Primary Atypical Lipomatous Tumor of the Orbit: A Case Report.

PURPOSE: To describe a case of primary atypical orbital lipomatous tumor (ALT). CASE REPORT: A 35-year-old man presented with a two-month history of left eye proptosis and vertical diplopia. His visual acuity was 20/30 OD and 20/60 OS. External examination showed proptosis and downward displacement of the left eye with mild lid erythema. Extraocular movements were reduced in the left eye, with 10% and 70% motility in upgaze and abduction/adduction, respectively. Imaging showed a mass (22 × 16 × 46 mm) in the superior left orbit that infiltrated the orbital fat and the superior rectus muscle. …

Presentation, Treatment, and Prognosis of Secondary Melanoma within the Orbit.

BACKGROUND: Ocular melanoma is a rare but often deadly malignancy that arises in the uvea, conjunctiva, or orbit. Uveal melanoma is the most common type, with conjunctival melanoma being the second most frequently observed. Melanoma accounts for 5-10% of metastatic or secondary orbital malignancies, but only a minute proportion of primary orbital neoplasia. The aim of this study was to characterize the clinical presentation, treatment, and prognosis in patients presenting with melanoma metastatic to, or secondary within, the orbit. METHODS: A retrospective cohort study of patients presenting …

MRI and FDG PET/CT Findings of Primary Orbit Leiomyosarcoma.

Primary leiomyosarcoma of the orbit is extremely rare. Here we report the case of a 32-year-old woman who presented with proptosis and diplopia of the left eye. MRI examination showed a solid mass in the left orbit with invasion to adjacent tissue, and the lesion had intense FDG uptake with SUVmax of 18.7 on F-FDG PET/CT. Primary orbit leiomyosarcoma was diagnosed by surgery-pathology. F-FDG PET/CT has an advantage in excluding secondary or metastatic orbit malignancy.

Natural killer/T-cell lymphoma invading the orbit and globe.

Natural killer/T-cell lymphomas are extremely rare and carry high mortality rates. Epidemiologically, these cancers tend to affect mainly Asian and South American patients and are associated with Epstein-Barr virus seropositivity. This report details a 78-year-old Vietnamese woman who presented initially with vitritis of unknown cause, but later developed proptosis and conjunctival involvement as her disease spread. Biopsies of the orbit, ethmoid sinus, and conjunctiva were found to be significant for natural killer/T-cell lymphoma. The case highlights the diagnostic difficulty of this tumor g…

CASE REPORT Penetrating Injury of the Orbit by a Needlefish.

OBJECTIVE: We present a very rare case of penetrating injury into the orbit by a needlefish. The patient underwent extirpation twice at another hospital. METHODS: We performed foreign body removal from a right subbrow incision under general anesthesia. RESULTS: The foreign body was successfully removed and the patient's diplopia recovered gradually after surgery. CONCLUSIONS: Treatment was similar to that used for penetrating injury. As there is a risk of secondary infection, it is important to completely remove the fish body, followed by vigorous irrigation and debridement.

Evaluation and management of 2 ferocactus spines in the orbit.

A 49-year-old woman, who had fallen face first in a cactus 1 week earlier, presented with a small, mobile, noninflamed subcutaneous nodule at the rim of her right lateral orbit with no other functional deficits. A CT scan was obtained, which revealed a 4-cm intraorbital tubular-shaped foreign body resembling a large cactus spine. A second preoperative CT scan, obtained for an intraoperative guidance system, demonstrated a second cactus spine, which was initially not seen on the first CT scan. Both spines were removed surgically without complication. The authors discuss factors that can cause d…

Refractory pyoderma gangrenosum of the orbit and the lacrimal sac.

Pyoderma gangrenosum (PG) is a rare, idiopathic, necrotizing, and non-infectious ulcerating skin disease included among the neutrophilic dermatoses. It rarely affects the eye, orbit, and/or ocular adnexa. We describe one case of PG affecting both orbits and the lacrimal sac in a patient with Crohn's disease.

Silicone spacer repair of medial rectus after iatrogenic orbit fracture.

A 51-year-old woman underwent endoscopic nasal polypectomy and ethmoidectomy with accidental entry into the right orbit causing enophthalmos and transection of the medial rectus muscle (MR). The repair of a fracture and of a damaged MR is technically challenging, particularly when large portions of bone and muscle are missing. We report a rare case of repair of the bony defect with an implant and reattachment of the MR with a silicone retina band, through a combined transcaruncular and transconjunctival approach. Postoperatively, the patient had residual enophthalmos and strabismus; further su…

[Clinico-pathological correlations: posterior compartment of the eye and orbit].

