Nine female patients with the rare congenital arteriovenous malformation of the pelvis were treated at New York University Medical Center in the past 8 years. The patients varied significantly in the anatomic location of the arteriovenous malformation, clinical presentation, and natural course of disease. From 1972 to 1986, 14 patients underwent a conservative operation for transitional cell carcinoma of the renal pelvis. Most of these patients had low grade (12), noninvasive (10) tumors involving a solitary functioning kidney (12). The operations performed were open pyelotomy with tumor excision and fulguration (8 patients), partial nephrectomy (5) and percutaneous nephroscopic fulguration (1). There was 1 operative death. A systematic detection of endocrine cells in the renal pelvis and ureter was carried out, using Grimelius stain and immunohistochemical techniques. Ninety specimens of pelvic and ureteral mucosae were investigated. Throughout the pelvic urothelium, endocrine cells were very uncommon, patchily distributed, and serotonin-storing. A case of hyperkeratotic squamous epithelium of the distal renal pelvis and ureter, clinically identifiable as leukoplakia, is reported in a solitary kidney. Because of the need to conserve renal parenchyma in this patient, the lesion was treated conservatively at operation after careful exploration and full thickness biopsy failed to reveal malignancy. Close monitoring of this potentially premalignant lesion has documented no progression at twenty-five months.

Sixteen patients with transitional cell carcinoma of the renal pelvis seen at our Hospital between December 1973 and February 1984 were reviewed and the diagnostic tools were evaluated. The patients (11 males, 5 females) ranged in age from 35 to 81 years (mean 63.8). Ten tumors were found on the left side and 6 on the right side. The most frequent symptom was macrohematuria (93.8%). We studied 20 patients with lumbosacral radiculoplexopathy from radiation treatment and 30 patients with plexus damage from pelvic malignancy. Indolent leg weakness occurred early in radiation disease, whereas pain marked the onset of tumor plexopathy. Eventually, all radiation cases had weakness, which was bilateral in most of them and painless in one-half of them. Tumor patients typically had unilateral weakness, which was painful in all of them. Unilateral hydronephrosis was induced by temporary ligature of the left ureter in 29 rabbits. In 21 animals so treated, chronic pyelonephritis was simultaneously induced by intravenous application of a suspension of E. coli. Autopsy reports on urothelial tumors of the renal pelvis and ureter in 602 cases were analyzed to investigate the mode of metastasis. Seventy one cases, or 12%, showed no metastasis. Triple osteotomy of the pelvis was performed on 9 dysplastic hip joints in 5 dogs. Acetabular redirection and fixation were unopposed by muscular forces, and bony union was achieved in all cases without complications. Two months postoperatively all hip joints were clinically and radiographically stable.
