The diameter of collagen fibrils was studied in a patient with porphyria cutanea tarda in a specimen obtained from the sclerodermatous skin of the back. A bimodal distribution of the diameter of the fibrils was observed with maxima at 33.5 nm and 68.7 nm. The mean diameter was 67 nm with a standard deviation of 10.6 nm. These results are similar to those obtained previously in localized scleroderma (morphoea).

Uroporphyrinogen decarboxylase was measured in the presence of ferrous iron, using mitochondria-free rat liver extracts as enzyme source. The activity of the enzyme was found to be increased at concentrations of ferrous iron from 0,01 mM, with maximal activity exhibited from 0,1 mM. Enzyme kinetics indicate that uroporphyrinogen decarboxylase reversibly binds ferrous iron, with a binding constant of approximately 5 x 10(4) mol-1. It is proposed that the effect of phlebotomy on patients with porphyria cutanea tarda is to mobilze storage iron in the liver to the active ferrous form, which activa

Porphyria cutanea tarda and erythropoietic protoporphyria have common morphological features. Six patients with porphyria cutanea tarda and three patients with erythropoietic protoporphyria were studied with the electron microscope. Ultrastructural finding of connective tissue, basal lamina, and blood vessels are discussed.

Porphyria cutanea tarda (PCT), the most common form of porphyria, may be one of the rare cutaneous manifestations of hepatic tumors, benign, malignant, or metastatic. The liver damage associated with PCT may predispose to the development of hepatocellular carcinoma. Our experience and a review of the literature suggest that in patients with PCT in whom the usual precipitating factors are absent, or in patients with PCT of long duration and an unexplained exacerbation, liver scan is indicated to rule out the presence of a hepatic tumor.

Extensive investigations form 1962 to 1973 on 23 patients with porphyria cutanea tarda are reported. Two collectives of patients with different treatments are compared. One collective was treated conventionally with "liver therapy, abstinence from alcohol, and rest in bed", the other collective with "phlebotomy, abstinence from alcohol, and rest in bed". Both collectives showed a nearly equal decrease of uroporphyrin excretion. The unfavourable effect of alcohol in this disease was confirmed. In the post-observation period, minimal excretion of uroporphyrin was found to be more frequent and to

A screening test for urinary porphyrin excretion was performed amound 1433 diabetes patients (552 males and 881 females). In three patients (0.21)% the diagnosis of porphyria cutanea tarda was ascertained. In spite of the increasing usage of sulfonylurea antidiabetic drugs, previously reported as potential porphyria-inducing agents, the authors could not observe an increased number of porphyria cases among the diabetics compared to the proportions in similar groups examined 10-12 years ago. The sulfonylurea Oradian was the likely etiopathogenic factor in only one out of three patients. The abo

A group of seven patients with porphyria cutanea tarda (PCT) precipitated by excessive alcohol consumption (A) was compared with a group of nine patients with PCT precipitated by estrogen therapy (B). Comparison was based on clinical signs, biochemical and morphologic evidence of liver disease, results of serum iron studies and response to therapy. Group A patients were men of mean age 57 years; group B patients were women of mean age 39 years who had been taking estrogen orally, either for contraception (in combination with progesterone) or as replacement therapy. Clinical signs were essentia

Porphyria cutanea tarda (PCT) is characterized biochemically by excessive hepatic synthesis and urinary excretion of uroporphyrin I. Clinical evidence has implicated iron in the pathogenesis of PCT. The synthesis of the normally occurring isomer of uroporphyrin, namely uroporphyrin III, from porphobilinogen (PBG) requires two enzymes; uroporphyrinogen I synthetase and uroporphyrinogen III cosynthetase (COSYN). In the absence of COSYN only uroporphyrinogen I is formed. These experiments were designed to study the effect of iron on porphyrin biosynthesis in porcine and human crude liver extracts
