[Chronic electric stimulation of the internal globus pallidus and subthalamic nucleus in Parkinson disease].

PATHOPHYSIOLOGY: In Parkinson's disease, the neurodegenerative process of the nigrostriatal dopaminergic pathways induces an increase in activity of the subthalamic nucleus and the medial globus pallidus, which cause inhibition of thalamo-cortical outputs explaining parkinsonism. HIGH-FREQUENCY DEEP BRAIN STIMULATION: The adverse effects induced by lesions of subcortical structures (thalamotomy, pallidotomy) have increased interest in chronic electrical stimulation proposed as a new therapy in Parkinson's disease. This technique is reversible and can be modulated with less adverse effects. TW

[Physical diagnostics--tendon reflexes].

Clinical examination of the tendon reflexes is helpful in localising lesions in the nervous system, which is an important step in the diagnosis of neurological disorders. The large intra- and interindividual variations of reflex amplitudes make interpretation difficult. Published interobserver agreement studies provide conflicting results. The diagnostic specificity of unequivocally abnormal reflexes is moderate to good (70-95%, decreasing with increasing age), but the sensitivity is lower (50-70%), both for peripheral and for central lesions. This implies that the finding of a clearly abnorma

Psychogenic or dissociative fugue: a clinical investigation of five cases.

Dissociative fugue (formerly psychogenic fugue) is a rare and little understood dissociative disorder. Following a review of the pertinent literature, five cases of dissociative fugue are described. These cases were systematically studied with a comprehensive history, mental status examination, physical and neurological evaluation, review of previous medical and psychiatric records, and psychological testing including MMPI, WAIS-R, electroencephalogram, and Dissociative Experiences Scale. An unexpected finding was that, in some cases, associated criminal activity may allow the person with diss

Response of major depression and Tourette's syndrome to ECT: a case report.

There have been few published reports on the use of electroconvulsive therapy in the treatment of Tourette's syndrome. We present a case of a woman with preexisting Tourette's syndrome, which had been latent for some years, that was exacerbated with the onset of a major depressive episode. Both the tics and the depressive symptoms completely remitted after eight sessions of electroconvulsive therapy. Possible mechanisms of this effect are discussed.

Cauda equina syndrome. Implications for the orthopaedic nurse in a clinical setting.

Cauda equina syndrome (CES) is a presentation of signs and symptoms that indicate nerve compression involving one or more of the nerve roots in the lumbar spine. The Clinical Nurse Specialist working in a general orthopaedic practice must become familiar with the presentation of CES to appropriately assess the patient and alert the physician. Prompt and accurate care of the patient is critical when cauda equina syndrome is suspected to minimize permanent nerve damage. This article describes the syndrome, its signs and symptoms, basic triage techniques, plan of care, and recovery for the patien

Clinical assessment of adventitious movements.

Many procedures with variable validity and reliability have been developed in research settings to evaluate adventitious movements and related phenomena in specific populations, e.g., people with schizophrenia treated with dopamine antagonists, but these only provide global assessments or rate specific movements. A battery for rating individuals with possible movements disorders in a comprehensive way in clinical settings is needed so a protocol to assess briefly and thoroughly potential movement disorders was videotaped for five prepubertal boys with autistic disorder and severe mental retard

Dystonia secondary to electrical injury: surface electromyographic evaluation and implications for the organicity of the condition.

We describe a patient who developed right arm dystonia following an electrical injury. The patient's arm remained adducted, and flexed at the elbow and wrist, with all movement resulting in pain and tremor. Surface electromyographic evaluation revealed constant tonic activity of multiple upper and lower arm muscles at rest, that was not distractible. Voluntary and passive movement of the elbow or wrist resulted in high amplitude EMG activity, with motor grouping at 11 Hz at the elbow and 8 Hz at the wrist. Although a diagnosis of psychogenic dystonia was entertained, the stereotyped nature of

[Orbital myositis].

Orbital myositis is a subgroup of the nonspecific inflammatory syndrome or orbital pseudotumor and is characterized by a primary inflammation of extraocular muscles. The authors describe a 70-year-old patient with acute proptosis, ocular pain and right ophthalmoplegia, whose orbital computed tomographic scan showed enlargement of the homolateral extraocular muscles. Clinical presentation and complementary tests were compatible with the diagnosis of orbital myositis however, because of the particular aspects, which included retinal central vein occlusion, optic nerve lesion, distension of the s

[The classification and topical diagnosis of cerebral circulatory disorders in newborn infants].

