Orthopaedic Surgical Skills Simulation: Comparing Training Utility of Cadaveric Models to Sawbones in Resident Education.

This study aimed to compare the effect of surgical simulation between high-fidelity cadaveric (HF) and low-fidelity Sawbones (LF) models on teaching fracture care assessed with surgical performance using HF cadaveric models. Fourteen orthopaedic surgery residents (PGY-1-PGY-5) were divided into HF and LF groups, matched by training level. Each group practiced fracture fixation on their respective fidelity-level models with OTA Type 33-C1 distal femur fractures: the HF group on Rimasys pre-fractured cadaveric models (Rimasys GmbH, Cologne, Germany) and the LF group on Sawbones (Pacific Research

Antimicrobial Activity of the Most Common Antibiotic-Releasing Systems Employed in Current Orthopedic Surgery: in vitro Study.

PURPOSE OF THE STUDY Infections of joint replacements represent one of the most serious problems in contemporary orthopedics. The joint infections treatment is usually multimodal and involves various combinations of drug delivery and surgical procedures. The aim of this study was to evaluate and compare the bacteriostatic and bactericidal properties of the most common antibiotic carriers used in orthopedic surgery: bone cements mixed with antibiotic and porous calcium sulfate mixed with antibiotic. MATERIAL AND METHODS Three commercial bone cements (Palacos®, Palacos® R+G, Vancogenx®) and comm

Volume and Variability of Foot and Ankle Case Exposure During Orthopaedic Residency: 2014-2019.

BACKGROUND: Exposure to a comprehensive breadth and volume of surgical cases is a fundamental component of orthopaedic education, though standardization of case exposures across residency programs is limited to a small amount of required case minimums. Significant variability in exposure to subspecialty cases, such as foot and ankle surgeries, can create distinctly different residency experiences. METHODS: Accreditation Council for Graduate Medical Education (ACGME) surgical case logs from 2014 to 2019 for leg/ankle and foot/toes were examined following the 2013 implementation of case minimum

Risk Factors for Venous Thromboembolism in Children Undergoing Orthopedic Surgery.

Venous thromboembolism (VTE) is an uncommon but highly morbid and potentially preventable complication in children. This study aimed to characterize the incidence of, and risk factors for, VTE in children undergoing orthopedic surgery. A retrospective analysis was performed using the 2012 to 2017 American College of Surgeons National Surgical Quality Improvement Program Pediatric (NSQIP-P) database. Patient demographics, comorbidities, operative variables, and perioperative outcomes were compared between patients who did and did not develop a VTE. In total, 81,490 pediatric patients who underw

Liposomes for Intra-Articular Analgesic Drug Delivery in Orthopedics: State-of-Art and Future Perspectives. Insights from a Systematic Mini-Review of the Literature.

Background and objectives: Liposomal structures are artificial vesicles composed of one or several lamellae of phospholipids which surround an inner aqueous core. Given the amphoteric nature of phospholipids, liposomes are promising systems for drug delivery. The present review provides an updated synthesis of the main techniques for the production of liposomes for orthopedic applications, focusing on the drawbacks of the conventional methods and on the advantages of high pressure techniques. Materials and Methods: Articles published in any language were systematically retrieved from two major

Opioid induced constipation management in orthopaedic and trauma patients: treatment and the potential of nurse-initiated management.

BACKGROUND: Opioid analgesics treat moderate to severe pain with proven analgesic efficacy, although their use is associated with dose-limiting side effects, such as constipation. Orthopaedic and trauma patients are at high risk of developing opioid-induced constipation (OIC) due to reduced mobility and increased opioid requirements to manage prolonged pain after injury. OBJECTIVES: To examine the evidence base to guide clinicians on the most effective or tolerated laxative regimen for the management of OIC and nurse-initiated management of OIC. METHOD: A review of the literature was underta

The effect of minimally displaced posterior malleolar fractures on decision making in minimally displaced lateral malleolus fractures.

PURPOSE: Despite the high incidence of cases of minimally displaced lateral or posterior malleolus ankle fractures, treatment guidelines are still an issue of controversy. The purpose of this study was to delineate treatment preferences among orthopaedic surgeons in these fractures with and without concomitant posterior malleolus fractures. We hypothesized that concomitant minimally displaced fractures of the posterior malleolus can shift treatment preference towards operative intervention. METHODS: A questionnaire-based study was conducted among orthopaedic surgeons attending the 2012 Europe

Mixed-Methods Evaluation to Identify Factors Influencing High-Value Surgeon Decisions in Orthopedics: An Example in Anterior Cruciate Ligament Reconstruction.

RATIONALE: Many advances have been made to identify novel, effective orthopedic care practices. For all that is known about the comparative effectiveness of various clinical decisions in anterior cruciate ligament (ACL) injury care, surprisingly little is known about how surgeons approach decision-making in the face of this evidence. Implementation science models such as Capability, Opportunity, Motivation - Behavior (COM-B) offer a way to organize and comprehensively understand how clinicians make decisions. AIMS AND OBJECTIVES: This evaluation aimed to understand and quantify the factors th

Damage control does not offer a survival advantage and increases the risk of serious complications compared with early total care in severely injured patients with femoral shaft fractures.

BACKGROUND: Damage control orthopedics (DCO) was proposed to minimize the second hit of extensive surgical procedures in severely injured patients when treated with early fracture fixation (early total care [ETC]). The impact of DCO and ETC on the outcomes of severely injured patients sustaining femoral shaft fractures (FSFs) is unclear. We hypothesized that DCO is associated with lower mortality and decreased incidence of complications compared with ETC. METHODS: The Trauma Quality Improvement Project database was queried from 2007 to 2021. Adult patients 14 years or older with FSF and Injur

The quality of informed consent obtained for orthopedic surgeries-elective versus trauma: A prospective interview-based study.

BACKGROUND: Orthopedic surgeons routinely obtain informed consent prior to surgery. Legally adequate informed consent necessitates a thorough discussion of treatment options and risks and proper documentation. However, the quality of informed consent in orthopedic trauma patients is an under-researched area. PURPOSE: To assess the quality of the informed consent process in trauma compared with elective orthopedic patients and to assess patients' emotional state at the time of signing consent form. METHODS: Sixty-two consecutive patients undergoing either elective total joint arthroplasty ( N
