BACKGROUND: Nasal continuous positive airway pressure (NCPAP) is used to support preterm infants recently extubated, those experiencing significant apnoea of prematurity and those with respiratory distress soon after birth as an alternative to intubation and ventilation. This review will focus exclusively on identifying the most effective pressure source and interface for NCPAP delivery in preterm infants.
OBJECTIVES: In preterm infants extubated to NCPAP following intermittent positive pressure ventilation (IPPV) for respiratory distress syndrome (RDS) or in those treated with NCPAP soon aft…

BACKGROUND: Vertical displacement of the arm relative to the heart causes inverse changes in blood pressure of approximately 0.8 mmHg for every centimetre change in arm position. Therefore a potential confounding issue in assessing diurnal variation in blood pressure during ambulatory blood pressure monitoring (ABPM) is arm position during sleep. An increase in the number of patients with 'excessive' nocturnal dipping (> 20% decrease in night/day blood pressure) was observed following the creation of an instructional videotape in which patients were advised to muffle the noise of the monitor w…

Assessments of dynamic cerebral autoregulation usually measure the cerebral blood flow velocity (CBFV) response to changes in arterial blood pressure (ABP). We studied the effect of substituting ABP by cerebral perfusion pressure (CPP), expressed as the difference between ABP and intracranial pressure (ICP), in estimates of dynamic autoregulation obtained by transfer function analysis. CBFV, ABP and ICP were recorded during periods of physiological stability in 30 patients with severe head injury. Transfer function analysis was performed using the following combinations of input-output variabl…

OBJECTIVE: Studies on the effects of chronic exposure to industrial noise on clinic blood pressure (BP) at rest have yielded inconsistent results. The aim of this study was to evaluate the effect of occupational noise exposure on ambulatory blood pressure (ABP) in normotensive subjects.
METHODS: We studied 476 normotensive workers, aged 20-50 years (systolic blood pressure (SBP) < 140, diastolic blood pressure (DBP) < 90), at a metallurgical factory; 238 were exposed to high levels of noise (> 85 dB), while 238 were not exposed (< 80 dB). Clinical evaluation included measurements of casual BP…

The present research was aimed at determining the number of self-measured blood pressure (BP) readings needed to attain reliable estimates of true BP in hypertensive patients. Correlation coefficients and standard deviation of differences between pairs of measurements as well as generalizability theory were applied to data from a controlled study on stress management training for essential hypertension (García-Vera et al., 1997). Forty-three hypertensive patients self-recorded 48 readings of BP (at home and at work) at both the pretreatment and the posttreatment (separated by a period of 2 mon…

The purpose of this study was to investigate the compatibility between oscillometric and auscultatory methods, and to determine whether one is preferable to the other for ambulatory 24 h blood pressure monitoring. We used an A & D TM 2421 blood pressure monitoring device. Our investigation included 281 patients with suspected hypertension or undergoing treatment for hypertension. We were able to compare 18,790 paired measurements. The difference (auscultatoric--oscillometric) in paired single readings between the two methods was 1.4 +/- 19 mmHg (mean +/- SD) for systolic and -2.4 +/- 18 mmHg f…

OBJECTIVE: To perform ambulatory blood pressure monitoring (ABPM) in high school students and to compare the 24-hour values with casual blood pressure (BP).
METHODS: Ambulatory BP monitoring was carried out in 190 high school students, 15 or 16 years of age, with elevated casual BP during the first examination at school for hypertension; 66 had elevated casual BP at both the first and the second examinations (group H1), and 124 students were normotensive at the second examination (group H2). Sixty-two students who were consistently normotensive served as control subjects (group N). Average BP…

Intracranial pressure (ICP) was monitored continuously for one night in 36 patients with suspected symptomatic normal pressure hydrocephalus (NPH) to identify patients who might benefit from subsequent shunting. In 33 of these patients middle cerebral artery (MCA) blood flow velocity by means of transcranial Doppler sonography (TCD) and ICP were recorded simultaneously. ICP B-waves always paralleled changes in the TCD signal (TCD B-wave equivalents). The relative frequency of ICP B-waves was predictable by TCD, albeit slightly underestimated due to a generally lower relative amplitude of the T…

OBJECTIVE: To determine whether vasodilators produce a fall in mean blood pressure (MBP) that is directly proportional to baseline MBP.
DESIGN: The effects on blood pressure of three calcium channel blockers, nitrendipine, nisoldipine and verapamil, were compared with sodium nitroprusside, a drug acting through a different mechanism. The drugs were infused in conscious, unrestrained rats and their effect on MBP was measured.
SETTING: Primary experimental.
ANIMALS: Animals were divided into three groups according to their baseline MBP: high (greater than 119 mmHg), moderate (91 to 119 mmHg) …

Patients (pts) with essential hypertension normally exhibit a typical diurnal variation with a nocturnal blood-pressure (BP) decreased. A lack of this periodicity is often reported in pts with secondary hypertension. 24-h BP measurement was therefore performed in 308 pts with essential hypertension, and in 172 pts with secondary hypertension, in order to evaluate the diagnostic value of nocturnal BP decrease. Diagnoses of the secondary hypertensives were: renoparenchymatous hypertension (n = 29), diabetic nephropathy (n = 24), morbus Conn (n = 6), renal artery stenosis (n = 32), pheochromocyto…

