Stemless reverse total shoulder arthroplasty: a systematic review of contemporary literature.

PURPOSE: Reverse shoulder prostheses are increasingly used for treatment of rotator cuff tear arthropathy and other degenerative shoulder diseases. In recent years, aiming for bone stock preservation has led to the design of metaphyseal humeral components without a stem. The aims of this study were to evaluate the complication and reintervention rates, as well as the clinical and radiographic outcomes in patients who underwent reverse shoulder arthroplasty (RSA) with stemless implants. METHODS: A systematic review of the literature was completed until May 2020 using PubMed, EMBASE, CINAHL and

Cardiac implantable structural devices (CISDs) and cardiac implantable electronic devices (CIEDs) on chest radiography: a pictorial review.

Implantable cardiac devices are increasingly used to manage cardiovascular conditions. Consequently, radiologists are more frequently encountering the ever-expanding range of both cardiac implantable electronic devices (CIEDs) and cardiac implantable structural devices (CISDs) on chest radiography. Chest radiography remains a widely available, low-cost modality for confirming device presence and detecting complications such as gross malposition or migration. This pictorial review demonstrates the characteristic appearances of established and emerging CIEDs and CISDs on chest radiography, rangi

Dental Rehabilitation with Implants in a Pediatric Patient with Ectodermal Dysplasia.

The purpose of this report is to describe a seven year-old boy diagnosed with hypohidrotic ectodermal dysplasia who was rehabilitated with dental implants and partial removal prosthesis. The patient had only three dental elements: primary maxillary right first molar and primary maxillary right and left canines. The patient underwent treatment under general anesthesia in a hospital setting. Two dental implants were placed in the mandibular arch to support a dental prosthesis. Due to the COVID-19 pandemic, there was a prolonged period before continuing with the rehabilitative phase. Upon resumpt

Mammography: breast implants--types, complications, and adjacent breast pathology.

Approximately 1.5 million women in the United States currently have breast implants. The majority were placed for cosmetic augmentation, but 20% were placed for reconstruction after the loss or deformity of a breast. The augmented breast is a challenge to the mammographer. Many of the palpable and mammographically detected abnormalities in these patients are related to the implant itself. Since, however, there is breast tissue present with cosmetic augmentation, the full range of fibrocystic and neoplastic conditions that can affect the breast may be seen. The presence of the implant makes ima

[Multiple surgical possibilities for the correction of acetabulum failure in rheumatoid arthritis].

Pronounced osteoporosis of and surrounding the acetabulum commonly leads to early loosening of endoprosthetic hip replacements following implantation. Large osseous defects in the subimplant bed are the result and the reason why multiple revisions of loosened acetabular cups have to be performed so often in rheumatic surgery. The defects extend from the cranial to the central; ventral and dorsal anchorage losses may occur in addition to this. The anchorage of a new implant must be adapted to the individual circumstances. The new implant must be selected in accordance with point of surgical acc

Multicentre observational study of the Gatekeeper for faecal incontinence.

BACKGROUND: A variety of therapeutic approaches are available for faecal incontinence. Implantation of Gatekeeper prostheses is a new promising option. The primary endpoint of this prospective observational multicentre study was to assess the clinical efficacy of Gatekeeper implantation in patients with faecal incontinence. Secondary endpoints included the assessment of patients' quality of life, and the feasibility and safety of implantation. METHODS: Patients with faecal incontinence, with either intact sphincters or internal anal sphincter lesions extending for less than 60° of the anal ci

Osseointegrated implants in the management of childhood ear abnormalities: with particular emphasis on complications.

Little has been published about the difficulties encountered during the insertion of osseointegrated implants for the attachment of bone anchored hearing aids (BAHA) and auricular prostheses in children. This study examines this issue in the first 51 children implanted at our centre. During surgery, the most common problem encountered was the presence of thin bone resulting in incomplete insertion of fixtures. Five fixtures had failed to integrate and six fixtures were lost in the long-term, however, only five children required revision surgery. The reason why few patients require revision was

Early fixation of the humeral component in stemless total shoulder arthroplasty : a radiostereometric and clinical study with 24-month follow-up.

AIMS: Stemless humeral implants have been developed to overcome stem-related complications in total shoulder arthroplasty (TSA). However, stemless implant designs may hypothetically result in less stable initial fixation, potentially affecting long-term survival. The aim of this study is to investigate early fixation and migration patterns of the stemless humeral component of the Simpliciti Shoulder System and to evaluate clinical outcomes. METHODS: In this prospective cohort study, radiostereometric analysis (RSA) radiographs were obtained in 24 patients at one day, six weeks, six months, on

Radial head, radiocapitellar and total elbow arthroplasties: a review of recent literature.

The outcome of prosthetic elbow surgery is continually evolving. We thoroughly reviewed the literature on this issue to analyse the indications, outcomes and complications of the numerous types of implants currently in use. Radial head replacement is recommended in comminuted fractures of the radial head and in post-traumatic conditions. Medium- and long-term results prove to be satisfactory in the majority of cases, with no evidence to indicate that some prostheses (monopolar vs. bipolar; cemented vs. press-fit) are more effective than others; nonetheless, the bipolar-cemented implant was fou

Limb salvage and complication management after (sub-) total humerus resection for primary malignant bone tumors in early childhood.

