Young women's recent experience of labour and birth care in Queensland.

BACKGROUND: young parenthood continues to be an issue of concern in terms of clinical and psychosocial outcomes for mothers and their babies, with higher rates of medical complications such as preterm labour and hypertensive disease and a higher risk of depression. The aim of this study was to investigate how young age impacts on women's experience of intrapartum care. METHODS: secondary analysis of data collected in a population based survey of women who had recently given birth in Queensland, comparing clinical and interpersonal aspects of the intrapartum maternity care experience for 237 e

Exploring the influence of the attachment organizations of novice therapists on their delivery of emotionally focused therapy for couples.

Clinicians' own internal resources for understanding relationships--that is, their attachment organizations--have been found to influence the process and outcome of treatment. The current study addressed whether the attachment organizations of novice couple and family therapists were associated with couples' experiences of their therapists, therapeutic alliance, session impact, and emotionally focused couple therapy (EFT) fidelity (i.e., especially as related to targeting and working with attachment needs and overt and underlying emotions). Novice couple and family therapists delivered EFT, an

Prevention of vector transmitted diseases with clove oil insect repellent.

Vector repellent is one element in the prevention of vector-borne diseases. Families that neglect protecting their children against vectors risk their children contracting illnesses such as West Nile virus, eastern equine encephalitis, Lyme disease, malaria, dengue hemorrhagic fever, yellow fever, babesiosis, Crimean-Congo hemorrhagic fever, Rocky Mountain spotted fever, Southern tick-associated rash illness, ehrlichiosis, tick-borne relapsing fever, tularemia, and other insect and arthropod related diseases (CDC, 2011). Identification of families at risk includes screening of the underlying b

The response of mental health professionals to clients seeking help to change or redirect same-sex sexual orientation.

BACKGROUND: we know very little about mental health practitioners' views on treatments to change sexual orientation. Our aim was to survey a representative sample of professional members of the main United Kingdom psychotherapy and psychiatric organisations about their views and practices concerning such treatments. METHODS: We sent postal questions to mental health professionals who were members of British Psychological Society, the British Association for Counselling and Psychotherapy, the United Kingdom Council for Psychotherapy and the Royal College of Psychiatrists. Participants were ask

The negotiation of intelligibility in an aphasic dyad.

Employing conversation analysis as a research technique, this study investigates the ways that unintelligibility is accounted for and overcome within a therapeutic encounter between an individual with aphasia and dysarthria and his clinician. The results emphasize the collaborative nature of intelligibility negotiation and demonstrate how both the individual with the impairment and his clinician employ various interactional strategies and knowledge resources to turn unintelligibility into intelligibility so that the therapeutic encounter may continue.

Compassion fatigue and burnout: the role of Balint groups.

General practitioners are often the 'first port of call' for patients with a range of mental health problems, many of whom have a history of trauma or loss. Exposure to emotionally difficult situations puts them at risk of burnout and compassion fatigue. Balint groups are groups of GPs, usually facilitated by a psychiatrist, who discuss the doctor-patient relationship and provide peer support. Participation in Balint groups, along with other professional and personal activities, has the potential to prevent compassion fatigue and burnout in participants.

Lesbian, gay, bisexual, transgendered, and questioning youth: the importance of a sensitive and confidential sexual history in identifying the risk and implementing treatment for sexually transmitted infections.

STIs are one consequence of specific risk-taking behaviors, not sexual orientation. Providers who care for adolescents should be aware of the STIs for which LGBTQ youth are at risk and the necessary screening (see Table 1). Although it is vital to recognize that LGBTQ youth are at risk for STIs, it is important not to view the youth within this narrow perspective. LGBTQ youth, like all adolescents, should be appreciated for their individuality, idealism, and resiliency. Most LGBTQ youth emerge from adolescence as productive and healthy adults. Treating adolescents with understanding and respec

[About the use of seclusion in psychiatry: the patients' point of view].

In this study of psychiatric inpatients' perceptions of the seclusion-room experience, 67 admitted inpatients were interviewed during 6 Months within 3 days of the experience, and 24 hours after. A 35-items semistructured interview schedule was used to obtain information on six research questions. Subjects' perception of the reasons for their seclusions varied greatly from perceptions of staff members. Findings implied that for some patients seclusion may have been unnecessary, but for others it was beneficial. Subjects who reported out-of-control impulses or pathological intensity of relation

The effectiveness of staff support: evaluating active support training using a conditional probability approach.

Active Support, a package of procedures which includes activity planning, support planning, and training on providing effective assistance, was introduced in five community residences serving 19 adults with severe mental retardation following a multiple baseline design. Real-time observational data were collected on the level of assistance residents received from staff and their engagement in activity. Active Support was shown in a companion paper (Jones et al., 1999) to increase the levels of assistance residents received and their engagement in activity. Increased assistance was particularly

An evaluation of the efficacy of supervised cognitive behavioral self-help bulimia nervosa.

Eighty two patients with bulimia nervosa were managed by providing them with supervision in the use of a highly structured cognitive behavioral self-help manual. Their progress was assessed in an open clinical trial. The 67 patients who completed the course of self-help experienced considerable benefit; the frequency of bulimic episodes and self-induced vomiting decreasing by 80% and 79%, respectively. Compared to those who benefited, those who had a poor outcome or dropped out of treatment were more than twice as likely to have had anorexia nervosa in the past and were somewhat more likely to

The extent of support for ordinary living provided in staffed housing: the relationship between staffing levels, resident characteristics, staff: resident interactions and resident activity patterns.

Staffed housing has become an accepted alternative to institutional residential services for people with learning disabilities on the expectation that such provision will promote 'ordinary' patterns of living. Information on staffing levels and the behavioural characteristics of residents together with direct observational data on staff: resident interactions and resident engagement in activity were collected on 15 housing services in South Wales and analysed to explore the interrelationship between these key input and outcome variables. Staffing levels were found to be related to resident cha

Family-centered critical care: an option or obligation?

The patient was dying after undergoing aggressive treatment for a malignancy. Patient and family wanted "everything" to be done and the patient was transferred to the ICU for treatment of acute respiratory failure. The next day the patient's condition deteriorated further. The family decided against chest compressions or defibrillation; however, other aggressive treatment was continued. A "chemical code" was initiated and the patient was ventilated. The family was informed. As they stood in the hall outside the unit, the patient's wife asked if she could be with her husband. A nurse explained

The Armero tragedy: lessons for mental health professionals.

A U.S. mental health consultant worked closely with medical personnel soon after a volcanic eruption and mud avalanche killed about 22,000 persons and devastated the area around Armero, Colombia. The consultant conducted workshops and courses on crisis intervention for health personnel operating disaster relief units and for mental health professionals, pediatric nurses, and family workers; she also provided consultations to clinic and shelter directors and case consultation with hospitalized victims. Observations of early postdisaster responses of hospitalized victims showed recurring themes

Priming primary care providers to engage in evidence-based discussions about cannabis with patients.

Cannabis use has become increasingly common in the U.S. in recent years, with legalization for medical and recreational purposes expanding to more states. With this increase in use and access, providers should be prepared to have more conversations with patients about use. This review provides an overview of cannabis terminology, pharmacology, benefits, harms, and risk mitigation strategies to help providers engage in these discussions with their patients. Current evidence for the medical use of cannabis, cannabis-related diagnoses including cannabis use disorder (CUD) and withdrawal syndromes
