We reviewed the records of all children (younger than 16 years of age) who presented with a diagnosis of optic neuritis (ON) identified through the comprehensive records-linkage system at the Mayo Clinic and identified 94 cases between 1950 and 1988 with a documented history of idiopathic ON. A case of 31-year-old man with Leber's idiopathic stellate retinopathy is presented. On the eye fundus there was optic disc swelling and maculopathy with yellow star-like foci. The visual acuity was -0,01. Antibiotics and encorton were applied in the treatment. During the therapy the visual acuity was improved, maculopathy and disc swelling reduced. We wanted to present the above case because of its unique occurrence and diagnostic difficulties.

We report a case of rhinogenous optic neuritis in a 50-year-old man demonstrating drastic diurnal variation in visual function. His left vision was 20/15 in the morning, which gradually decreased to counting fingers at night. Pupillary reaction and visual field showed the same fluctuation as vision. Complete remission of visual symptoms was achieved after endonasal ethmoid sinusotomy.

Sixty-nine cases of Optic Neuritis were studied in order to evaluate the percentage of evolution into multiple sclerosis. We observed an incidence rate of 53.6% which is somewhat high respect to data present in literature. The various findings obtained in the present study were compared with those of the literature and the similarities and discrepancies underlined.

15 patients with unilateral optic neuritis and 2 patients with bilateral optic neuritis were treated with 1000 mg methylprednisolone i.v. per day for 5 days. 24 patients with clinically isolated optic neuritis (ON) were examined with MRI. Only 5 patients (22.9%) had a normal MRI scan. The number of detected clinically silent lesions ranged from 0 to 38. They were mainly located in the frontal and parietal white matter. All patients with more than 3 lesions on MRI had pathological findings in CSF. There was no correlation between the number and location of lesions and age at onset of ON.

The C wave was recorded from 20 eyes suffering from neuritis due to demyelinating disease and in 5 eyes showing optic atrophy due to trauma or to increased intracranial pressure. Our results show that in cases with optic neuritis the C wave amplitude was clearly lower than normal. The disparity between clinical visual function and pattern visual evoked response (VER) was studied in 53 patients who had suffered an attack of optic neuritis (ON) more than six months before. Magnetic resonance imaging of the brain was performed on ten patients who had isolated optic neuritis. Seven patients had abnormal images, with a distribution of lesions similar to that seen in multiple sclerosis, demonstrating subclinical dissemination in the central nervous system.

In two case of cystic craniopharyngioma the initial presentation was misleading and suggested acute retrobulbar optic neuritis: 1) isolated and unilateral drop of visual acuity of recent onset; 2) total remission in a few weeks, then a relapsing and steroid-sensitive course; 3) increase of C.S.F. gammaglobulins content in one case, normal CT scan in the second one. The diagnosis was corrected 36 and 18 months respectively after the onset of the clinical disorders.

In 1974 we examined 30 patients 0.5-14 (mean 5) years after acute unilateral optic neuritis (ON), when no clinical signs of multiple sclerosis (MS) were discernible. 11 of the patients had oligoclonal bands in the cerebrospinal fluid (CSF).
