Risk of congenital toxoplasmosis in women with low or indeterminate anti-Toxoplasma IgG avidity index in the first trimester of pregnancy: an observational retrospective study.

OBJECTIVES: Congenital toxoplasmosis (CT) affects one to ten fetuses per 10 000 live newborns in western countries. Without knowing pre-conception serostatus, it is hard to date the infection when anti-Toxoplasma IgG and IgM antibodies are positive at first screening. Although a high IgG avidity index (AI) in the first trimester excludes CT, the same cannot be said of intermediate and low AI. The aim of this study was to estimate the risk of CT when intermediate or low AI is detected in the first trimester of pregnancy. METHODS: Our observational retrospective study enrolled women with positi

DNA Methylation and Expression Patterns of Selected Genes in First-Trimester Placental Tissue from Pregnancies with Small-for-Gestational-Age Infants at Birth.

Variations in DNA methylation levels in the placenta are thought to influence gene expression and are associated with complications of pregnancy, like fetal growth restriction (FGR). The most important cause for FGR is placental dysfunction. Here, we examined whether changes in DNA methylation, followed by gene expression changes, are mechanistically involved in the etiology of FGR. In this retrospective case-control study, we examined the association between small-for-gestational-age (SGA) children and both DNA methylation and gene expression levels of the genes WNT2, IGF2/H19, SERPINA3, HERV

First-trimester diagnosis and management of Cesarean scar pregnancies after in vitro fertilization-embryo transfer: a retrospective clinical analysis of 12 cases.

BACKGROUND: Although Caesarean scar pregnancy (CSP) is rare, it can cause life-threatening complications. The increasing rate of Cesarean delivery plus rapid development of in vitro fertilization-embryo transfer (IVF-ET) may increase the occurrence of CSP as well as the ratio of heterotopic CSP (HCSP)/CSP. Therefore, early diagnosis and management of CSP are necessary to avoid serious complications. And the purpose of this article is to evaluate the importance and feasibility of the first-trimester diagnosis and management of CSP after IVF-ET. METHODS: All the 12 cases were secondary infertil

Changes in natural killer cell activity in normal pregnant and postpartum women: increases in the first trimester and postpartum period and decrease in late pregnancy.

Changes in the activity and number of natural killer (NK) cells in peripheral blood in normal pregnant and postpartum women were examined. NK activity was measured in a 4-h 51Cr-release assay and evaluated by conventional relative lytic units and absolute lytic units which represent the total NK activity within a fixed volume of circulating blood. The number of NK cells was analyzed with FITC-conjugated monoclonal antibodies and by use of an automated flow cytometer. Unexpectedly, the relative NK activity increased in the first trimester and also for 1 month postpartum compared to the activity

Assessment of Gaps in Knowledge, Attitudes, and Practices Regarding Antenatal Care Among Primigravida Women in the First Trimester of Pregnancy and Their Information-Seeking Behavior.

Background Good quality antenatal care (ANC) lays the foundation for a healthy pregnancy and favorable maternal and neonatal outcomes. Low utilization of ANC services is a global public health problem leading to preventable maternal and neonatal mortality and morbidity. This study aimed to assess the knowledge, attitudes, and practices (KAP) of primigravida women regarding ANC in the first trimester of pregnancy and to examine their associations with demographic variables at a selected community health center (CHC) in the district of Gautam Buddha Nagar, Uttar Pradesh. It also identified prefe

Association of first trimester peripheral blood count-derived immune markers with the risk of incident hypertensive disorders of pregnancy: a retrospective cohort study.

Emerging evidence links maternal immune dysregulation to hypertensive disorders of pregnancy (HDP), yet gestational immune alterations preceding symptom onset remain unclear. This study aimed to evaluate the associations between first-trimester immune biomarkers and incident HDP risk across clinical subtypes. This retrospective cohort study enrolled pregnant women aged ≥18 years undergoing first-trimester antenatal screening at a tertiary hospital from March to November 2023. First-trimester peripheral immune markers-neutrophils, monocytes, lymphocytes, and platelets-were measured, with derive

Association between vaginal washing and group B Streptococcus colonization from periconception through the first trimester of pregnancy in a cohort of Kenyan women.

INTRODUCTION: Vaginal washing has been associated with adverse reproductive health outcomes including pelvic inflammatory disease, reduced fecundability, and HIV acquisition. This analysis tested the hypothesis that vaginal washing is associated with increased risk of group B streptococcus (GBS) colonization. METHOD: Women planning pregnancies contributed monthly visits during which vaginal fluid specimens were collected and urine pregnancy testing was performed. In women who became pregnant, additional samples were collected at 9-12 weeks gestation. Broad-range 16S rRNA gene PCR with next ge

Feasibility of an Obesity Prevention Program for Latino Families from First Trimester of Pregnancy to Child Age 18 Months and Predictors of Program Attendance.

Background: The high prevalence of obesity in Latino families with low income necessitates prevention beginning in pregnancy and continuing through infancy. Due to systemic inequities, adverse social determinants of health (SDoH) and mental health symptoms may limit program efficacy by presenting barriers to attendance. We sought to assess: (1) the feasibility of the Starting Early Program (StEP) Prenatal, a 17-session intervention beginning early in pregnancy and continuing to 18 months postpartum; and (2) the effects of adverse SDoH (material hardship, low social support) and mental health s

Neutrophil-Lymphocyte Ratio as a Screening Test for Preeclampsia in the First and Early-Third Trimesters of Pregnancy: A Cohort Study : Neutrophil-Lymphocyte Ratio and Preeclampsia.

BACKGROUND: Preeclampsia (PEC), affecting 5-10% of all pregnancies, is a multisystem disease diagnosed by hypertension (HTN) and protein in the urine or multi-organ problems without signs of proteinuria occurring after 20 weeks of gestation. PEC is the most common cause of maternal death and can lead to premature delivery. The current study aimed to investigate the accuracy of neutrophil-lymphocyte ratio (NLR) for predicting PEC in the first and early-third trimesters of pregnancy in a normal population in the south of Iran. MATERIALS AND METHODS: A large-scale cohort study was performed from

Methotrexate therapy followed by laparotomy to manage a viable first-trimester cesarean scar ectopic pregnancy in a low-resource setting: A case report.

Cesarean scar ectopic pregnancy (CSEP) is rare, occurring in 1:1800 to 1:2625 pregnancies. It is classified into two types: endogenous, which grows inside the uterine cavity; and exogenous, which grows outward, toward the bladder. Both types are associated with increased maternal morbidity and mortality. The case report describes a 25-year-old woman with a viable first-trimester CSEP treated with both methotrexate and operative resection. Management was in a low-resource setting.

Particulate Air Pollution, Blood Mitochondrial DNA Copy Number, and Telomere Length in Mothers in the First Trimester of Pregnancy: Effects on Fetal Growth.

Growing evidences have shown that particulate matter (PM) exposures during pregnancy are associated with impaired fetal development and adverse birth outcomes, possibly as a result of an exaggerated systemic oxidative stress and inflammation. Telomere length (TL) is strongly linked to biological age and is impacted by oxidative stress. We hypothesized that PM exposure during different time windows in the first trimester of pregnancy influences both mitochondrial DNA copy number (mtDNAcn), an established biomarker for oxidative stress, and TL. Maternal blood TL and mtDNAcn were analysed in 199
