Development of grating acuity in children treated for unilateral or bilateral congenital cataract.

PURPOSE: To study the development of grating acuity in children treated for dense congenital unilateral or bilateral cataract and to examine how variations in treatment affect grating acuity during early childhood. METHODS: The authors used optokinetic nystagmus (OKN), preferential looking (PL), or both to measure the grating acuity of children treated for congenital cataract in one eye (n = 63) or both eyes (n = 77) whenever possible from the time of treatment until 3 years of age. At each age, the authors compared patients' monocular acuity to that of children with no history of eye disorde

Visual results after primary intraocular lens implantation or contact lens correction for aphakia in the first year of age.

PURPOSE: The optimal role of intraocular lenses (IOLs) in infants remains a controversial topic. Some ophthalmologists advocate correction with a contact lens (CL), whereas others recommend an IOL correction. Our study compared visual acuity, ocular alignment, retreatment rate and binocular vision outcomes in children treated with these two methods at our clinic. METHODS: This study included 41 children with unilateral congenital cataract who underwent cataract surgery with posterior capsulorhexis and anterior vitrectomy, coupled with (IOL group, n=18) or without (CL group, n=23) primary IOL

Identification of Heterogenous Treatment Response to Corticosteroids Among Patients With Ventilator-Dependent Respiratory Failure: An Observational Cohort Study.

BACKGROUND: Corticosteroids increasingly are recommended as adjunctive treatments in critical illness syndromes; however, the existing literature supporting their benefit remains uncertain. Ventilator-dependent respiratory failure (VDRF) is a critical illness syndrome with low-acuity and high-acuity subphenotypes identifiable with electronic health record (EHR) data. RESEARCH QUESTION: Do conditional average treatment effects (CATEs) from corticosteroids with respect to hospital mortality differ by clinical subphenotypes in VDRF? STUDY DESIGN AND METHODS: In this multicenter, retrospective o

Effect of visual acuity on biometry prediction error after cataract surgery.

PURPOSE: To determine the effect of visual acuity on biometry prediction error. SETTING: Postgraduate teaching hospital. METHODS: The study was an observational case series of the first operated eye of 2149 consecutive patients who had cataract surgery. Biometry prediction error was defined as the difference between the planned refraction determined by biometry and the spherical equivalent of the final refraction. The principal outcome measure was the percentage of eyes within +/-1.00 diopter (D) of the intended refraction. This outcome was calculated for patients with visual loss caused by

Comparison of suppression, stereoacuity, and interocular differences in visual acuity in monovision and acuvue bifocal contact lenses.

PURPOSE: The relationship between visual acuity and stereoacuity has been well documented: as binocular visual acuity increases, stereoacuity improves. We compared interocular differences in visual acuity and stereoacuity in two presbyopic soft contact lens modalities, monovision and a new soft bifocal contact lens, the Acuvue Bifocal. The Acuvue Bifocal is hypothesized to show a smaller interocular acuity difference, increased stereoacuity, and decreased suppression over monovision at distance and near. METHODS: Monovision patients wearing Acuvue or Surevue soft contact lenses were tested fo

Association among visual hallucinations, visual acuity, and specific eye pathologies in Alzheimer's disease: treatment implications.

OBJECTIVE: Studies suggest a link between visual acuity and visual hallucinations in dementia, but links with specific eye pathologies have not been evaluated. METHOD: Fifty patients (20 with visual hallucinations, 30 without) with probable Alzheimer's disease had an evaluation of psychotic symptoms. Visual acuity was measured before and after refractions, and ophthalmological examinations included standardized assessments for cataracts and macular degeneration. RESULTS: Impaired visual acuity and the severity of cognitive impairments were significantly associated with visual hallucinations.

Surgical removal of subfoveal choroidal neovascularization without removal of posterior hyaloid: a consecutive series in younger patients.

PURPOSE: Subfoveal choroidal neovascularization (CNV) remains a common and important cause of visual loss. Previous studies have suggested that submacular surgery may improve or maintain visual acuity, particularly in younger patients. The majority of reported cases included removal of the posterior hyaloid during vitrectomy. The authors present a consecutive series of patients age 55 or younger with subfoveal CNV removal without posterior hyaloid removal. METHODS: Seventeen patients without age-related macular degeneration (ARMD), with subfoveal CNV from choroiditis, presumed ocular histopla