Limb-salvage after (sub-) total humerus resection in skeletally immature patients, aged less than 10 years, remains the domain of individual case decisions due to a small number of affected patients. Seven children aged ≤ 10 years at the time of (sub-) total bone sarcoma resection of the humerus between 2018 and 2023 were included in this retrospective study. Limb salvage with an intact motor function of the lower arm was achieved in 100% of cases at a mean follow-up of 89.8 months. Reconstruction survival was 28.6%. 61.8% (n = 8/13) of complications necessitated revision operations (n = 7). D

Mid-term clinical outcome comparison of long-stemmed monopolar osseointegrated and short-stemmed bipolar radial head prostheses.

INTRODUCTION: Comparative data to guide implant choice for radial head replacements are lacking. Here, we compared the clinical results achieved using two different types of radial head prostheses. METHODS: Data from patients with comminuted radial head fractures (n = 66), who underwent radial head arthroplasty with either short-stemmed bipolar (n = 31, Group 1: rHead Small Bone Innovations/USA) or monopolar long-stemmed osseointegrated rigidly fixed (n = 35, Group 2: MoPyC Tornier/France) prostheses, were retrospectively reviewed. Patients were followed-up for an average of 42 months (16-64

A Digitally-Controlled Integrated Circuit Solution for Tinnitus Treatment with Charge Balancing.

An integrated circuit specified for tinnitus treatment is described. This chip, realized using a 0.18um BCD high-voltage CMOS process, is capable of generating current stimulus with any wave shape directly into the inner-ear tissue without the need for off-chip control circuitry. Used as part of a multi-chip module that can be implanted into the inner ear, this core chip contains an 8-bit digital-to-analog converter, an amplitude control block, a novel high-voltage drive and charge balance circuit, a high-voltage level shifter, an SRAM, a ROM, and an on-chip central control unit. The chip can

Experience with bone morphogenetic protein-2 and interpositional grafting of edentulous maxillae: a comparison of Le Fort I downfracture to full-arch (horseshoe) segmental osteotomy done in conjunction with sinus floor grafting.

This paper is a retrospective report of the treatment of six patients with severely resorbed maxillae. Patients were treated, based on the amount of maxillary retrognathia, with either a Le Fort I downfracture or a "horseshoe" interpositional sandwich osteotomy, along with sinus elevation. Recombinant human bone morphogenetic protein-2 in an absorbable collagen sponge carrier was used for grafting in all patients, either alone or in combination with other grafting materials. Implants were placed and the patients were restored with fixed prostheses. Both grafting techniques are described, and t

Radial head arthroplasty: state of the art.

Radial head arthroplasty has joined the armamentarium of options for the treatment of complex radial head fractures, elbow instability, and arthritic conditions. A variety of implants has been introduced in the past decade; these differ in metallic composition, design, and method of fixation. Good short- and intermediate-term outcomes have been reported with the use of loose-fitting prostheses. Press-fit devices restore stability and improve pain and motion but are associated with a greater likelihood of implant loosening, leading to revision surgery. Postoperative elbow stiffness, pain, ulnar

Primary hip replacement in young osteoarthritic patients current practices in one UK region.

Approximately 10% of primary hip replacements performed each year for osteoarthritis are in patients aged 55 or younger. These patients have a longer life expectancy and a higher activity level than an older cohort, which may translate to higher revision rates. We utilized a regional hip register (Trent and Welsh Arthroplasty Audit Group (TWAAG)) to review current surgical practice in this age group. From 01/01/2000 31/12/2002, we were notified of 7,678 primary THRs for osteoarthritis. Of these 911 (11.7%) were performed on patients aged 55 or less. Age, gender, grade of lead operating surgeon

Biological effects of clinically relevant wear particles from metal-on-metal hip prostheses.

The problems of osteolysis and late aseptic loosening associated with ultra-high molecular weight polyethylene (UHMWPE) particles has lead to a renewed interest in metal-on-metal prostheses. Wear particles generated by modern Co-Cr-on-Co-Cr prostheses are nanometre in size (range: 10-120 nm; mean: about 40 nm), an order of magnitude smaller than the size of UHMWPE known to be critical for activation of osteolytic cytokines by macrophages. Co-Cr wear particles will induce osteolytic cytokine production by human macrophages, but only at high volumetric concentrations. Unlike UHMWPE, Co-Cr partic

Reconstruction of large mandibular and surrounding soft-tissue defects using distraction with bone transport.

Reconstruction of large bone and soft-tissue defects of the inferior third of the face is possible using various surgical techniques. Patients who require these procedures need to be in good general health, may have sequelae linked to donor sites, and require several interventions to achieve good aesthetic and functional results. The aim of this study was to report outcomes in patients with large mandibular and soft-tissue defects treated using osteogenic distraction with bone transport. Between 2001 and 2008, 14 patients had distraction with bone transport. Most patients were men (92.1%). The

Reconstruction of the external ear using implant-supported alloplasts-Our experience.

Surgical reconstruction of the missing external ear is difficult, and the results are often far from satisfactory. An implant-retained auricular prosthesis is a suitable alternative. Microtia, malformation, deformity, and partial or complete loss of the external ear may be due to various congenital or acquired factors. A case series of three patients treated with implant-retained auricular prostheses is presented in this article. For each missing pinna, two titanium implants were placed in the temporal bone. After 6 months of osseointegration, the implants were loaded. All three cases were reh
