A patient is described with systemic sclerosis (SSc) and subcutaneous cholesterol crystals mimicking calcinosis cutis. Though not detectable radiographically, basic calcium phosphate crystals were identified by alizarin red staining of aspirate from one digit. The value of crystal identification by microscopy in cases of SSc with presumed calcinosis cutis, but without radiographic evidence of subcutaneous calcific deposits, is emphasized.

This review examines the application of olfactory testing in the early stages of mild cognitive impairment (MCI) associated with Alzheimer's disease (AD), highlighting its potential and challenges in early screening and intervention. Olfactory function is typically divided into three domains: odor identification, odor discrimination, and odor threshold. Among these, odor identification and discrimination are closely linked to higher cognitive processes and exhibit significant impairment in patients with AD and MCI. Moreover, the anatomical and functional characteristics of the olfactory system

Tuberous sclerosis complex is an autosomal dominant multisystem disorder. Spontaneous mutations occur in up to 60% of patients with gene loci located on chromosomes 9q34 (TSC1) and 16p13 (TSC2). Diagnosis is established with the identification of various neurocutaneous markers and multiple organ system hamartomas. The variable expression of severity, the potential for cognitive dysfunction, and epilepsy compound the clinical picture. The intracranial abnormalities include the identification of migration and hamartomatous brain lesions, such as tubers, subependymal nodules, and subependymal gia

Dry electroencephalography (EEG) electrodes enable rapid, gel-free setups, which are crucial for point-of-care diagnostics, but often face challenges with comfort and signal quality-especially in a clinical context. Novel "flower" dry electrodes are a special type of reusable scalp electrodes for dry EEG, featuring a distinct flower-like shape with angled pins in three intertwined layers. While the new electrode design has been validated in an in vivo study on healthy volunteers, we tested its clinical applicability in a proof-of-concept study involving three patients diagnosed with epilepsy a

PRACTICAL RELEVANCE: Cardiovascular and kidney diseases are highly prevalent and frequently coexistent in cats, particularly those of advanced age. Several pathways of interaction between the heart and kidneys likely contribute to progressive functional decline of these systems during disease of one or both. Identification of heart or kidney disease in a cat should prompt the clinician to consider how the disease and its treatment might impact the function of the other organ and how coexisting primary disease of the other might impact case management and patient prognosis.

Artificial intelligence (AI) is assuming an increasingly prominent role in the diagnosis and management of acute myocardial infarction. Its main objective is to enable earlier and more accurate diagnosis, enhance the interpretation of the ECG, accelerate reperfusion times, and ultimately improve patient outcomes. The ECG represents an ideal substrate for the application of deep learning, owing to the vast availability of digital tracings, the association with confirmed diagnoses, and the inclusion of numerous clinical variables. Several systems even allow the automated analysis of photographs

Diabetes mellitus comprises a heterogeneous group of metabolic disorders differing in etiology, clinical course, and outcomes. Traditional classifications, such as type 1 and type 2 diabetes mellitus, fail to capture the full heterogeneity, including variation in insulin deficiency, insulin resistance, and complication burden. To address these limitations, we propose the Diabetes Grade-Stage Classification, an integrated system that combines pathophysiology-based grading with complication-based staging. Grading quantifies metabolic dysfunction through the assessment of insulin deficiency and i

Sepsis is defined as life-threatening organ dysfunction caused by a dysregulated host response to infection. Treatments that influence this dysregulated host response are sparse. The immunopathophysiology of sepsis entails overzealous inflammation causing acute organ dysfunction, as well as a profound and/or persistent anti-inflammatory response that increases susceptibility to secondary infection. The immune response in sepsis is under the influence of various endogenous and exogenous factors, including genetic makeup, age, sex, comorbidities, metabolism, prior microbial exposure and medicati

An international outbreak of Monkeypox (mpox), a zoonotic orthopox virus, was confirmed by the World Health Organization in May 2022. The outbreak represented the first sustained community transmission of mpox beyond West or Central Africa, with speculated causes including declining smallpox vaccination rates, increased international travel, expanding populations, and sexual interactions. This study aimed to assess the understanding and recognition of mpox among sexual medicine experts including the identification of pertained genital lesions. An anonymous electronic survey was developed, addr

Aneurysmal subarachnoid haemorrhage (aSAH) is a rare yet profoundly debilitating condition associated with high global case fatality and morbidity rates. The key determinants of functional outcome include early brain injury, rebleeding of the ruptured aneurysm and delayed cerebral ischaemia. The only effective way to reduce the risk of rebleeding is to secure the ruptured aneurysm quickly. Prompt diagnosis, transfer to specialized centers, and meticulous management in the intensive care unit (ICU) significantly improved the prognosis of aSAH. Recently, multimodality monitoring with specific in

