Giant colonic diverticulum is an uncommon complication of diverticulosis, with only 52 previously reported cases. Complications secondary to the giant air cysts occurred in only six instances, and all but one occurred in the sigmoid colon. Two additional cases are described, presenting atypical location, age, and complication by pneumoperitoneum.

An 86-year-old woman with a tracheostomy developed airway obstruction caused by viscid sputum. Forceful manual ventilation through her obstructed airway precipitated tension pneumothorax, pneumomediastinum, pneumopericardium, pneumoperitoneum, and pneumoretroperitoneum. The changing pattern of hyaline membrane disease and its iatrogenic complications during respiratory treatment are reviewed. The typical roentgenologic symptoms of pulmonary interstitial emphysema, pseudocyst, pneumomediastinum, pneumoperitoneum, pneumothorax, pneumopericardium, bronchopulmonary dysplasia and pulmonary heamorrhage are described and illustrated. Their relevance for clinical management is discussed.

A rare case combining pneumothorax, pneumomediastinum, pneumopericardium, pneumoperitoneum, pneumorrhachis, air in retroperitoneum and extensive subcutaneous emphysema simultaneously in a severely anorectic male with BMI 9.2 (22.8 kg) and multiple vomitings is presented. A case of a male newborn infant with spontaneous perforation of Meckel's diverticulum is reported. The clinical course consisted of delayed occlusive disorders and bloody stools following 36 h of normal gastrointestinal transit, without pneumoperitoneum and infection.
