Historically, children with liver tumors were uniformly considered to have a poor prognosis and were approached warily by pediatric surgeons and oncologists. Over the last decade, however, advances in diagnostic imaging technology, discovery of effective chemotherapy, and improved surgical techniques have greatly facilitated the management of children with hepatic tumors. Although orthotopic liver transplantation has emerged as a viable treatment option in the management of hepatic malignancies, including hepatoblastoma and hepatocellular carcinoma, its use is still controversial.

Asymptomatic ovarian tumors in menopausal women do not always require surgical removal. Experts recommend an initial evaluation with transvaginal ultrasound to help characterize the tumor's malignant risk. Low-risk tumors can be monitored with ultrasound, whereas high-risk tumors should be referred to a gynecologic oncologist. Indeterminate tumors require secondary testing with the strategies outlined in this Practice Pearl.

The search for an effective treatment for operable breast cancer combining cosmetically acceptable local control and maximal probability of cure is being conducted worldwide. The role of surgery is changing, with less emphasis on super-radical procedures. A combined approach is evolving in which treatment depends upon the participation of surgeon, pathologist, radiotherapist and medical oncologist. The results of tested combined treatment schedules are reviewed in this paper.

In this report, we discuss this clinical case, when the laryngeal remnant, or "neo-larynx", is not part of the clinical target volume, it should be delineated as a bona-fide organ at risk or avoidance structure: recognizing the post-surgical anatomy could help maximize the preservation of the functional outcome. In addition, raising awareness on this rare yet very challenging disease presentation could foster a more homogeneous practice among radiation oncologists treating laryngeal cancer analogue to thoracic cancer.

Considering intracranial tumours, only few indications of protontherapy, such as chordoma, chondrosarcoma or uveal melanoma, are uniformly approved in the world. Other indications, excluding paediatric pathologies, are still debated. The aim of this article is to describe the rationale for the use of protonbeam irradiation for meningioma, pituitary adenoma, craniopharyngioma, paraganglioma, glioma, and schwannoma, and to inform the radiation oncologists if prospective studies or randomized studies are opened for inclusions. This article deals only with indications for adults.

How can we better understand and improve our practice around the physical and psychological well-being of women treated with radiation therapy for pelvic malignancy? In this issue, Summerfield et al report the results of a nationwide survey capturing practices around the management of radiation therapy-induced vaginal adhesions and stenosis (RTVAS) across New Zealand. This study highlights the need for oncologists to improve care around a challenging but critically important aspect of women's health beyond a cancer diagnosis.

A seven-year-old girl who presented with precocious puberty was diagnosed with an estrogen-secreting right ovarian tumor. Right salpingo-oophorectomy with staging was performed by gynecologic oncologists. Intraoperative frozen section reported sex cord stromal tumor. Histopathological report confirmed sex cord stromal tumor with annular tubules with brisk mitotic counts (17 per 10 high-power fields). The post-operative course was uneventful. She remained asymptomatic with normalisation of serum estradiol upon six-month follow-up.

Therapeutic monoclonal antibodies have long been an effective tool deployed by physicians of different specialties, particularly by oncologists. Bispecific antibodies opened up new horizons in the treatment of cancer as they allow to involve a patient’s endogenous immunity in his or her fight against tumor. The present review covers all the formats and strategies used in engineering of bispecific antibodies which reached the stage of clinical trials and also focuses on the available results of these clinical trials.

Patients with connective tissue disorders are clinically challenging for radiation oncologists as these patients may be at increased risk for radiation-related skin toxicity. A clinical dilemma presents itself in a patient with lupus who presents with confluent skin lesions from mycosis fungoides requiring radiotherapy. In this report, we discuss an innovative technique used to develop an immobilization device that also effectively functioned as a uniform bolus with distinct dosimetric advantages to the use of a facial moulage.

Frequent laboratory tests and diagnostic imaging examinations after primary treatment of breast cancer do not improve survival or influence the quality of life. The economic impact should be also taken into consideration. Intensive follow-up should not be recommended as a routine policy based on evidence-based medicine. However, patients usually want to be seen frequently by a physician and undergo diagnostic tests even though free of symptoms. This dilemma for oncologists has not been resolved.