The classification of disorders of cerebral circulation (DCC) was presented on the basis of clinical-neurosonic study of 100 newborn children with the signs of DCC. There were-determined the same degrees of DCC severity as in hypoxic-ischemic encephalopathy (that is light, manifested, severe ones). There weren't observed any signs of focal damages of brain in light form of DCC, although there might sometimes be found the small ischemic damages of brain and injuries of hemorrhagic type in one third of patients. In manifested DCC there were revealed the signs of focal cerebral damages which were

Comparison of clinical rating scales in treatment of cervical dystonia with botulinum toxin.

Seventy-six consecutive patients with cervical dystonia. (CD) treated with botulinum toxin were assessed with the Tsui rating scale, the Toronto Western Spasmodic Torticollis Rating Scale (TWSTRS), and a global scale of improvement. Tsui, TWSTRS, and TWSTRS subscale scores all showed significant improvement. Tsui and TWSTRS score reduction rates correlated significantly with each other, and each correlated with the global scale. Analysis of these scales and TWSTRS subscales indicates that an objective scale of severity such as either the Tsui rating scale or TWSTRS severity subscale used in co

Management of Bell's palsy.

Bell's palsy or idiopathic palsy is the most common disorder affecting the facial nerve. Diagnosis is primarily one of exclusion. There is typically an acute unilateral facial paresis that evolves in 24 to 48 hours. Etiology and pathophysiology are heavily disputed, and as of yet unknown. The natural history of Bell's palsy is favorable. Eight-four percent show satisfactory recovery without any treatment, however 16% suffer moderate to severe sequelae. Prognosis is influenced by degree of paresis, age of patient, and time until first signs of recovery. Prognostic testing currently involves var

Superficial siderosis--a cause of audiovestibular failure.

Bilateral vestibular end organ failure in adult life is a rare condition with some specific known causes, such as relapsing polychondritis, autoimmune inner ear disease, Lues venerium, and an acute effect of gentamicin. This case report draws attention to a rare condition that is potentially recognizable early in its development. The patient has superficial siderosis, which is iron deposits over the cerebrum, resulting in progressive neurologic failure involving all of the systems. Early in its course before other symptoms appear, there is development of progressive hearing loss and vestibular

[Disorders of autonomic regulation in Parkinson syndrome].

Ever since the first description of Parkinson's disease, it has not only been associated with the classical triad of akinesia, rigor and tremor but also with autonomic regulation disorders. However, the studies published on this subject show differing and partly contradictory results. A large number of studies demonstrate a distinct autonomic disorder. On the other hand there are authors who question an affliction of the autonomic nervous system and others who attribute the autonomic disorder to Parkinson specific medication. To make matters worse, the various pathogeneses of Parkinson's syndr

Essential tremor and its variants.

Essential tremor is the commonest of movement disorders. Although sometimes prefaced with the term 'benign', it often causes significant disability. Diagnosis is based on the clinical finding of a postural tremor, predominantly affecting the upper limbs, that is absent at rest and not associated with extrapyramidal or cerebellar signs. There are, as yet, no specific anatomical, physiological, biochemical or genetic markers for the condition. Postural limb tremors, clinically indistinguishable from essential tremor, may occur in patients who have, or will later develop, other neurological condi

Clinical diagnosis of Parkinson's disease. Proposal of diagnostic subgroups classified at different levels of confidence.

The objective of this paper is to evaluate the accuracy of conventional diagnostic criteria for Parkinson's disease and give an overview of alternative causes to parkinsonian syndromes. We also propose a new approach to the clinical diagnosis of Parkinson's disease, which may improve the diagnostic accuracy. The available information on autopsy findings in patients clinically diagnosed as Parkinson's disease shows that 20 to 30% of these patients do not have the typical neuropathological features with Lewy bodies and cell loss in the substantia nigra. The use of selected additional clinical cr

Use of clomipramine to treat ritualistic stereotypic motor behavior in three dogs.

Atypical, stereotypic motor and locomotor behavior was observed in 3 dogs. In 1 dog, the behavior included circling, vocalization, and snapping; in another, circling associated with pica and aerophagia; and in a third, gradually intensifying locomotor behavior stimulated by bright light. This behavior interfered with normal interaction between their owners and the dogs, and has been associated with obsessive-compulsive disorders. Treatment with clomipramine, along with other psychotropic medications and behavioral modification, suppressed the inappropriate behavior in all dogs; in 2 of the 3 d

[Thoracic outlet syndrome: what is the current status?].

Under certain circumstances patients with a constitutional predisposition, mainly with atypical ligaments and bands or a cervical rib may develop a Thoracic Outlet Syndrome (TOS). TOS can be divided into three subgroups: arterial, venous and neurogenic. Most of our cases have neurogenic symptoms combined with signs of arterial compression. The complications of TOS are arterial and venous. "TOS" is a clinical diagnosis. A history of upper extremity paresthesia that is exacerbated by abduction is typical for TOS. Objective neurological findings are usually lacking. Positive provocative maneuvers
