BACKGROUND: Oral health problems are common among care-dependent older adults living in nursing homes. Developing strategies to prevent the deterioration of oral health is therefore crucial to avoid pain and tooth loss. A standardized work widely used in nursing homes in Sweden is the quality register Senior Alert (SA), which assesses age-related risks concerning e.g. pressure sores, falls, malnutrition and oral health. The oral health assessment is performed with the Revised Oral Assessment Guide-Jönköping (ROAG-J), which also includes planning and implementation of preventive oral care inter

Although body mass index (BMI) is widely used to diagnose obesity, it is an imprecise measure of health. The Edmonton Obesity Staging System (EOSS) assesses the health impacts of elevated BMI based on medical, mental and functional complications and has been shown to predict mortality more accurately than BMI alone. This study aimed to develop a simple clinical support tool for structured assessment of the EOSS domains in routine clinical practice. An EOSS-based clinical support tool was developed through an iterative Delphi process with obesity-care experts, including pilot testing with patie

This study developed and evaluated a health management program based on the participant-centered concept of action research to reduce cardiovascular disease (CVD) risk factors among blue collar workers. Data from structured questionnaires completed by 32 workers in a small-to-medium sized workplace from September 2015 to October 2016 as well as participants' anthropometrical (weight and waist) and biological (blood pressure, glucose, total cholesterol, triglyceride, high-density lipoprotein (HDL), and low-density lipoprotein (LDL) cholesterol) data were analyzed using paired t-test and Fisher'

BACKGROUND: The fact that tuberculosis can be treated with the DOTS strategy (Directly Observed Treatment, Short-course) is not enough to control the disease. Patients have to find their way to tuberculosis treatment first. To better understand the route to tuberculosis treatment in rural Nepal we interviewed twenty-six patients under treatment.
METHODS: In semi-structured interviews patients shared their disease history and health seeking behaviour. The analysis focused on the encounters with the health care system before enrolment in the tuberculosis treatment program.
RESULTS: Patient rou

BACKGROUND: Primary Health Care (PHC) is a strategy endorsed for attaining equitable access to basic health care including treatment and prevention of endemic diseases. Thirty four years later, its implementation remains sub-optimal in most Sub-Saharan African countries that access to health interventions is still a major challenge for a large proportion of the rural population. Community-directed treatment with ivermectin (CDTi) and community-directed interventions (CDI) are participatory approaches to strengthen health care at community level. Both approaches are based on values and principl

BACKGROUND: Regionalized care delivery purportedly optimizes care to vulnerable very low birth weight (VLBW; <1500 g) infants. However, a comprehensive assessment of quality of care delivery across different levels of NICUs has not been done.
METHODS: We conducted a cross-sectional analysis of 21,051 VLBW infants in 134 California NICUs. NICUs designated their level of care according to 2012 American Academy of Pediatrics guidelines. We assessed quality of care delivery via the Baby-MONITOR, a composite indicator, which combines 9 risk-adjusted measures of quality. Baby-MONITOR scores are mea

AIM: The aim of this study was to evaluate the effects of person-centred care (PCC) after acute coronary syndrome (ACS) in relation to educational level of participants.
METHOD: 199 Patients <75years with ACS were randomised to PCC plus usual care or usual care alone and followed for 6months from hospital to outpatient care and primary care. For the PCC group, patients and health care professionals co-created a PCC health plan reflecting both perspectives, which induced a continued collaboration in person-centred teams at each health care level. A composite score of changes that included gene

OBJECTIVE: To determine the impact of delirium on withdrawal of life-sustaining treatment (WLST) after intracerebral hemorrhage (ICH) in the context of established predictors of poor outcome, using data from an institutional ICH registry.
METHODS: We performed a single-center cohort study on consecutive patients with ICH admitted over 12 months. ICH features were prospectively adjudicated, and WLST and corresponding hospital day were recorded retrospectively. Patients were categorized using DSM-5 criteria as never delirious, ever delirious (either on admission or later during hospitalization)

