Left Upper Lobe Lung Cancer With Swyer-James-MacLeod Syndrome.

Swyer-James-MacLeod syndrome (SJMS) is characterized by unilateral hyperlucency of the lung on chest roentgenogram, unilateral reduction in vascularity on chest computed tomography, and unilateral loss of perfusion on pulmonary ventilation/perfusion scan. The prevalence of SJMS is reported to be 0.01%, with most cases occurring in children and only a few cases in adults. Therefore, SJMS associated with primary lung cancer is extremely rare. We present a case of left upper lobe lung cancer with SJMS by performing robot-assisted thoracoscopic left pneumonectomy.

Wedge extended bronchoplasty with caliber adjustment by membranous suture.

Extended bronchoplasty for the left lower lobe lung tumors with interlobar lymph node involvement is a useful surgical technique for avoiding pneumonectomy. Typically, sleeve bronchoplasty, in which the superior division bronchus and the left main bronchus are separated and anastomosed, is chosen due to the difference in caliber of the anastomosis; herein, we report a wedge extended bronchoplasty in which the superior division bronchus and the left main bronchus were not completely separated. The main point of this technique is to adjust the difference in caliber by suturing the main bronchial…

A case of surgically resected lung cancer in a patient with Kartagener's syndrome.

Kartagener's syndrome is a rare inherited disorder with a triology of symptoms (bronchiectasis, sinusitis and situs inversus) and is also associated with abnormalities of the cilia of the respiratory epithelium. Lung cancer arising in Kartagener's syndrome is very rare and to date only 5 cases have been reported in the English and Japanese literature. We report on a case of a 65-year-old Japanese male Kartagener's syndrome patient with squamous cell carcinoma of the lung. A left pneumonectomy was performed and no recurrence was found within 2 years.

Primary endobronchial schwannoma.

Primary intrabronchial neurilemmas, also called schwannomas, are extremely rare tumors that originate from Schwann cells. We report a case of intrabronchial schwannoma arising in the left main bronchus in an 8-year-old child. The diagnosis was made by bronchofiberscopic biopsy. Although the tumor was benign, the patient was subjected to left pneumonectomy because of destruction of the pulmonary parenchyma distal to the obstruction. The diagnosis of intrabronchial schwannoma was supported pathologically with characteristic findings on Antoni A and Antoni B areas. This case is unique in that it …

Endotracheobronchial neurofibromas.

Among benign tracheobronchial neoplasms, neurofibromas of neurogenic origin are exceedingly rare. In a search world literature, only 23 cases of endotracheobronchial neurofibromas in 21 series were found. We report another case of a 52-year-old man who presented to our hospital with the symptoms of intermittent productive cough and fever. Bronchoscopy revealed a round tumor obstructing the lumen of the right main bronchus. Right sleeve pneumonectomy was performed, and neurofibroma was confirmed by pathologic examination. The literature is reviewed.

Postoperative changes in a bronchial stump following covering with free fat tissue in a rat model.

OBJECTIVES: Covering the bronchial stump with free fat tissue has been used as minimally invasive prophylaxis against bronchial stump fistulas; however, postoperative changes in the bronchial stump have not been well validated. Our goal was to examine changes in the bronchial stump in response to covering with free fat tissue in a rat model. METHODS: A left pneumonectomy was performed on 16 Wistar/ST rats, 12 of which had a bronchial stump covered with free subcutaneous fat tissue. Four rats that underwent a left pneumonectomy alone were sacrificed on postoperative day 7, and the 12 rats whos…

Wedge bronchoplastic lobectomy for non-small cell lung cancer as an alternative to sleeve lobectomy.

OBJECTIVES: Sleeve lobectomy was introduced for patients with lung cancer whose pulmonary reserve was inadequate for pneumonectomy. However, the safety and survival benefits of wedge bronchoplastic lobectomy as an alternative to sleeve lobectomy have not been thoroughly studied. This study was performed to evaluate the safety and oncologic results of wedge bronchoplastic lobectomy for lung cancer. METHODS: We retrospectively analyzed 191 patients who underwent wedge bronchoplastic lobectomy and mediastinal lymph node dissection from 2001 to 2009. RESULTS: There were 174 male patients with a …

Obstructing endobronchial fibrous histiocytoma: potential for lung salvage.

In the past 10 years two children, ages 3 1/2 and 7 years, have been seen with central endobronchial fibrous histiocytoma causing complete atelectasis. Each child had a 4- to 6-month history of cough, dyspnea and fever. After antibiotic failure, x-rays demonstrated complete unilateral lung collapse. Bronchoscopy in case 1 demonstrated total occlusion of the left mainstem bronchus with 80% stenosis of the right. Left pneumonectomy was ultimately performed with removal of tumor frm the right bronchus. Ten years later the patient is well. Case 2 had an extrinsic etiology ruled out by computed tom…

Prospective evaluation of mediastinoscopy for assessment of carcinoma of the lung.

