Impact of Nutritional Management on Survival of Critically Ill Malnourished Patients with Refeeding Hypophosphatemia.

BACKGROUND: Early nutritional therapy may aggravate hypophosphatemia in critically ill patients. AIM: To investigate the influence of the type nutritional therapy on the survival of critically-ill malnourished patients at refeeding hypophosphatemia risk. METHODS: Retrospective cohort study including malnourished, critically-ill adults, admitted from June 2014-December 2017 in an intensive care unit (ICU) at a tertiary hospital. Refeeding hypophosphatemia risk was defined as low serum phosphorus levels (<2.5 mg/dL) seen at two timepoints: before the initiation and at day 4 of the nutritional

Potential Benefits of Medical Nutrition Therapy for Timely Ventilator Weaning in the Critically Ill.

Nutritional support and metabolic assessment are crucial in weaning from invasive mechanical ventilation. This review explores strategies for metabolic assessment and tailored nutritional interventions and their impact on weaning outcomes, highlighting individualized nutritional interventions. While indirect calorimetry provides direct metabolic data, diaphragm ultrasound can complement metabolic monitoring by assessing respiratory muscle function and detecting early signs of muscle wasting. Protein intake in critically ill patients remains controversial, as early high protein administration (…

SEPAR and SEEN Consensus Document on Nutritional Assessment and Management in Patients with COPD.

Malnutrition and sarcopenia are prevalent and often underdiagnosed in individuals with chronic obstructive pulmonary disease (COPD), with significant implications for morbidity, mortality, and quality of life. Despite their clinical relevance, current COPD guidelines provide limited direction regarding nutritional assessment and intervention. This consensus document, developed jointly by the Spanish Society of Pulmonology and Thoracic Surgery (SEPAR) and the Spanish Society of Endocrinology and Nutrition (SEEN), offers evidence-based recommendations for the nutritional assessment and managemen

Current practices for nutritional evaluation and care during the treatment of pediatric oncology patients: a survey among AIEOP centers.

UNLABELLED: Nutritional status plays a crucial role in the mortality rates of the pediatric oncology patients. However, there is a lack of systematic approaches for nutritional assessment in this population. This study aims to assess the current practice for nutritional assessment and care of pediatric cancer patients in Italy. A 25-items web-based, nation-wide questionnaire was circulated as of January 9, 2023 among physicians within the AIEOP network, composed of 49 national centers, out of which 21 routinely perform HCT. This survey examined the practices of 21 Italian pediatric oncology ce

Multidisciplinary Assessment and Individualized Nutritional Management of Dysphagia in Older Outpatients.

INTRODUCTION: The evidence on the efficacy of nutrition therapy to prevent complications of dysphagia is based on observational studies that used different tools for nutritional and dysphagia assessment, and different scales for the definition of diet textures, rendering their results incomparable and the knowledge on dysphagia management inconclusive. METHODS: This retrospective observational study was performed in 267 older outpatients who were assessed for dysphagia and nutritional status by a multidisciplinary team at the Clinical Nutrition Unit of IRCCS INRCA geriatric research hospital

[Nutrition and inflammatory bowel disease].

In this paper, the causes and consequences of nutritional deficiencies in inflammatory bowel disease are reviewed. Particular emphasis is made on their pathophysiological implications. In addition, the basis for nutritional intervention in these patients are described (both in terms of dietary counseling and the use of specialized nutritional support). Particular mention is made on the current evidences in favour of a primary therapeutic role of artificial nutrition and some specific nutrients in Crohn's disease.

Re-assessing the role of peri-operative nutritional therapy in patients with pancreatic cancer undergoing surgery: a narrative review.

Pancreatic cancer is the most common medical condition that requires pancreatic resection. Over the last three decades, significant improvements have been made in the conditions and procedures related to pancreatic surgery, resulting in mortality rates lower than 5%. However, it is important to note that the morbidity in pancreatic surgery remains r latively high, with a percentage range of 30-60%. Pre-operative malnutrition is considered to be an independent risk factor for post-operative complications in pancreatic surgery, such as impaired wound healing, higher infection rates, prolonged ho

Prevalence, impact, and management of adult disease-related malnutrition in African hospitals: A narrative review and insight from resource-limited clinical settings.

