Management and control of communicable diseases in schools and other child care settings: systematic review on the incubation period and period of infectiousness.

BACKGROUND: Information on the incubation period and period of infectiousness or shedding of infectious pathogens is critical for management and control of communicable diseases in schools and other childcare settings. METHODS: We performed a systematic literature review (Pubmed and Embase) to identify and critically appraise all relevant published articles using incubation, infectiousness or shedding, and exclusion period as parameters for the search. No language, time, geographical or study design restrictions were applied. RESULTS: A total of 112 articles met the eligibility criteria. A r

Toxoplasma Transmission to the Fetus during the Periconceptional Period.

In Tunisia, toxoplasmosis screening in pregnancy is common but not regulated. This study evaluated congenital toxoplasmosis after primary infections during the periconceptional period and the accuracy of diagnostic tools. We analyzed 209 women at the Pasteur Institute of Tunis (2005-2021) with primary infections classified as periconceptional (n = 50, 23.9%), first trimester (n = 121, 57.9%), or second trimester (n = 38, 18.2%). Congenital toxoplasmosis was confirmed in 29 cases (13.9%), diagnosed prenatally in 22 cases, and diagnosed postnatally in 7 cases. Transmission rates were 14%, 8.2%,

The Critical Period Microbiota Shape Brain Plasticity.

The gut microbiota is increasingly recognized as a regulator of brain function, yet its role in experience-dependent plasticity during postnatal development remains largely unknown. Here, we show that disrupting the gut microbiota with antibiotics during critical periods of visual cortex development impairs ocular dominance plasticity (ODP) in juvenile mice. Antibiotic treatment induces marked changes in microbial community composition and is accompanied by extensive transcriptional remodeling of the visual cortex, including pathways involved in extracellular matrix organization, blood-brain b

Genomic profiles of the Khazar-Period inhabitants of Phanagoria.

The Phanagoria settlement, located on the Taman Peninsula, is a unique archaeological site. The history of the ancient city spans approximately 15 centuries, making Phanagoria a witness to many historical events, including those related to the early history of the Old Rus' state. The early medieval period of Phanagoria's history is of particular interest due to its insufficient study. The Phanagoria Expedition of the Institute of Archaeology of the Russian Academy of Sciences, in collaboration with the Phanagoria Museum-Reserve, studied the city's cultural strata from the 7th-10th centuries. Susac syndrome with uneventful pregnancy and postnatal period.

We report the follow-up of a previously published case of Susac syndrome (SuS) in a woman who underwent fertility-preserving treatment with gonadotropin-releasing hormone agonists during cyclophosphamide therapy. Four years after initial disease control, she conceived spontaneously following withdrawal of mycophenolate and went on to have an uneventful pregnancy and postpartum period, delivering a healthy baby. At the time of writing, approximately 2 years after delivery, she remains symptom-free. While the majority of previously reported cases demonstrate adverse pregnancy outcomes in SuS, th

An Idiopathic Hepatic Hematoma Presenting in the Neonatal Period.

Hepatic hematoma in newborns is an extremely rare condition, usually secondary to birth trauma or a vascular malformation. Clinically, it is often asymptomatic but can lead to serious complications, including internal bleeding and even signs of liver failure. We report an unusual case of intraparenchymal hepatic hematoma of undetermined etiology in a newborn, without associated birth trauma. This is a male newborn, born at term, admitted on the third day of life for respiratory distress associated with pallor of the skin and mucous membranes and abdominal distension with scrotal and inguinal e

Intermittent hydrarthrosis (periodic synoviosis): A narrative review.

Intermittent hydrarthrosis (IH), also termed periodic synoviosis or periodic benign synovitis, is a rare cause of recurrent, self-limited joint effusion-most frequently involving the knee-with striking periodicity and complete intercritical recovery. Despite being recognized for decades, IH remains underdiagnosed and is often mistaken for palindromic rheumatism, crystal-induced arthritis, septic arthritis, proliferative synovial disorders, and autoinflammatory syndromes. Historical descriptions emphasized a benign, non-destructive course, minimal systemic inflammation, and a lack of specific b…

Critical period plasticity enables credit assignment.

Synaptic plasticity is often guided by instructive inputs to neural circuits, but learning only succeeds when these instructions reach neurons that mediate relevant outputs. This creates the credit assignment problem: how does a neuron receive instructions suited to its own behavioral function? Here we report a developmental mechanism that enables credit assignment by using instructive inputs to organize the downstream architecture through which learning is expressed. In the olivocerebellar learning system, the inferior olive provides instructive inputs that guide plasticity within the cerebel

Restless Legs Syndrome and Periodic Limb Movements.

OBJECTIVE: This article describes the diagnostic criteria of restless legs syndrome (RLS) and periodic limb movements. The pathophysiology of RLS and its management are discussed. The approach to address RLS augmentation is also reviewed. LATEST DEVELOPMENTS: In the 2025 American Academy of Sleep Medicine RLS treatment guidelines, long-term dopamine agonist therapy for RLS was downgraded because of the accumulating evidence of augmentation. In this context, it is important to optimally use IV iron therapy in patients with RLS. Ferric carboxymaltose is more likely to cause hypophosphatemia com

Burden of fatigue in cryopyrin-associated periodic syndromes.