Cytosolic peptide: N-glycanase (PNGase/NGLY1 in mammals) is an amidase (EC:3.5.1.52) widely conserved in eukaryotes. It catalyzes the removal of N-glycans on glycoproteins, converting N-glycosylated Asn into Asp residues. This enzyme also plays a role in the quality control system for nascent glycoproteins. Since the identification of a patient with an autosomal recessive genetic disorder caused by NGLY1 gene dysfunction, known as NGLY1 deficiency or NGLY1 congenital disorder of deglycosylation (OMIM: 615273), in 2012, more than 100 cases have been reported worldwide. NGLY1 deficiency is chara

Despite diagnostic advancements, the development of reliable prognostic systems for assessing the risk of cancer recurrence still remains a challenge. In this study, we developed a novel framework to generate highly representative machine-learning prediction models for oral tongue squamous cell carcinoma (OTSCC) cancer recurrence. We identified cases of 5- and 10-year OTSCC recurrence from the SEER database. Four classification models were trained using the H2O ai platform, whose performances were assessed according to their accuracy, recall, precision, and the area under the curve (AUC) of th

Tumor organoids have been pushed forward as advanced model systems for in vitro oncology drug testing, with the eventual goal to direct personalized cancer treatments. However, drug testing efforts suffer from a large variation in experimental conditions for organoid culturing and organoid treatment. Moreover, most drug tests are restricted to whole-well viability as the sole read-out, thereby losing important information about key biological aspects that might be impacted due to the use of administered drugs. These bulk read-outs also discard potential inter-organoid heterogeneity in drug res

Patients with subarachnoid hemorrhage (SAH) may develop posthemorrhagic hydrocephalus (PHH), which is treated with surgical cerebrospinal fluid (CSF) diversion. This diversion is associated with risk of infection and shunt failure. Biomarkers for PHH etiology, CSF dynamics disturbances, and potentially subsequent shunt dependency are therefore in demand. With the recent demonstration of lipid-mediated CSF hypersecretion contributing to PHH, exploration of the CSF lipid signature in relation to brain pathology is of interest. Despite being a relatively new addition to the omic's landscape, lipi

Gaucher Disease (GD) is a condition resulting from an autosomal recessive inheritance pattern, characterized by a deficiency of the lysosomal enzyme beta-glucocerebrosidase. This leads to the accumulation of glucocerebroside and other glycolipids in multiple tissues, causing damage to various organ systems. The diagnosis of GD can be challenging due to its heterogeneity, non-specific symptoms, and variability across different geographic regions and age groups. Although GD is suspected based on symptoms or signs, the diagnosis is confirmed through the measurement of deficient b-glucocerebrosida

Glycoside hydrolases (GHs) are a class of enzymes with emerging roles in a range of disease. Selective GH inhibitors are sought to better understand their functions and assess the therapeutic potential of modulating their activities. Iminosugars are a promising class of GH inhibitors but typically lack the selectivity required to accurately perturb biological systems. Here, we describe a concise synthesis of iminosugar inhibitors of N-acetyl-α-galactosaminidase (α-NAGAL), the GH responsible for cleaving terminal α-N-acetylgalactosamine residues from glycoproteins and other glycoconjugates. Niemann-Pick disease type C (NPC) is an autosomal recessive neurovisceral disease characterized by progressive neurodegeneration with variable involvement of multisystemic abnormalities. Crohn's disease (CD) is an inflammatory bowel disease (IBD) with a multifactorial etiology influenced by variants in NOD2. Here, we investigated a patient with plausible multisystemic overlapping manifestations of both NPC and CD. Her initial hospitalization was due to a prolonged fever and non-bloody diarrhea. A few months later, she presented with recurrent skin tags and anal fissures. Later, her neurologica

Monitoring allograft function after kidney transplant has routinely relied on the use of nonspecific markers, such as serum creatinine, glomerular filtration rate, proteinuria, and donor-specific antibodies. These traditional markers have low sensitivity and fail to detect subclinical changes. Diagnosis of renal allograft dysfunction still requires an allograft biopsy, as it remains the criterion standard for assessment of graft status. However, renal biopsy is an invasive procedure, and sampling errors may result in misdiagnosis, perhaps causing graft failure. New biomarkers have been develop

OBJECTIVES: Candida auris is an emerging, multidrug-resistant yeast transmitted in healthcare settings. Conventional methods of speciation are unable to identify C. auris to species level. Three methods, namely VITEK® MS v.3.0, VITEK®2 and target sequencing of the internal transcribed spacer (ITS) region and 28S rRNA gene, were evaluated. Antifungal susceptibility testing (AFST) was also performed, and risk factors for acquisition of C. auris candidaemia were studied.
METHODS: Between November 2016 and November 2017, 203 Candida spp. were isolated from blood cultures, of which 11 isolates tha
