Leuconostoc species are rarely pathogenic in humans, but may cause infection in patients at risk. A 7-year-old girl with p-ANCA-positive crescentic glomerulonephritis, treated with peritoneal dialysis, developed peritonitis due to Leuconostoc species. She had a history of treatment with vancomycin and a brief course of immunosuppressive therapy. The peritonitis responded well to ampicillin therapy. To date, only 47 cases of Leuconostoc infection, including our patient, have been reported in the medical literature; 25 of the cases occurred in children. Only 1 prior case has been reported in the…

Processing of bone allografts with strong acids and γ-sterilization results in decreased biomechanical properties and reduction in osteogenecity and osteoconductivity. High hydrostatic pressure (HHP) treatment could be a gentle alternative to processing techniques usually applied. HHP is known to induce devitalization of cancellous bone while preserving biomechanical stability and molecules that induce cell differentiation. Here, a specific HHP protocol for devitalization of cancellous bone was applied to rabbit femoral bone. Allogeneic bone cylinders were subsequently implanted into a defect

BACKGROUND: Although various inflammatory mediators have been previously shown to be released into the peritoneal cavity during peritonitis in peritoneal dialysis patients, those that are involved in governing changes in peritoneal permeability to small solutes and protein remain incompletely defined.
METHODS: We determined the importance of prostanoid production in the enhanced protein loss observed during acute peritonitis by inhibition experiments using indomethacin, an inhibitor of cyclooxygenase activity. The association between changes in peritoneal permeability and the generation of in…

Most common complications of peritoneal dialysis are those related to anatomic defect of the abdominal wall and peritoneum. The aim of this … toneal cavity with the abdominal ostium of tuba uterina was found not to be an important factor for peritonitis.

We report the uncommon occurrence of ventriculo-peritoneal shunt-associated peritoneal metastases in two patients with pineoblastoma. In one patient, the peritoneal cavity was the only site of recurrence; there was no evidence of disease recurrence in the central nervous system. One other patient with recurrent intracranial disease had synchronous, but asymptomatic, peritoneal metastases which were detected on an elective ultrasound. Although rare, peritoneal metastases appear to respond well to systemic chemotherapy. Ultrasound surveillance of the abdomen should be considered as a part of the

We present a 13-year-old girl with Arnold-Chiari syndrome and uremia secondary to neurogenic bladder. She had been treated with continuous ambulatory peritoneal dialysis (CAPD) for 13 months prior to the development of peritonitis. The patient demonstrated no improvement with a 3-day therapy of intraperitoneal vancomycin and netilmicin. Meanwhile, smear of centrifuged dialysate revealed acid fast bacilli on two occasions. We, then, started anti-TB therapy with oral isoniazid (INAH), rifampin and ethambutal. The symptoms subsided within three days. In the first week, the patient lost her perito…

An uraemic patient under intermittent peritoneal dialysis was found in the 9th month of treatment to present ultrafiltration drop and increase of low-molecular weight substances. Two instances of laparotomy for heavy abdominal bleeding revealed peritoneal thickening, laminar intestinal concrescence and diffuse haemorrhage. The patient died in the 13th month of treatment under symptoms of ileus. Other reasons excluded, sclerosing peritonitis was attributed to the formalin solution employed as disinfectant for the abdominal catheter. The causative factors are listed together with the chances of …

A 48-year-old man receiving maintenance hemodialysis for 3 years and continuous ambulatory peritoneal dialysis for 1 year developed a clinical picture compatible with peritonitis. Three successive fluid cultures were negative, and only after filtration of a large volume of peritoneal fluid a fungus identified as a Rhizopus sp was isolated in cultures of the filtering devices. The same fungus was also isolated from the peritoneal catheter cuff. Intravenous amphotericin B was administered and both the abdominal and general conditions of the patient improved transiently. Twenty days after initiat…

We report a case of peritonitis in a patient on continuous ambulatory peritoneal dialysis caused by Listeria monocytogenes. The patient was taking oral corticosteroids for underlying systemic lupus symptoms, and had classic signs and symptoms of peritonitis. There was initial clinical deterioration while in vitro data indicating that the organism was sensitive to vancomycin with a minimum inhibitory concentration of 0.5 micrograms/ml. The patient rapidly responded to intravenous ampicillin. We believe this to be the second reported case of L. monocytogenes peritonitis in a CAPD patient.

This study investigated the possibility that the peritoneum is capable of synthesizing phosphatidylcholine (PC), a lubricant surfactant, in an amount similar to that produced by pulmonary alveoli. The synthesis of PC by rat lung (positive control), liver (negative control), and transparent mesentery (test tissue) was determined by in vitro incubation of these tissues in the presence of (methyl-14C) choline chloride for three hours at 37 degrees C in Warburg flasks. All lipid material present in tissue and incubation media was extracted by the Folch technique. Carrier egg PC was added to each s

Candida albicans peritonitis developed in a 48-year-old man with a perforated gastric ulcer who subsequently was treated with intravenous amphotericin B. The drug penetrated well into the inflamed peritoneal cavity and eradicated the organism from the peritoneal fluid. Nevertheless, at post-mortem, Candida organisms were demonstrated in a gall-bladder empyema and within the gall-bladder wall. Because intra-abdominal organs may be involved in Candida peritonitis, the use of high dose amphotericin B administered either intravenously, intraperitoneally, or both intravenously and intraperitoneally…

A series of 30 cases of post-operative peritoneal irrigation dialysis for generalised peritonitis. In the light of peritoneal physiology, details are given of the technique for insertion of the peritoneal drains as well as the composition of the dialysate. In addition to its mechanical role, continuous lavage of this type is useful in restoring and maintaining water and electrolyte balance, the limitations of which are described, together with a direct anti-infectious action and temperature equilibration. P.I.D. seems to be associated with a considerable reduction in mortality from acute gener…

Introduction Acute secondary peritonitis due to hollow viscus perforation is a life-threatening surgical condition with significant morbidity and mortality, depending on the severity with outcomes that differ in the Western and developing world. Various scoring systems have been developed to assess the severity and its relation to morbidity and mortality. We conducted this study to evaluate the role of the Mannheim peritonitis index (MPI) in predicting outcomes in perforation peritonitis patients in a rural hospital in India. Materials and methods A prospective study of 50 patients with hollow…

BACKGROUND: The outcomes of culture-negative peritonitis in peritoneal dialysis (PD) patients have been reported to be superior to those of culture-positive peritonitis. The current study aimed to examine whether this observation also applied to different subtypes of culture-positive peritonitis.
METHODS: This multicentre registry study included all episodes of peritonitis in adult PD patients in Australia between 2004 and 2014. The primary outcome was medical cure. Secondary outcomes were catheter removal, hemodialysis transfer, relapsing/recurrent peritonitis and peritonitis-related death. …

BACKGROUND: Rhodococcus infections are exceedingly rare and largely reported in immunocompromised hosts, with no consensus on optimal therapy due to limited evidence.
CASE PRESENTATION: This article reports for the first time a case of peritoneal dialysis-associated peritonitis caused by Rhodococcus pyridinivorans in a patient with diabetic nephropathy. The dialysate leukocyte count declined substantially with combination antimicrobial therapy, but complete normalization occurred only after replacing icodextrin with a glucose-based dialysate.
CONCLUSIONS: These findings support the use of co…