Disease-related malnutrition (DRM) is a worldwide problem regarded as a global policy priority, but occurs on an exaggerated scale in Africa. While interpretation of studies is difficult due to the variety of nutrition assessment techniques, DRM in acute care in-patient African settings is commonly reported in the range of 45-75%, with nutritional risk reaching 84%. Challenges to the comprehensive management of DRM in the resource-limited clinical settings with few dietitians reviewed in this paper include the following: first, lack of routine nutrition screening resulting in more than 90% of

Preoperative Nutritional Optimization and Gastroenterological Challenges for Children with Severe Neurologic Impairment and Medical Complexity.

Optimizing nutrition in children with medical complexity is a crucial preoperative task for clinicians. A comprehensive nutrition assessment should include the evaluation of coexisting gastrointestinal conditions that affect nutritional status, as well as detailed physical, anthropometric, and laboratory assessments. Using this comprehensive evaluation and assessment, personalized nutrition plans that include oral and enteral nutrition support can be developed. Since nutrition interventions take time, clear communication between the nutrition and surgical teams is vital to ensure the appropria

Medical nutrition therapy for adult intestinal failure: A review of current perspectives.

Intestinal failure (IF) is a clinical condition resulting from intestinal resection or disease-associated malabsorption or dysmotility. It is characterized by the reduction of functional gut capacity below the minimum needed for adequate digestion and absorption of nutrients. The priority of management is maintaining optimal nutritional and metabolic support until maximum bowel adaptation. The nutritional intervention is the cornerstone of treatment for IF, incorporating the parenteral nutrition (PN) support, enteral nutrition (EN) and oral feeding regimen. The individualized nutritional strat

Nutrition and Lower Extremity Ulcers: Causality and/or Treatment.

The association between malnutrition, impaired wound healing, and the presence of chronic wounds has been recognized for a long time. It is widely believed that the lack of adequate nutrition increases the risk of developing wounds which have a great likelihood of progressing to chronicity due to lack of appropriate healing responses. This risk is particularly high in the aging population. For the individual patient, as well as patient populations, the diagnosis of malnutrition has been in dispute; further, there is lack of agreement of when and how to intervene nutritionally in those with wou

[Malnutrition in the elderly--a challenge for health services].

Protein-energy malnutrition (PEM) remains common in elderly and chronically ill individuals. PEM is an independent risk factor for death in the elderly, and contributes to increased risk of infection, hip fracture, pressure sores and depression. Intervention studies indicate that nutritional treatment may confer positive effects in patients with chronic obstructive lung disease, during rehabilitation following hip fracture, and in elderly patients with multiple disorders. However, the scientific foundation for this is still weak, and for many wasting disorders there are no available data suppo

Nutrition in the critically ill patient.

Critically ill patients are hypermetabolic and have increased nutrient requirements. Although it is assumed that nutritional support is beneficial in this group of patients there are no well designed clinical trials to test this hypothesis. The rationale for nutritional support, therefore, is based upon clinical judgement. Although it is not known how long a critically ill patient can tolerate what is effectively starvation, the loss of lean tissue which occurs in catabolic patients (20-40 g nitrogen/day) suggests that depletion to a critical level may occur after 14 days. Total parenteral nut…

Nutrition Therapy in Sepsis.

Sepsis is characterized by early massive catabolism, lean body mass (LBM) loss, and escalating hypermetabolism persisting for months to years. Early enteral nutrition should attempt to correct micronutrient/vitamin deficiencies, deliver adequate protein and moderated nonprotein calories, as well-nourished patients can generate reasonable endogenous energy. After resuscitation, increasing protein/calories are needed to attenuate LBM loss and promote recovery. Malnutrition screening is essential, and parenteral nutrition can be safely added when enteral nutrition is failing based on preillness m
