Polymyositis (PM), a rare autoimmune inflammatory myopathy, typically presents with symmetric proximal muscle weakness and elevated muscle enzymes. A muscle biopsy is confirmatory for the diagnosis. We report a case of a woman in her 30s with scrub typhus and anti-nuclear matrix protein 2 (NXP2) positive PM. Idiopathic inflammatory myopathies (IIM) are a group of rare and heterogeneous autoimmune diseases, and the most common subtypes are dermatomyositis (DM) and polymyositis (PM). Despite extensive efforts, the underlying mechanism of IIM remains unclear. The association between interstitial lung disease and polymyositis/dermatomyositis is well known. It severely affects patients' quality of life, worsens prognosis and represents a major risk factor for premature death. Current treatment is unclear and therapeutic options are based on case series.