OBJECTIVES: Fatigue in cryopyrin-associated periodic syndromes (CAPS) often persists despite controlling inflammation. This study explores fatigue burden, its link to disease activity, and factors tied to chronic fatigue in autoinflammatory conditions. METHODS: A single-centre study (2007-2024) of children and adults with CAPS analysed demographics, clinical data, inflammation, treatment, disease activity, and fatigue scores. Relationships between these factors and predictors of chronic fatigue were assessed. RESULTS: A total of 108 patients with CAPS were included; 53 were female (49%). Med

Urinary incontinence during pregnancy and in the postpartum period.

Pregnancy and childbirth involve anatomical and physiological changes in the pelvic floor that can negatively affect the urinary-continence mechanism. Thus, in both periods, the rates of urinary incontinence increase notably, with stress urinary incontinence being the most common type. The pathophysiological mechanisms involved in urinary incontinence in pregnancy and postpartum are not fully understood. Stress urinary incontinence is more closely linked to urethral sphincter deficiency than to impaired urethral support, although both conditions have been noted during pregnancy and postpartum.

Effects of burnout syndrome on pregnancy and the postpartum period.

OBJECTIVE: The aim of this study was to examine the prevalence of burnout among pregnant working individuals and its effects on both obstetric complications and issues that may arise in the early postpartum period. METHODS: Healthy pregnant women were questioned regarding their working life and Maslach Burnout Inventory. They were divided into two groups-burnout and non-burnout-according to the burnout score. The two groups were compared in terms of pregnancy and postpartum complications and breastfeeding. RESULTS: Burnout was identified in 164 pregnant women (54.1%). The age of the women wi

Molar pregnancy in the perimenopausal period: a case report.

Molar pregnancy is an uncommon diagnosis in perimenopausal women and poses a high risk of complications, including hemorrhagic shock and malignant transformation. We report the case of a 51-year-old woman who presented with severe abdominal pain and profuse vaginal bleeding. Imaging revealed a markedly enlarged uterus with a large intracavitary mass, and serum β-hCG levels were elevated. The patient developed hemorrhagic shock and underwent an emergency hemostatic hysterectomy. Histopathological examination confirmed a complete hydatidiform mole with early invasive features, without choriocarc

Hypokalemic Paralysis Is Not Always Periodic: A Case Series.

Potassium is vital for cellular function, particularly in excitable tissues like nerves and muscles, which rely on potassium gradients to function normally. Hypokalemia can lead to severe issues such as muscle weakness and irregular heart rhythms. This case series presents four instances of hypokalemic paralysis, a neuromuscular condition that can be either periodic or isolated. The cases vary in presentation and treatment approach, highlighting the need to differentiate between primary and secondary causes of hypokalemia for optimal management. Fast and accurate diagnosis through laboratory t

Spontaneous Pneumomediastinum in the Postpartum Period: The Macklin Effect.

Spontaneous pneumomediastinum (SPM) in the postpartum period is defined by the presence of air within the mediastinum in the absence of any apparent traumatic or iatrogenic cause. We report the case of a 32-year-old woman who developed SPM in the immediate postpartum period. Her medical history was notable only for a previous miscarriage; she had no personal or family history of asthma, atopy, or other respiratory disease. Clinically, the patient presented with acute retrosternal chest pain and sudden-onset dyspnea, accompanied by a single episode of mild hemoptysis following prolonged labor.

Thyrotoxic Hypokalemic Periodic Paralysis: Pathophysiological Mechanisms.

Thyrotoxic hypokalemic periodic paralysis (THPP) is a rare but potentially fatal complication of thyrotoxicosis, characterized by transient episodes of muscle weakness in the setting of hypokalemia and underlying hyperthyroidism. Although thyrotoxicosis is more common in females, THPP predominantly affects males, especially in Asian populations, in which its prevalence is notably higher. Early recognition is essential to prevent serious complications such as cardiac arrhythmias and respiratory failure; however, THPP is frequently misdiagnosed, particularly in Western countries, due to clinical

Thyrotoxic periodic paralysis: diagnostic and management considerations.

Thyrotoxic periodic paralysis is a rare but potentially life-threatening complication of thyrotoxicosis characterised by hypokalaemia-induced flaccid paralysis due to an intracellular potassium shift rather than total body depletion. We report a male in his early 40s of Filipino origin who awoke with acute bilateral lower-limb weakness after he had consumed a large carbohydrate meal and performed strenuous exercise within the preceding 12 hours. Venous blood gas was normal; initial serum potassium was 1.7 mmol/L with phosphate 1.07 mmol/L. The ECG showed sinus tachycardia, first-degree atriove

Fulminant Celiac Disease Presenting in the Postpartum Period.

Celiac disease (CD) is an immune-mediated disorder of the small intestine triggered by dietary exposure to gluten in genetically susceptible individuals. Adult CD usually has an insidious onset with gastrointestinal symptoms, most often diarrhea and weight loss. The association between CD and reproductive abnormalities has been well described in the literature, but it is uncommon for CD to initially manifest during pregnancy or the postpartum period. We report a case of adult CD in a previously healthy woman with a life-threatening presentation during the postpartum period.