INTRODUCTION: In a discussion of the risks and benefits of pediatric cochlear implantation, device failure and the need for revision surgery is often overlooked. The failure rate has not been investigated extensively for this population of patients. Hearing results are under-reported following revision surgery as well. We will review our experience with cochlear implant failure, revision, and hearing results when available to better guide the preoperative counseling of families considering cochlear implantation.
METHODS: Retrospective chart review of all children undergoing cochlear implantat

The risks of poor transition include delayed and inappropriate transfer that can result in disengagement with healthcare. Structured transition care can improve control of chronic digestive diseases and long-term health-related outcomes. These are the first nationally developed guidelines on the transition of adolescent and young persons (AYP) with chronic digestive diseases from paediatric to adult care. They were commissioned by the Clinical Services and Standards Committee of the British Society of Gastroenterology under the auspices of the Adolescent and Young Persons (A&YP) Section. Elect

Some surgical procedures can be performed more safely outside the OR. One procedure commonly performed within the neonatal intensive care unit (NICU) is patent ductus arteriosus (PDA) ligation. Staff members at one tertiary care facility assessed current management and delivery of bedside PDA ligation surgery and identified several problem areas, including communication issues regarding patient and unit readiness, design of instrument sets, and organization and transportation of surgical supplies. Implementing solutions to these problems resulted in a decrease in the amount of time the surgica

BACKGROUND: There are only few studies on the evaluation of the efficiency of oncologic rehabilitation. The objective of this study was to determine the efficiency of a complex rehabilitation programme consisting of medical care, physical therapy, sports therapy, psychological support and health care seminars. Therefore the rehabilitation requirement at admission and the rehabilitation result at discharge were assessed in a patient sample.
METHODS: To determine the rehabilitation requirement and the rehabilitation result, a self-assessment patient questionnaire was used (IRES, Indicators of R

Metropolitan Houston with a population of four million has the nation's poorest freeway system. Its two Level I trauma centers are adjacent within a centrally located freeway loop, therefore the city is ideally suited for a trauma scene helicopter transport service. During 1981 there were 577 flights to the scene of injury (blunt, 466; penetrating, 111). Flights were requested by 60 agencies (EMS, law enforcement, etc.). All flights were manned by a surgical resident and flight nurse. The flight distances ranged from 2 to 57 miles (average, 14.4). Three hundred six flights (53%) were within th

BACKGROUND: Historically, health literacy has been viewed as the patient's problem; however, it is now accepted that the responsibility for improving health literacy lies with the health care professionals and systems. An Institute of Medicine report outlines the health literacy attributes, such as printed patient education and technology, which both play a role in patient decision-making and engaging them in their health care. Research suggests that patients who are engaged in their health care have improved health outcomes. For health care organizations to accommodate the needs of all patien

Malnutrition is highly prevalent among cancer patients, particularly those with head and neck and gastrointestinal malignancies, older age, and advanced disease. It results from disease burden, treatment-related factors, prolonged hospitalization, and unrecognized nutrition-impact symptoms, leading to poor surgical outcomes, increased complications, mortality, and health care costs. Despite being a modifiable perioperative risk factor, nutritional status is often overlooked. Comprehensive preoperative nutritional screening, assessment, and intervention-guided by established tools and the Nutri

INTRODUCTION: In extremely and very preterm infants, predicting individual risks for adverse outcomes antenatally is challenging but necessary for risk-stratified perinatal management and parents' participation in decision-making about treatment. Our aim was to develop and validate prediction models for short-term (neonatal period) and medium-term (3 years of age) outcomes based on antenatal maternal and fetal factors alone.
MATERIAL AND METHODS: A population-based study was conducted on 31 157 neonates weighing ≤1500 g and born between 22 and 31 weeks of gestation registered in the Neonatal

BACKGROUND: Routine admission following primary cleft palate repair is the standard of care at most institutions. Insurance companies have demonstrated increasing resistance to hospitalization longer than a "short stay"(23 hour) observation period following palatoplasty. The purpose of this study was to identify factors related to length of stay following palate repair.
METHODS: Retrospective chart review was conducted for 100 consecutive patients undergoing primary cleft palate repair from May 2009 to February 2013. Demographic and perioperative data were collected and two-sample Student t-t