Between 1979 and 1984, mediastinoscopy was performed on 1,000 of the 1,500 patients admitted to the Thoracic Surgical Service of the Toronto … Eighty-six percent had resectable lesions (67% curative, 18% palliative) and 14% unresectable. The pneumonectomy rate in this group was 35%. These current data support our previous opinion that routine mediastinoscopy can be done with negligible morbidity and provides essential information for the classification and management of cancer of the lung.

Unilateral absence of pulmonary artery and ventricular septal defect in an infant.

A 2-month-old boy was diagnosed as having a rare combination of absence of left pulmonary artery and ventricular septal defect. He underwent intracardiac repair at the age of 8 months. Reconstruction of the left pulmonary artery was impossible because it could not be visualized through a median sternotomy. Although his early postoperative course was uneventful, he needed ipsilateral pneumonectomy 3 months later because of unremitting hemoptysis. Hilar left pulmonary artery, which used to exist, was not found even by histologic examination.

Robotic left transthoracic left main bronchial sleeve resection with preoperative three-dimensional planning.

Robotic left main bronchial sleeve resection for endobronchial tumors avoids pneumonectomy while preserving the entire ipsilateral lung parenchyma. Preoperative three-dimensional computed tomography reconstruction guides dissection around the pulmonary artery, pulmonary veins, and left atrium, and Firefly fluorescence imaging enables precise intraoperative tumor localization without needle puncture. We hereby describe a patient with a typical carcinoid tumor of the left main bronchus, who successfully underwent a minimally invasive lung -preserving operation to achieve negative surgical margin…

Diode Laser in Management of Tracheobronchial Typical Carcinoid: A Case Report with Review of Literature.

Tracheobronchial carcinoids are relatively uncommon neoplasms with malignant (though indolent) potential and variable behavior. Clinical presentation is usually similar to any obstructive tracheobronchial lesions or rarely with presentation of carcinoid syndrome. Management varies from Pneumonectomy to use of lung sparing bronchoscoplastic techniques. We report the case of a 32-year-old male patient diagnosed with carcinoid of left main bronchus. Patient was successfully treated with trans-bronchoscopic diode laser. At 24 months of follow up patient remains asymptomatic and recurrence-free.

Endobronchial malignant lymphoma: unusual manifestations--a report of two cases.

Two unusual manifestations associated with endobronchial malignant lymphoma are reported. A 58-year-old white female was first seen with massive hemoptysis requiring blood transfusion and pneumonectomy for histiocytic lymphoma (large cell type). The second case was that of a 47-year-old black male treated previously for poorly differentiated lymphocytic lymphoma (medium-sized cell type) who had a relapse in the lungs. Increasingly copious sputum production led to respiratory distress which required extensive radiation therapy and chemotherapy for its control.

Squamous Cell Carcinoma of the Lung Masquerading as Pulmonary Mucormycosis.

Pulmonary mucormycosis is a rare fungal infection that often affects the lungs in immunocompromised or diabetic patients. We describe the case of a 65-year-old man presenting with cavitations and cultures positive for Rhizomucor and undergoing right pneumonectomy. Final pathologic examination of the lung lesions revealed keratinizing squamous cell carcinoma with concomitant Mucor infection. The patient was successfully treated with no evidence of disease recurrence at last available follow-up. Here we discuss the case, other unusual presentations of mucormycosis, and current guidelines for tre…

Management of Postpneumonectomy Bronchopleural Fistula: From Thoracoplasty to Transsternal Closure.

Development of postpneumonectomy empyema with bronchopleural fistula is a life-threatening condition that requires prompt action. Although measures should be taken to prevent bronchopleural fistula at time of pneumonectomy, many patients experience this complication. Management focuses on drainage of the pleural space, control of the pleural infection including repair of the bronchopleural fistula, and obliteration of the residual pleural cavity. Multiple techniques and procedures have been developed over time to achieve these goals. Knowledge of the diverse therapeutic options is important to…

Bronchopleural Fistula and Empyema After Anatomic Lung Resection.

Empyema after anatomic lung resection is rare but causes serious morbidity, particularly if associated with a bronchopleural fistula. Careful assessment of preoperative risk factors and proper surgical technique can minimize risks. Empyema after segmentectomy or lobectomy may respond to simple drainage and antibiotics, or may require decortication with or without muscle transposition. After pneumonectomy, treatment principles include initial drainage of the intrathoracic space, closure of the fistula if present, and creation of an open thoracostomy, which is packed and later closed. Success ra…

[A rare etiology of massive haemoptysis: a case description].

We present here a case of severe haemoptysis occurring 2 times in a man having some anomaly of lung development, i.e. a right pulmonary artery agenesis. We ought to keep in mind these anomalies of lung development which are quite rare. Moreover in absence of pulmonary artery, the ipsilateral lung parenchyma is vascularized by systemic circulation and exposed to very high systemic pressure responsible for very severe haemoptysis requiring sometimes a pneumonectomy to be controlled. Finally we make a short review on etiology of lung haemoptysis.
