High frequency stimulation (HFS) is used to control abnormal neuronal activity associated with movement, seizure, and psychiatric disorders. Yet, the mechanisms of its therapeutic action are not known. Although experimental results have shown that HFS suppresses somatic activity, other data has suggested that HFS could generate excitation of axons. Moreover it is unclear what effect the stimulation has on tissue surrounding the stimulation electrode. Electrophysiological and computational modeling literature suggests that HFS can drive axons at the stimulus frequency. Therefore, we tested the

The question whether general tetanus arises from the independent sum of multiple local tetani or results from the actions of the transynaptic tetanus neurotoxin (TeNT) in higher brain centres remains unresolved. Despite the blood-borne dissemination of TeNT from an infected wound, the access to the central nervous system is probably prevented by the blood-brain barrier. However, several long-term sequelae (e.g. autonomic dysfunction, seizures, myoclonus, and sleep disturbances) present after the subsidence of muscle spasms might be indicative of central actions that occur farther away from low

Biological assay and imaging techniques have made visible a great deal of the machinery of mental illness. Over fifty years of investigation of mood disorders using these technologies has identified several biological regularities in these disorders. Here we present a narrative connecting genetic, cytokine, neurotransmitter, and neural-systems-level findings in major depressive disorder (MDD). Specifically, we connect recent genome-wide findings in MDD to metabolic and immunological disturbance in this disorder and then detail links between immunological abnormalities and dopaminergic signalin

Oligodendrocytes are one of the major cell types in cerebral white matter. Under normal conditions, they form myelin sheaths that encircle axons to support fast nerve conduction. Under conditions of cerebral ischemia, oligodendrocytes tend to die, resulting in white-matter dysfunction. Repair of white matter involves the ability of oligodendrocyte precursors to proliferate and mature. However, replacement of lost oligodendrocytes may not be the only mechanism for white-matter recovery. Emerging data now suggest that coordinated signaling between neural, glial, and vascular cells in the entire

BACKGROUND AND AIM: Late-onset Charcot-Marie-Tooth (CMT) disease remains under-recognized and may mimic acquired neuropathies in older adults. We aimed to determine the prevalence and clinical, electrophysiological, and genetic characteristics of genetically confirmed late-onset CMT and to identify features distinguishing it from late-onset non-CMT neuropathies.
METHODS: We retrospectively reviewed all patients who underwent next-generation sequencing (NGS) for suspected hereditary neuropathy between 2015 and 2025 at a single neuromuscular referral center. Late-onset CMT was defined by sympto

OBJECTIVE: IgM paraproteinemic neuropathies are traditionally considered large-fiber disorders, while small-fiber involvement remains poorly investigated. We assessed small-fiber dysfunction using thermal quantitative sensory testing in IgM paraproteinemic neuropathies, comparing anti-myelin-associated glycoprotein-positive and negative patients, and explored relationships with nerve conduction studies, disability, quality of life, and serum neurofilament light chain.
METHODS: Thirty-nine patients (24 anti-myelin-associated glycoprotein-positive, 15 negative) and 20 healthy volunteers underwe

In Fabry disease (FD) neuropathic pain is often the first symptom due to impairment of the small fibres of peripheral nerves. The problem of large fibre involvement in FD has not been described well. This prompted us to investigate electrophysiological changes in motor fibres with different conduction velocities in FD patients. The study group consisted of 20 patients with FD (8 women and 12 men); mean age was 40.65 ± 11.94 years, and a matched control group. Medical history, neurological examination, together with standard neurography and conduction velocity distribution (CVD) tests of the uln

BACKGROUND: Superficial radial neuropathy (SRN) is an uncommon sensory mononeuropathy, and data on neuropathic pain in SRN are limited. This study aimed to determine the frequency of neuropathic pain in patients with SRN and to evaluate its associations with neurophysiological severity.
METHODS: We retrospectively reviewed adult patients diagnosed with SRN. Neuropathic pain was evaluated using the Douleur Neuropathique 4 Questions (DN4) and the Leeds Assessment of Neuropathic Symptoms and Signs (LANSS). Neurophysiological severity was categorized as mild, moderate, or severe according to the

