Involving people living with brain injuries and communication difficulties in service improvement: an appreciative inquiry approach.

BACKGROUND: Person-centred health communication balances the contributions of service providers and users. However, people living with brain injuries typically find their voices excluded from service developments. Person-centred communication in service development promotes collaboration, self-identity and empowers services to be designed by and for their population. METHODS: Seven people living with acquired brain injury and eight healthcare staff members from two specialist brain injury rehabilitation centres engaged in Appreciative Inquiries to develop their vision for the service and plan

Current status of cardiac rehabilitation among representative hospitals treating acute myocardial infarction in South Korea.

Cardiac rehabilitation services are mostly underutilized despite the documentation of substantial morbidity and mortality benefits of cardiac rehabilitation post-acute myocardial infarction. To assess the implementation rate and barriers to cardiac rehabilitation in hospitals dealing with acute myocardial infarction in South Korea, between May and July 2016, questionnaires were emailed to cardiology directors of 93 hospitals in South Korea; all hospitals were certified institutes for coronary interventions. The questionnaires included 16 questions on the hospital type, cardiology practice, and

Engaging Patients in Setting a Patient-Centered Outcomes Research Agenda in Hematopoietic Cell Transplantation.

The goal of patient-centered outcomes research (PCOR) is to help patients and those who care for them make informed decisions about healthcare. However, the clinical research enterprise has not involved patients, caregivers, and other nonproviders routinely in the process of prioritizing, designing, and conducting research in hematopoietic cell transplantation (HCT). To address this need the National Marrow Donor Program/Be The Match engaged patients, caregivers, researchers, and other key stakeholders in a 2-year project with the goal of setting a PCOR agenda for the HCT community. Through a

Mental health consumers and providers dialogue in an institutional setting: a participatory approach to promoting recovery-oriented care.

OBJECTIVE: This brief report presents the preliminary findings of a participatory project, to answer a question raised by stakeholders in mental health services: How can providers and patients create a process for knowledge exchange to support recovery-oriented care? METHOD: Participatory action research (PAR) and narrative phenomenological methodology guided the selection of methods, which consisted of an iterative process between telling stories and dialoguing about personal values related to recovery. The sample consisted of three occupational therapists, a psychiatrist, an academic-clinic

[Engaging patients with serious mental illness in care services by Critical Time Intervention].

Patients with serious mental illness who are difficult to engage in services often face multiple complex problems, like homelessness, debts, and poor hygiene and social skills. Critical Time Intervention (CTI) is a time-limited, manual-guided case management approach intended to enhance continuity of care for mentally ill patients. We present two patients who received intensive outpatient CTI care. A 42-year-old woman diagnosed with depression and addiction to alcohol and medication and a 38-year-old man diagnosed with autism were offered practical and emotional support while they became linke

How to find non-dependent opiate users: a comparison of sampling methods in a field study of opium and heroin users.

BACKGROUND/AIM: The first aim is to better understand the potentials and limitations of different sampling methods for reaching a specific, rarely studied population of drug users and for persuading them to take part in a multidisciplinary study. The second is to determine the extent to which these different methods reach similar or dissimilar segments of the non-dependent opiate-using population. METHOD: Using ethnographic fieldwork (EFW) and targeted canvassing (TARC; small newspaper advertisements and website announcements), supplemented by snowball referrals, we recruited and interviewed

Engaging Patients in Discussions About Medication Deprescribing.

BACKGROUND: Deprescribing, intentional medication discontinuation or dose reduction, can reduce potentially inappropriate medication use and medication-related harms. Engaging patients in deprescribing discussions may increase likelihood of deprescribing and promote shared decision-making. OBJECTIVE: To examine the impact of patient-directed educational brochures on patient engagement and deprescribing discussions with primary care providers (PCPs). DESIGN: We mailed medication-specific brochures 2 weeks prior to each patient's PCP appointment (4/12/2021-10/7/2022), followed by a mailed surv

Compared with assessment via endoscopic examination, assessment through the examination of resected regional lymph nodes can significantly reduce the mortality rate of lung cancer patients: a retrospective study of 222,563 participants from the SEER database.

BACKGROUND: Lymph node assessment is essential for determining treatment and evaluating the surgical scope in patients with lung cancer, thus significantly impacting survival prognosis. Surgical resection and endoscopic biopsy are the primary methods for obtaining lymph node samples, yet it remains controversial to determine which approach can provide greater benefit. This study aims to analyze the prognosis and survival outcomes of lung cancer patients who undergo assessment through the examination of resected regional lymph nodes, assessment via endoscopic examination, or assessment based on

Selective cutoff reporting in studies of the accuracy of the Patient Health Questionnaire-9 and Edinburgh Postnatal Depression Scale: Comparison of results based on published cutoffs versus all cutoffs using individual participant data meta-analysis.

OBJECTIVES: Selectively reported results from only well-performing cutoffs in diagnostic accuracy studies may bias estimates in meta-analyses. We investigated cutoff reporting patterns for the Patient Health Questionnaire-9 (PHQ-9; standard cutoff 10) and Edinburgh Postnatal Depression Scale (EPDS; no standard cutoff, commonly used 10-13) and compared accuracy estimates based on published cutoffs versus all cutoffs. METHODS: We conducted bivariate random effects meta-analyses using individual participant data to compare accuracy from published versus all cutoffs. RESULTS: For the PHQ-9 (30 s

Selecting, refining and identifying priority Cochrane Reviews in health communication and participation in partnership with consumers and other stakeholders.

BACKGROUND: Priority-setting partnerships between researchers and stakeholders (meaning consumers, health professionals and health decision-makers) may improve research relevance and value. The Cochrane Consumers and Communication Group (CCCG) publishes systematic reviews in 'health communication and participation', which includes concepts such as shared decision-making, patient-centred care and health literacy. We aimed to select and refine priority topics for systematic reviews in health communication and participation, and use these to identify five priority CCCG Cochrane Reviews. METHODS:

Patient and family engagement in the analysis of adverse events in health care: a scoping review protocol.

OBJECTIVES: This review will map patients' and families' engagement in the analysis of adverse events in health care, along with the methods or tools employed in that process of analysis, and the strategies employed to engage patients and their families in the analysis. INTRODUCTION: Patient and family engagement in health care has gained importance, expanding these individuals' roles in identifying and analyzing adverse events. Their involvement may reveal unrecognized failures and improve patient safety. However, evidence on how patients and families are engaged in this process, including m

Shared decision-making in depression: From a generalized ethical imperative to a personalized approach? A narrative review.

BACKGROUND: While both evidence-based medicine as well as personalized patient-centered medicine are glorified, clinical practice seems to be only partially influenced by guidelines or by patients' views. Against this background, shared decision-making (SDM) became a hype but remains an ill-defined concept: it can be considered as an umbrella concept covering therapeutic alliance, physician and patient preferences, information giving, information retrieving, information receiving, patient involvement in goal setting, decision-making, and treatment choice. METHODS: A narrative review based on

Design of a cluster-randomized, hybrid type 1 effectiveness-implementation trial of a care navigation intervention to increase substance use disorder treatment engagement: study protocol.

BACKGROUND: Practical and motivational barriers can deter people from engaging in substance use disorder (SUD) treatment, even those who seek treatment. Care navigation is a psychosocial intervention that seeks to facilitate patients' timely access to care by identifying and intervening upon barriers. Few trials have tested the effectiveness of care navigation when embedding in real-world healthcare, and no trials have studied the process of implementing care navigation into clinical practice. This protocol describes a study that will evaluate whether care navigation can increase treatment eng