Diseases of the posterior compartment and the orbit are characterised by histological findings, most of which can be reproduced clinically. Examples are the examination of calcifications in retinoblastoma by ultrasonography. In the present review, histological findings of tumour and other diseases of the posterior ocular compartment and the orbit are presented and correlated with the clinical pictures and imaging techniques: uveal melanoma, choroidal nevus, choroidal metastases, choroidal hemangioma, retinoblastoma, Coat's disease, sympathetic ophthalmia, pleomorphic adenoma (benign mixed tumo…

Mantle cell lymphoma of the maxillary sinus invading the orbit.

BACKGROUND: Lymphomas are the most common primary orbital tumor in adults 60 years of age and older. Mantle cell lymphoma (MCL) is a distinct subtype of B-cell non-Hodgkin's lymphoma. This case report describes a presentation of MCL with ocular involvement and discusses the management options. CASE REPORT: A 63-year-old black male Vietnam veteran presented with symptoms of numbness of the right tooth and gum area with right maxillary facial pain, which he described as stabbing in nature. After extensive evaluation, the patient's symptoms were found to be caused by MCL of the right maxillary s…

Invasive aspergillosis of orbit in immunocompetent patients: treatment and outcome.

OBJECTIVE: Invasive sino-orbital aspergillosis in immunocompetent patients is a rare clinical entity and often misdiagnosed. Invasive sino-orbital aspergillosis is potentially fatal; however, mortality may be avoided with timely treatment. Presently, there are no universal guidelines for its treatment. We present the clinical features, treatment, and outcome in 15 cases of invasive sino-orbital/orbital aspergillosis in healthy individuals. DESIGN: Retrospective case series. PARTICIPANTS: Fifteen cases of invasive sino-orbital/orbital aspergillosis in immunocompetent individuals. METHODS: We…

CT based three-dimensional measurement of orbit and eye anthropometry.

PURPOSE: To measure eye and orbit anthropometric variation within the normal population by using CT images and to determine the effects of age and sex on eye and orbit anthropometry. Quantification of eye and orbit anthropometric variation within the normal population and between persons of different age and sex is important in the prediction and prevention of eye injury. METHODS: A systematic method was developed to align head CT images three dimensionally and to measure ocular and orbital parameters in 39 subjects. Twenty-four measurements were collected along the orbital rim, to quantify t…

Successful treatment of infantile haemangiomas of the orbit with propranolol.

BACKGROUND: Propranolol is a novel therapeutic agent in the treatment of cutaneous infantile haemangiomas. We assessed the effect of propranolol therapy in infantile haemangiomas of the orbit. METHODS: A case series of four patients with orbital infantile haemangiomas were referred for management in our tertiary referral hospitals. Two of the patients had inadequate responses to prior corticosteroid therapy. One of the patients was commenced on propranolol at 2.5 years of age when the lesion was not in the proliferative phase. This represented the first case report of propranolol treatment fo…

Cutaneous malignant melanoma metastatic to the eye, lids, and orbit.

The incidence of malignant cutaneous melanoma is increasing faster than any other cancer. Thus, it will become an increasingly common source of metastatic disease to the eye, lids, and orbit. Herein, we have performed a systematic review of previously published cases including patient characteristics, clinical presentation, diagnostic techniques, current treatments, and outcomes. At the time of ocular diagnosis, nearly all reported patients had a known history of cutaneous melanoma and synchronous non-ocular metastases. Several aspects help in differentiating the tumors from primary uveal mela…

Primary neuroendocrine tumor of the orbit progressing to neoplastic meningitis.

A 60-year-old man presented with multiple cranial neuropathies and an identifiable left orbital lesion along the course of the supraorbital nerve. The pathologic features of the excised orbital lesion were consistent with a poorly differentiated primary neuroendocrine carcinoma. Four years after his diagnosis, the patient succumbed to neoplastic meningitis. No other primary tumor site was identified or clinically apparent during his illness.

[Complex diagnosis of fragment injuries of the eye and orbit].

The paper presents data on the prevalence and pattern of fragment injuries of the eye and orbit in the Sverdlovsk Region in 2000-2004. It describes the authors' system for complex diagnosis of metallic foreign bodies, which is based on the wide use of electron location in combination with other clinical, instrumental, and special studies. The comparative results of treating patients with the standard and proposed system, which demonstrate the advantages of the latter in small-sized fragments in particular, are given.

Intramuscular haemangioma of the orbicularis oculi muscle of the orbit.

Intramuscular haemangiomas are uncommon, benign tumours frequently arising in the trunk and extremities. When they appear in the head and neck, the most frequent sites are the trapezius and the masseter muscles, rarely other sites. The case is reported of an intramuscular haemangioma with an unusual location in the orbital part of the orbicularis oculi muscle of a 54-year-old female. Clinical, radiological and histological findings as well as the treatment approach are discussed.
