A transverse study of the monocular, steady-state accommodation response/stimulus curves for 49 normal subjects with ages between 17 and 49 years shows that the slope of the curve reduces only slowly with age up to about 40 years and then declines rapidly as complete presbyopia is approached. Such behaviour appears to result from the accommodation system changing its characteristics with age to make optimal use of the available objective amplitude of accommodation and the depth-of-focus of the eye.

Undetected vision loss commonly occurs in older adults adding undue stress to those on dialysis. Poor vision is associated with increased risk of falls and decreased quality of life. Common visual impairments--presbyopia, cataracts, age-related macular degeneration, glaucoma, and diabetic retinopathy--often are detectable by visual acuity testing. Using various methods of visual acuity testing, nephrology nurses can perform vision testing quickly and inexpensively. Other nursing interventions also can improve eyesight.

Spectacles are the most important optical aid and one of the most significant of inventions. History shows that ground rock crystals have been used for thousands of years. In Europe, eyeglasses were used from 13th century AD. Because of the loss of the eye's accommodation due to ageing--presbyopia--most people need eyeglasses for reading after the age of 50. Despite the developments of modern technology, it is still impossible to replace all use of eyeglasses.

The common form of hardness of hearing seen in old age is not, in the first instance, due to physiological age-related changes as is, for example, presbyopia, but rather to a complex sensorineural pattern of injury. The underlying causes are not yet fully understood. Typical medical risk factors such as hypertension, diabetes mellitus, etc. apparently play a smaller role than increasing environmental noise and modern life styles. The various forms of presbyacusis and their presentation in the pure tone audiogram are discussed.

The most common "physiologic" change that occurs in the eyes after age 40 is loss of elasticity of the lens, resulting in presbyopia. Xanthelasmas commonly occur at this age and should be removed only for cosmetic reasons. Entropion and ectropion can be quite irritating; both results from laxity of the eyelid tissues. Relaxation of eyelid tissues also contributes to blepharochalasis. Glaucoma and formation of cataracts are not normal changes but do appear with increasing frequency in persons over 40.

The tentative observation that the decrease of lenticular glutathione in man and in cattle may be under genetic control is extended to other biological functions which show a systematic reduction with age. Ocular and visual parameters are shown to decline consistently with the view that the human eye has evolved in keeping with other biological functions sustaining a life-span of approximately 120 years. Analysis of the data suggests that presbyopia represents an outlier in the distribution of ocular attributes, and should not be used as a biomarker for ageing.

16 healthy and ophthalmologically normal truck drivers are tested with a battery of tests according to the routine described in the following paper. This includes isopter perimetry, colour discrimination, recovery of sensitivity during dark adaptation and refraction at both far and near points, in order to record night myopia as well as night presbyopia. The ranges of individual variability are reported. As far as refraction is concerned, it seems of interest to stress the individual differences in a sort of "transient anisometropia" under incoming twilight.

The authors describe a syndrome diagnosed in workers who perform tasks requiring visual strain. The syndrome comprises worse visual control, discomfort in orbital area and characteristic changes of refraction and accommodation. The young people having emmetropia and myopia demonstrate temporary and later consistent increase of refraction, and the elderly ones--decreased refraction and early development of presbyopia. In both cases marked fall of accommodation was shown. The article suggests also a possible scheme describing pathogenesis of the syndrome.

PURPOSE: To report a case of keratitis and corneal melt after conductive keratoplasty (CK) enhancement.
METHOD: Case report.
RESULTS: A 52-year-old woman with emmetropic presbyopia had undergone previous CK for monovision and underwent CK enhancement 4 years later. Postoperatively, she was managed with ketorolac tromethamine 0.4% and developed keratitis and corneal melt.
CONCLUSIONS: This is the first reported case of keratitis and corneal melt associated with nonsteroidal antiinflammatory topical medications in a post-CK patient.

The first part of this paper is devoted to a brief survey of novel aspects of ocular senescence which are shown to fit into the general, apparently genetic, non-pathological framework of senescence. Monovision (MV) contact lens correction of presbyopia induces substantial reductions in stereoacuity and small reductions in binocular visual acuity (VA). This study examined those effects in a group of successful and a group of unsuccessful MV patients. Compared to performance with a full binocular correction, the unsuccessful group demonstrated significant losses in both functions.
