Navigating the Road to Immunization Equity: Systematic Review of Challenges in Introducing New Vaccines into Sub-Saharan Africa's Health Systems.

Background/Objectives: Over the past 50 years, developing new vaccines has been pivotal in responding to emerging and re-emerging diseases globally. However, despite substantial partner support, introducing new vaccines in sub-Saharan Africa remains challenging. This systematic review documents the barriers to new vaccine introduction in sub-Saharan Africa by distinguishing between vaccines integrated into routine immunization programs and those introduced primarily for outbreak response. Methods: A comprehensive electronic search was conducted across five databases for articles published in E

Reframing Dutch drug policies: a new era for harm reduction.

In this article the authors offer their perspective on the changes in the Dutch harm reduction field. From the 1970s to the 1990s, the Netherlands emerged as a leader in harm reduction services, driven by grassroots movements like the Medisch-sociale Dienst Heroïne Gebruikers (MDHG) (Medisch-sociale Dienst Heroïne Gebruikers (MDHG) translates to Medical-Social Service Heroin Users in English) in Amsterdam and Junkiebond in Rotterdam. These organisations advocated for health-centred policies, initiated needle exchange programmes, and created safe consumption spaces. Their efforts led to signifi

The emergence of custodial health nursing as a specialty whose time has come: An Australian experience.

BACKGROUND: Nurses provide healthcare in prisons worldwide. Working within security restraints, in environments not designed for nursing care, custodial health nurses (CHNs) use specialist nursing skills and knowledge to do essential work. Rapid increases in prisoner age, infirmity and ill-health of prisoners mandate their access to these nurses. AIM: To raise awareness of the CHNs struggle for specialty status within the nursing profession, public health frameworks and prisons. SOURCES OF EVIDENCE: Publicly available information is organised and analysed through the author's lens of 20 year

Impact of the GeneXpert MTB/RIF Technology on Tuberculosis Control.

Molecular technology revolutionized the diagnosis of tuberculosis (TB) with a paradigm shift to faster, more sensitive, clinically relevant patient care. The most recent molecular leader is the GeneXpert MTB/RIF assay (Xpert) (Cepheid, Sunnyvale, CA), which was endorsed by the World Health Organization with unprecedented speed in December 2010 as the initial diagnostic for detection of HIV-associated TB and for where high rates of drug resistance are suspected. South Africa elected to take an aggressive smear replacement approach to facilitate earlier diagnosis and treatment through the decisi

Gender differences in the structural connectome of the teenage brain revealed by generalized q-sampling MRI.

The question of whether there are biological differences between male and female brains is a fraught one, and political positions and prior expectations seem to have a strong influence on the interpretation of scientific data in this field. This question is relevant to issues of gender differences in the prevalence of psychiatric conditions, including autism, attention deficit hyperactivity disorder (ADHD), Tourette's syndrome, schizophrenia, dyslexia, depression, and eating disorders. Understanding how gender influences vulnerability to these conditions is significant. Diffusion magnetic reso

Mainstreaming nutrition into maternal and child health programmes: scaling up of exclusive breastfeeding.

Interventions to promote exclusive breastfeeding have been estimated to have the potential to prevent 13% of all under-5 deaths in developing countries and are the single most important preventive intervention against child mortality. According to World Health Organization and United Nations Children Funds (UNICEF), only 39% infants are exclusively breastfed for less than 4 months. This review examines programme efforts to scale up exclusive breastfeeding in different countries and draws lesson for successful scale-up. Opportunities and challenges in scaling up of exclusive breastfeeding into

Does social-economical transformation influence the incidence of type 1 diabetes mellitus? A Polish example.

BACKGROUND: The dramatic increase in the incidence of diabetes mellitus type 1 (DMT1) in all countries of Central and Eastern Europe seems to be correlated with the rapid transformations of the political, economical, and social conditions. The aim of this study was to analyze the increase of DMT1 incidence vs. changes of parameters describing economical conditions, medical care standards, and level of hygiene. MATERIALS AND METHODS: The study was based on the Upper Silesia, Poland, prospective register of DMT1 cases, a part of the EURODIAB program. The analyzed parameters were number of salmo

GOBI versus PHC? Some dangers of selective primary health care.

This article enters the debate concerning comprehensive versus selective primary health care by focussing on UNICEF's 'child survival revolution'. It is argued that UNICEF is dangerously mistaken in believing that its present emphasis on selective primary health care is a precursor or 'leading edge' of comprehensive primary health care. The approach of UNICEF--diffusion of a package of technologies by campaigns organized from the top down--is more likely to undermine the social basis for comprehensive care. The kinds of implementation UNICEF has chosen in order to minimize costs and maximize i

An algorithmic system for arabic fake news detection using neural networks and transformer embeddings with class weighting.

Modern communication technologies have increased the speed of news dissemination. Information can now reach wide audiences almost instantly. At the same time, these technologies can reduce traditional verification mechanisms, contributing to the accelerated spread of false information and posing potential societal and political challenges. While research on fake news detection has increased, Arabic remains under-explored due to limited datasets and the language’s morphological and semantic complexity. We compiled a multi-source Arabic news dataset consisting of 7,474 articles. The dataset was

The Curriculum of Resistance: Medical Education as a Critical Line of Defense Against Policy Disaster for Transgender and Gender Nonconforming Individuals.