The functional urethral length (FUL), the maximum urethral closure pressure (MUCP), and the urethro vesical quotient (u/v) have been measured in 32 women suffering from urinary stress incontinence. The pressure was registrated in four different sectors of the urethra (left, posterior, right, anterior). We found statistical significant differences (t-Test, Wilcoxon-Test). The average FUL at the anterior urethral wall was 27.5 mm, at the posterior wall 30.2 mm. Concerning the UVDR and the u/v-quotient we found higher values at the ventral wall than at the posterior and lateral sides (9.79 kPa an…

The effects of perindopril on the 24-hour arterial pressure levels were evaluated by ambulatory recording in 21 patients (mean age 48 +/- 2 years) with mild to moderate hypertension. At the end of a 3 months treatment with perindopril (4 to 8 mg per day in one dose), comparison by variance analysis of the mean values of arterial pressure over 24 hours before and after treatment showed a significant decrease of SAP (from 144 +/- 3 to 133 +/- 3 mmHg, p less than 0.01) and DAP (from 95 +/- 2 to 87 +/- 2 mmHg, p less than 0.01). The fall in arterial pressures was more pronounced during day-time (7…

Cardiovascular changes resulting from continuous distending alveolar pressure during endotracheal intubation (CPAP) and continuous negative pressure (CNP) were studied in two groups of healthy newborn lambs at 6 and 11 mm Hg and -6 and -11 mm Hg, respectively. Heart rate, left ventricular pressure, and arterial blood gases were unchanged in each group. Cardiac output decreased 16 and 15% at 6 and 11 mm Hg CPAP (P less than 0.05), respectively, whereas a fall of 9% occurred at both -6 and -11 mm Hg CNP (P greater than 0.05). Central venous pressure (mm Hg) rose from a control value of 7.0 +/- 1…

Intracranial pressure (ICP) studies were carried out in 14 infants with severe birth asphyxia and brain damage. A markedly low cerebral perfusion pressure (CPP) was noted in infants who died and in 1 infant who survived with cerebral palsy. The long-term ICP tracing revealed negative waves and plateau waves in 2 infants. Cushing response was noted in 2 infants who had elevated ICP. The value and significance of evaluated CPP and of abnormal waveforms are discussed.

Malabsorption of cerebrospinal fluid was made experimentally by the administration of neutral carbon black suspension into the lateral ventricles of 22 adult cats. The compartmental analysis of intracranial pressure at the intraventricular fluid and tissue pressure in the cerebral white matter was carried out in the early stage of the communicating hydrocephalus. Following results were obtained: 1) Cerebrospinal fluid pressure from the lateral ventricle was always significantly higher than that from the brain tissue during the process of ventricular expansion. 2) Water content in the cerebral …

Intracranial pressure was raised by expansion of a supratentorial subdural ballon in anaesthetized baboons. Pressures were measured at several sites, both supratentorial and infratentorial, and cerebral blood flow was measured in each cerebral hemisphere separately. Pressures recorded from the right and left lateral ventricles corresponded closely throughout. Highly significant correlations were also obtained between the pressures in the right and left subdural spaces and the mean intraventricular pressure. There was, thus, no evidence of intracompartmental pressure gradients within the suprat…

Mean arterial pressure (MAP) is a critical haemodynamic parameter, reflecting the average pressure during the cardiac cycle and serving as a determinant of tissue perfusion. Despite its clinical importance, MAP is often estimated using empirical formulas in routine practice, which may lead to inaccuracies. While previous studies have highlighted MAP's dynamic interactions with other physiological signals, the fast, within-beat relationships between MAP and blood pressure measurements - systolic arterial pressure (SAP) and diastolic arterial pressure (DAP) - remain underexplored. To fill this g…

BACKGROUND: Due to technical limitations of commonly used tonometry devices, the effects of physical exercise on intraocular pressure (IOP) have never been studied during exercise. This study continuously monitored IOP variations in patients with glaucoma during physical exercise using an implanted IOP microsensor.
METHODS: In total, 10 participants (six male and four female) with open-angle glaucoma (age range: 67-79), previously implanted with an IOP sensor, were included. We monitored IOP telemetrically during a 10 min session on a bicycle ergometer with a stepwise power increase from 0 to…

Air insoles have provided insights for reducing the risk of diabetic foot ulcers (DFU). The pressure time integral (PTI) is an effective assessment that considers the time effect in various physical activities. We investigated the interactions between three different insole inner pressures (80, 160, and 240 mmHg) and two walking durations (10 and 20 min). The big toe (T1), first metatarsal head (M1), and second metatarsal head (M2) were investigated in 13 healthy participants. One-way analysis of variance (ANOVA) showed that the effects of each insole inner pressure significantly differed (P <…

The intracranial pressure is implicated in many homeostatic processes in the brain and is a fundamental parameter in several diseases such as e.g. idiopathic normal pressure hydrocephalus. The presence of a small but persistent pulsatile intracranial pulsatile transmantle pressure gradient (on the order of a few mmHg/m at peak) has recently been demonstrated in hydrocephalus subjects. A key question is whether pulsatile intracranial pressure and displacements can be induced by a small pressure gradient originating from the brain surface alone. In this study, we model the brain parenchyma as ei…
