literature.

Tumor-induced osteomalacia (TIO), or oncogenic osteomalacia (OOM), is a rare acquired paraneoplastic disease characterized by renal phosphate wasting and hypophosphatemia. Recent evidence shows that tumor-overexpressed fibroblast growth factor 23 (FGF23) is responsible for the hypophosphatemia and osteomalacia. The tumors associated with TIO are usually phosphaturic mesenchymal tumor mixed connective tissue variants (PMTMCT). Surgical removal of the responsible tumors is clinically essential for the treatment of TIO. However, identifying the responsible tumors is often difficult. Here, we repo

Pulmonary lymphangioleiomyomatosis and renal angiomyolipoma in a patient with systemic lupus erythematosus: A case report.

BACKGROUND: The co-incidence of systemic lupus erythematosus (SLE) and tuberous sclerosis with pulmonary lymphangioleiomyomatosis (LAM) and renal angiomyolipoma (AML) is rare. In such patients, the rupture of renal AML may result in fatal circumstances, but this may be preventable. METHODS: A 22-year-old Asian woman with SLE was admitted to our hospital with severe left-flank pain. Imaging studies showed the bilateral rupture of multiple renal AMLs. RESULTS: The patient underwent emergency selective transcatheter embolization (TE) of the left renal artery. After TE and massive hydration, the

Prognostic biomarkers for enhanced risk stratification in extraskeletal myxoid chondrosarcoma: a retrospective cohort study.

BACKGROUND: Extraskeletal myxoid chondrosarcoma (EMC) is a rare soft-tissue sarcoma driven by the EWSR1::NR4A3 fusion. Its unpredictable clinical course and lack of validated molecular prognostic tools complicate patient treatment. METHODS: We performed whole-transcriptome targeted RNA sequencing (TempO-Seq) on archival formalin-fixed paraffin-embedded (FFPE) tumor specimens from 12 molecularly confirmed EMC cases. To explore potential biological markers, the cohort was stratified into good-prognosis (n = 6; overall survival (OS) >8 years) and poor-prognosis (n = 6; OS < 8 years) groups. We c

Recent advances in fibroblast growth factor 23-related hypophosphatemic disorders.

PURPOSE OF REVIEW: Fibroblast growth factor 23 (FGF23) is a hormone to reduce blood phosphate concentration. Excessive actions of FGF23 induce FGF23-related hypophosphatemic disorders, such as X-linked hypophosphatemic rickets (XLH) and tumor-induced osteomalacia (TIO). We will summarize recent advances in the diagnosis and treatment of FGF23-related hypophosphatemic disorders. RECENT FINDINGS: The measurement of blood FGF23 is useful to make a diagnosis of FGF23-related hypophosphatemic disorders. It was reported that many patients with FGF23-related hypophosphatemic disorders, especially TI

Functional imaging in primary tumour-induced osteomalacia: relative performance of FDG PET/CT vs somatostatin receptor-based functional scans: a series of nine patients.

CONTEXT: Localization of phosphatonin-producing mesenchymal tumours in patients with primary tumour-induced osteomalacia (pTIO) is challenging. Functional imaging plays an important role in the localization of these tumours. OBJECTIVE: We studied the relative performance of different functional imaging modalities ((18) F-FDG PET/CT, (99) Tc-HYNIC-TOC SPECT/CT and (68) Ga-DOTATATE PET/CT) in tumour localization in cases of pTIO. DESIGN AND METHODS: Retrospective chart evaluation of 16 patients with confirmed TIO treated from 2006 to 2013 was conducted in a tertiary care referral centre. RESU

Hypophosphataemia-inducing mesenchymal tumour in the foot.

Tumour-induced (or oncogenic) osteomalacia is a paraneoplastic syndrome characterised by progressive fatigue, muscle weakness, bone pain, non-healing and recurrent fractures caused by mesenchymal tumours that secrete proteins that inhibit renal phosphate transport and 1α-hydroxylation of 25-OH-vitamin D. The potentially curative treatment of choice is complete surgical excision of the tumour.

Retrospective study of polymyositis in Auckland over 10 years.

Twenty-nine patients with polymyositis and dermatomyositis seen at the three main hospitals in Auckland between 1967 and 1977 have been reviewed. Of the five diagnostic criteria used, weakness of proximal muscles (including neck flexors) was the single most important clinical diagnostic feature. The certainty of diagnosis was increased if investigations included a range of muscle enzymes, electromyography and muscle biopsy. The incidence of underlying malignancy was 10 percent. Patients with polymyositis or dermatomyositis with an associated connective tissue disorder responded better to treat

Alterations of liver and spleen among workers exposed to vinyl chloride.

Preliminary review of hepatic biopsy and autopsy specimens obtained from workers engaged for prolonged periods in the polymerization of vinyl chloride indicates a fibrotic precursor lesion in the liver. It is the only lesion in some instances but also was found in the uninvolved liver of patients with angiosarcoma and in two instances in liver biopsy specimens obtained before angiosarcoma developed. This precursor stage is characterized by a conspicuous subcapsular fibrosis, a nonpathognomonic progressive portal fibrosis, and a borderline increase of intralobular connective tissue, all associa

Cancer risks among female farmers in Sweden.

There are few studies on cancer risks among female farmers, particularly on breast cancer and cancer in female genital organs. We have conducted a cohort study of cancer risk among 50,682 women with occupations in agriculture according to the Swedish 1970 census. The cohort was followed up in the nationwide, population-based, Cancer-Environment Register from 1 January 1971 until death or until 31 December 1987. Expected number of cases was based on annual cancer incidence in five-year age groups. The standardized incidence ratio (SIR) was computed as the ratio between observed and expected num

FGF23-secreting sinonasal tumour presenting with acute subdural haemorrhage and tumour-induced osteomalacia.

A female in her 50s developed a headache, collapsed and was noted to have an acute atraumatic subdural haemorrhage (SDH) requiring surgical evacuation and intracranial pressure-directed therapy. Her background included recurrent epistaxis, severe generalised bone pain and multiple insufficiency fractures and an undifferentiated autoimmune connective tissue disease. Chronic hypophosphataemia, elevated alkaline phosphatase and raised fibroblast growth factor 23 (FGF23) were also noted. An MRI head and subsequent 68Ga CT/positron emission tomography scan demonstrated an intensely avid tumour in t
