Cervical anterior spinal artery infarction associated with anomalous vertebral artery: a case report.

We report a unique case of cervical anterior spinal artery (ASA) infarction in a 49-year-old male with hypercholesterolemia and sleep apnea. The patient experienced sudden cervical pain, quadriparesis, areflexia, and urinary incontinence after swallowing a large food bolus. Imaging revealed an infarction at the C3-C5 levels and an anomalous right vertebral artery (VA) originating from the thoracic aorta, tightly enclosed between the aorta and a vertebral column with an anterior osteophyte. This aberrant VA was the primary vascular supply to the ASA, with no contribution from the left VA or sup…

Src Homology 2 Domain-Containing Protein Tyrosine Phosphatase Promotes Inflammation and Accelerates Osteoarthritis by Activating β-Catenin.

Osteoarthritis (OA) is a chronic articular disease characterized by cartilage degradation, subchondral bone remodeling and osteophyte formation. Src homology 2 domain-containing protein tyrosine phosphatase (SHP2) has not been fully investigated in the pathogenesis of OA. In this study, we found that SHP2 expression was significantly increased after interleukin-1β (IL-1β) treatment in primary mouse chondrocytes. Inhibition of SHP2 using siRNA reduced MMP3, MMP13 levels, but increased AGGRECAN, COL2A1, SOX9 expression in vitro. On the contrary, overexpression of SHP2 exerted the opposite result…

Surgical management of dysphagia due to diffuse idiopathic skeletal hyperostosis: the role of barium swallow fluoroscopy-a case report.

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) is a systemic condition that might lead to dysphagia because of massive osteophytes that may be present at multiple levels. Confirming the symptomatic level to guide surgical management and avoid extensive surgery is important, however, there is no globally accepted consensus on the topic. CASE DESCRIPTION: We report the case of a 51-year-old man, with no specific past medical history, who has been complaining of a 3-months pain in the left side of the tongue base with sensation of a lump in the throat and dysphagia. Computed tomogra…

Radicular Pain After Hip Disarticulation: A Clinical Vignette.

A 36-yr-old man with a history of industrial accident causing traumatic left hip disarticulation, pubic symphysis, and right sacroiliac joint fractures presented with a 3-yr history of left-sided lower back pain radiating down the amputated limb. Computed tomography lumbar spine showed osteophytes surrounding the sacroiliac joint bilaterally with reduced left L4-L5 foraminal space. A fluoroscopically guided left sacroiliac steroid injection led to mild improvement in low back pain. Magnetic resonance imaging of the lumbar spine without contrast showed transitional type L5 vertebral body with l…

Association between Patient History and Physical Examination and Osteoarthritis after Ankle Sprain.

UNLABELLED: Structural abnormalities on MRI are frequent after an ankle sprain. To determine the association between patient history, physical examination and early osteoarthritis (OA) in patients after a previous ankle sprain, 98 patients with persistent complaints were selected from a cross-sectional study. Patient history taking and physical examination were applied and MRI was taken. Univariate and multivariable analyses were used to test possible associations. Signs of OA (cartilage loss, osteophytes and bone marrow edema) were seen in the talocrural joint (TCJ) in 40% and the talonavicul…

Development of osteoarthritic features in estrogen receptor knockout mice.

OBJECTIVE: Estrogens are suggested to play a role in the development of osteoarthritis as indicated by the increased prevalence in women after menopause. We studied whether deletion of the estrogen receptor (ER) alpha, beta, or both in female mice results in cartilage damage, osteophytosis, and changes in subchondral bone of skeletally mature animals. METHODS: We studied knee joints of 6-month-old female ERalpha-/-, ERbeta-/-, and (double) ERalpha-/-beta-/- mice and their wild type (wt) littermates. The presence and size of osteophytes and osteoarthritic changes in cartilage were analyzed usi…

Etiologic factors of myelopathy. A radiographic evaluation of the aging changes in the cervical spine.

The radiographic characteristics of the cervical spine among older individuals were investigated in 100 normal subjects and compared with those of younger subjects. The cervical spine of the older subjects displayed narrowing of intervertebral discs and osteophytoses (posterior osteophytes as well as anterior osteophytes) at the levels of C5-6 and C6-7, where the range of motion was decreased. Such degenerative changes resulted in vertebrolisthesis, especially retrolisthesis, predominantly at the levels of C3-4 and C4-5, where intervertebral disc space was well maintained and mobility was well…

Patellar luxation causes abnormal mechanical loading and joint destruction in the femoral-tibial joint.

AIMS: Patellar luxation can cause abnormal mechanical loading and is a leading cause of patellofemoral joint damage, however its contribution to femoral-tibial joint damage is poorly elucidated. In this study, we sought to investigate the role of patellar luxation in the femoral-tibial joint in both mice and humans. METHODS: In this study, we generated a patellar luxation mouse model based on the standard operating procedure of destabilization of the medial meniscus (DMM) surgery which mimics osteoarthritis (OA). Then phenotypes including gait analysis, osteophyte formation, aberrant subchond…

The relation of femoral notch stenosis to ACL tears in persons with knee osteoarthritis.

OBJECTIVE: A significant risk factor for anterior cruciate ligament (ACL) tears in young athletes is a reduced femoral Notch Width Index (NWI). The purpose of this study was to test if persons with knee osteoarthritis (OA) and ACL tears have smaller NWI independent of prior joint injury and osteophyte volume. METHODS: We included 160 participants from the progression sub-cohort of the Osteoarthritis Initiative (OAI) Study, an ongoing 4-year, multi-center study, focusing on knee OA. The femoral notch width, the condylar notch width at 2/3 of the notch depth, and the intercondylar notch angle (…

Characterization of articular cartilage and subchondral bone changes in the rat anterior cruciate ligament transection and meniscectomized models of osteoarthritis.

Osteoarthritis (OA) is a chronic joint disease characterized by cartilage destruction, subchondral bone sclerosis, and osteophyte formation. Subchondral bone stiffness has been proposed to initiate and/or contribute to cartilage deterioration in OA. The purpose of this study was to characterize subchondral bone remodeling, cartilage damage, and osteophytosis during the disease progression in two models of surgically induced OA. Rat knee joints were subjected either to anterior cruciate ligament transection (ACLT) alone or in combination with resection of medial menisci (ACLT + MMx). Histopatho…