INTRODUCTION/AIMS: Triple A syndrome is an autosomal recessive disorder characterized by alacrima, achalasia, and adrenal insufficiency. Neurological involvement is common, but its electrodiagnostic (EDX) patterns are varied. The resulting phenotype, often marked by spasticity, can lead to diagnostic confusion with other motor neuron disorders. We aimed to define the neurological and EDX spectrum in a pediatric cohort and clarify their diagnostic relevance.
METHODS: This was a cross-sectional study of 17 pediatric patients with genetically confirmed triple A syndrome. All patients underwent n

INTRODUCTION/AIMS: Traditional methods for assessing the neurophysiological function of the hypoglossal nerve involve placing electrodes directly on the tongue, but these are limited by technical challenges. We propose an alternative method for hypoglossal nerve conduction studies.
METHODS: This study included 34 healthy adults, in whom compound muscle action potentials (CMAP) were recorded using an alternative electrode placement method, with the superficial active electrode positioned on the submental region and the reference electrode placed contralaterally. Repetitive nerve stimulation (R

OBJECTIVE: To investigate the correlation between nerve conduction study (NCS) parameters and the prognosis of patients with amyotrophic lateral sclerosis (ALS).
METHODS: NCS parameters were recorded from the median, ulnar, peroneal, tibial, and superficial peroneal nerves of 114 sporadic patients with ALS. The main endpoint was death or tracheostomy. Univariate Cox regression analysis was conducted for all the NCS parameters. Subsequently, multivariate analysis was performed using Cox stepwise regression, incorporating factors identified in the univariate analysis and known prognostic factor

OBJECTIVE: Cramp-fasciculation syndrome (CFS) is a heterogeneous condition with multiple underlying causes. Although dysfunction of slow K(+) channels has been reported in patients with CFS, testing all potential candidates for this problem using conventional in vitro functional analysis would be prohibitively cost- and labor-intensive. However, relatively economical and non-invasive nerve-excitability testing can identify ion channel dysfunction in vivo when combined with numerical modeling.
METHODS: Patients with CFS underwent nerve conduction study, needle electromyography, and nerve excit

Cardiac refractoriness and electrical inducibility of supraventricular tachycardia (SVT) were assessed at intervals of 1 to 2 hours over a period of 22 hours in 38 patients (25 with paroxysmal SVT). Daily variability of effective refractory period (ERP) had a mean range of 35 +/- 14 mseconds for the atria, 79 +/- 65 mseconds for the AV node, 24 +/- 10 m seconds for the ventricles, 51 +/- 27 mseconds for the retrograde Kent bundle, and 35 +/- 17 mseconds for the antegrade Kent bundle. Between 11 PM and 8 AM there was a significant prolongation of the ERP of the atria (ANOVA, p less than 0.001),

Benzoic acid, salicylic acid and 5-bromosalicylic acid have been studied for their mechanism of action on crayfish and squid giant axons. External application of 0.1 to 0.5 mM 5-bromosalicylic acid prolonged the falling phase of the action potential, and at concentrations higher than 1.5 mM, nervous conduction was blocked. Boltage clamp analyses showed that 5-bromosalicylic acid reduced both peak transient sodium conductance and steady-state potassium conductance. The curves relating the peak and steady-state conductances to membrane potential were shifted in the direction of depolarization. T

BACKGROUND: Multifocal motor neuropathy (MMN) and amyotrophic lateral sclerosis (ALS) can be difficult to differentiate, particularly at early disease stages for patients with hand-onset weakness and without upper motor neuron (UMN) signs. This study aimed to identify clinical and electrophysiological features that may facilitate early differentiation between MMN and ALS.
METHODS: We retrospectively analyzed the clinical, laboratory, and electrophysiological characteristics of patients diagnosed with MMN and ALS who underwent an identical nerve conduction study protocol comprising extended mo