Transgender and gender nonconforming (TGNC) individuals in the United States have faced escalating legislative hostility for more than a decade, with a marked acceleration following the legalization of marriage equality in 2015. Federal executive actions and state-level policies enacted in early 2025 represent an unprecedented escalation of this trend, sharply restricting access to gender-affirming care, legal recognition, and diversity, equity, inclusion, and belonging (DEIB) protections. Collectively, these actions constitute a policy-driven public health disaster with profound and potential

Rethinking Public Health Approaches to Crisis Response: Putting People First.

This commentary is written by the co-editors of the Collection entitled "Rethinking Public Health Approaches to Crisis Response: Putting People First". Specifically, in the call for submissions, we welcomed articles that detailed the actions, experiences, models, and mechanisms of involvement of community members and local knowledge in response to any clearly defined public health crisis, as a manifestation of how health systems are "putting people first". This commentary is a reflection on the seven papers eventually accepted for that Collection. The papers cover diverse settings, from South

Managing an infodemic in the global south: a systematic evidence synthesis from COVID-19 social listening reports.

The COVID-19 pandemic was accompanied by a widespread infodemic characterised by rapidly evolving information, misinformation, and uncertainty, particularly within low- and middle-income country contexts. Social listening emerged as a critical tool for risk communication and community engagement by enabling real-time monitoring of public concerns, misinformation, and information gaps. This study presents a systematic evidence synthesis of national COVID-19 social listening reports produced in South Africa between 2021 and 2023 to inform infodemic management in the Global South. Prioritising in a crisis: what next for malaria research?

Global malaria control is entering a period of profound uncertainty, with earlier gains now stalling or reversing amid a rapidly deepening shortfall in programme and research funding. At the same time, parasite and vector resistance, changing transmission patterns driven by climate and urbanisation, and growing conflict and displacement are reshaping malaria epidemiology and undermining the effectiveness of core tools. In this context, there is an urgent need to reconsider how malaria research is prioritised, funded and conducted. Drawing on a diverse group of stakeholders, this Perspective ar

Research priorities for advanced HIV disease in Latin America and the Caribbean region: a modified Delphi study.

INTRODUCTION: Advanced HIV disease (AHD) remains a leading cause of mortality in Latin America and the Caribbean (LAC), driven by late diagnosis, treatment gaps and structural barriers, particularly among key populations and children. Persistent disparities in healthcare access, stigma, and limited health system capacity highlight the need for targeted research to improve AHD outcomes in the region. METHODS: The modified Delphi process to prioritize AHD research questions in LAC was conducted between August and November 2024. Systematic reviews, expert consultations and two Delphi rounds invo

Administrative perspectives on the implementation and sustainability of state-supported health insurance schemes in Nigeria: a descriptive qualitative study.

BACKGROUND: Since the state-supported health insurance schemes (SSHIS) began in Nigeria, the perspectives of implementers and other administrative actors have been under-documented in the program evaluations. Bridging this information gap is crucial to addressing the challenges impeding the scheme's impact. This study, therefore, investigated the administrative stakeholders' perspective on the implementation and sustainability of the SSHIS in Nigeria. METHODS: This study adopted a descriptive qualitative case study design and was conducted in six Nigerian states with functional SSHIS, each re

Towards a Comprehensive Definition of Pandemics and Strategies for Prevention: A Historical Review and Future Perspectives.

The lack of a universally accepted definition of a pandemic hinders a comprehensive understanding of and effective response to these global health crises. Current definitions often lack quantitative criteria, rendering them vague and limiting their utility. Here, we propose a refined definition that considers the likelihood of susceptible individuals contracting an infectious disease that culminates in widespread global transmission, increased morbidity and mortality, and profound societal, economic, and political consequences. Applying this definition retrospectively, we identify 22 pandemics

Compulsory and voluntary admissions in comparison: A 9-year long observational study.

BACKGROUND: Few studies, so far, have been specifically designed to highlight the features related to Compulsory Admissions (CA) and Voluntary Admissions (VA) in Italian psychiatric emergency wards. AIMS: The main purpose of this observational study was to compare the sociodemographic and clinical characteristics of VA and CA and to explore possible predictors of re-admissions. METHODS: During a 6-month Index Period (February, the 1st-July, the 31st 2008) all psychiatric admissions were documented and then followed-up through all available informatic systems for the next 9 years. RESULTS: O

A comparative study of the proximity to nomadic travel routes and environmental factors on the occurrence of cutaneous leishmaniasis in Kohgiluyeh and Boyer-Ahmad province, southwestern Iran.

Cutaneous leishmaniasis (CL) is one of the most important health challenges in hyperendemic countries like Iran. Geospatial information systems-based studies have shown that factors, including land cover, altitude, slope temperature, rainfall and animal livestock, affect CL distribution in Kohgyloyeh and Boyerahmad province, southwestern Iran. However, the question of the influence of nomadic tribes, who travel with their goats and sheep, on CL is unanswered. We, therefore, investigated their role in CL epidemiology from 2008 to 2017 and compare them with geoclimatic factors. CL patient demogr
