Cervical spine clearance in pediatric trauma patients - Consensus algorithm of the Pediatric Spinal Trauma Group of the Spine Section of the German Society for Orthopedic and Trauma Surgery (DGOU).

PURPOSE: Although cervical spine injuries in pediatric patients are rare, clinical and radiological examinations are clinical routine. Currently, no standardized decision algorithm does exist for detection of these injuries in the European region. The proposed algorithm is aiming to safely identify significant injuries through consistent diagnostics while minimizing immobilization and radiation exposure at the same time. METHODS: The Pediatric Spinal Trauma Group of the Spine Section of the German Society for Orthopedic and Trauma Surgery (DGOU) developed an algorithm based on own scientific

Effects of radiation on the human gastrointestinal tract.

Radiation therapy directed at the abdomen may damage the digestive tract, the type and extent of injury depending on the dose of the radiation and the radiation sensitivity of the gut. Characteristic early changes are manifest in the mucosa of the gut: for later ulceration, changes in the collagen tissues and particularly in the vascular channels occur. This paper describes and characterizes injuries to the esophagus, stomach, small intestine and colon. It emphasizes the importance of recognizing radiation-induced damage to the gut which may occur early or late after radiation.

Wound Trauma Exacerbates Acute, but not Delayed, Effects of Radiation in Rats: Mitigation by Lisinopril.

The goal of this study is to understand and mitigate the effects of wounds on acute radiation syndrome (ARS) and delayed effects of acute radiation exposure (DEARE), for preparedness against a radiological attack or accident. Combined injuries from concomitant trauma and radiation are likely in these scenarios. Either exacerbation or mitigation of radiation damage by wound trauma has been previously reported in preclinical studies. Female WAG/RijCmcr rats received 13 Gy X-rays, with partial-body shielding of one leg. Within 2 h, irradiated rats and non-irradiated controls were given full-thick

Radiation damage to the lung: mitigation by angiotensin-converting enzyme (ACE) inhibitors.

Concern regarding accidental overexposure to radiation has been raised after the devastating Tohuku earthquake and tsunami which initiated the Fukushima Daiichi nuclear disaster in Japan in March 2011. Radiation exposure is toxic and can be fatal depending on the dose received. Injury to the lung is often reported as part of multi-organ failure in victims of accidental exposures. Doses of radiation >8 Gray to the chest can induce pneumonitis with right ventricular hypertrophy starting after ∼2 months. Higher doses may be followed by pulmonary fibrosis that presents months to years after exposu

Radiation safety and ergonomics in the electrophysiology laboratory: update on recent advances.

PURPOSE OF REVIEW: Risks associated with exposure to ionizing radiation in patients undergoing electrophysiology procedures and interventional cardiac electrophysiologists performing these procedures are a serious concern. Strategies to reduce radiation exposure are of obvious importance. In addition, interventional cardiac electrophysiologists have to perform procedures wearing heavy lead protection for prolonged periods, making them prone to cervical and lumbar spinal injuries. RECENT FINDINGS: Recently developed technologies, such as low-exposure radiographic imaging, novel radiographic im

[Prevention of radiation damage to the bladder and rectum using local application of dimethyl sulfoxide].

A method for the prevention of radiation injuries of the urinary bladder and rectum for cervical cancer patients was worked out. It was based on the local application of the radioprotective agent dimethylsulfoxide (DMSO) before a session of interstitial irradiation with the AGAT-B apparatus. Concomitant radiation therapy with DMSO was provided to 22 cervical cancer patients. The control group included 59 patients who received similar treatment without DMSO. The expression of early reactions and late injuries of the rectum and urinary bladder were significantly lower in the DMSO group. A radiop…

Radiation necrosis: relevance with respect to treatment of primary and secondary brain tumors.

Radiation injury to the central nervous system (CNS) manifests in multiple forms and is divided into three categories termed acute, early-delayed, and late-delayed injury patterns. Late-delayed radiation injury, primarily manifesting as leukoencephalopathy or radiation necrosis, is often progressive and may have a negative impact on quality of life. Radiation injury is believed to be a consequence of cell membrane and DNA injury with a pathological expression as vascular injury, depletion of oligodendroglial progenitor cells, and failure of cell-cell interactions that constitute the cellular n

[Consequences of radiation on tissues].

Tissular and cellular radiation-induced injuries. The authors examine morphological, tissular and cellular radiation-induced injuries utilizing anatomo-clinical case reports and experiments in the pig. One must distinguish early and delayed tissular lesions. In the latter, appearing in general after at least six months, vasculo-connective changes are the most important, leading to sclerosis. Indeed, because of the vascular damages the histo-haematic barrier is enlarged leading to a decrease of tissular resistance to injury, long and difficult tissular repairs after a surgical operation. Radiation hygienic evaluation of nuclear fission products (review).

Nuclear-fission products (NFP) are a complex mixture of radionuclides of variable organotropism. Their physicochemical properties depend on the conditions of their formation and age. In a contaminated environment they are a source of external, contact, and internal gamma- and beta-irradiation. Dangerous is the entry of NFP into the organism, especially that of children and pregnant women. In a radiation-hygiene review, the author sums up his own materials and those of other investigators on radiation injuries developing upon the contamination of the skin and penetration of NFP into the organis…

Radiation, diagnosis, and management.

The biomedical effects of ionizing and nonionizing radiations include responses that are known to have thresholds ("nonstochastic" effects) as well as responses that are presumed to have no thresholds ("stochastic" effects). The latter include mutagenic, carcinogenic, and teratogenic effects. Present radiation risk management strategies seek to protect completely against nonstochastic effects by preventing anyone from accumulating a dose of radiation in excess of the relevant threshold. Protection against stochastic effects, on the other hand, is sought by limiting the level of radiation expos

Nuclear/radiological terrorism: emergency department management of radiation casualties.

Recent world events have increased concern that hospitals must be prepared for radiological emergencies. Emergency departments (EDs) must be ready to treat patients suffering from injuries in combination with radiation exposure or contamination with radioactive material. Every hospital should have a Radiological Emergency Medical Response Plan, tested through periodic drills, which will allow effective handling of contaminated and injured patients. Treatment of life-threatening or severe traumatic injuries must take priority over radiation-related issues. The risk to ED staff from radioactive

Essentials of the system of radiological protection.

The system of radiological protection (the 'System') developed by the International Commission on Radiological Protection (ICRP) is built on nearly a century of efforts of numerous scientists and practitioners working together internationally. It rests on three enduring pillars: science, ethics, and experience. These pillars support the three fundamental principles that shape radiological protection strategies: justification, optimisation, and application of dose limits. Of note, optimisation of protection must be understood as a flexible and context-sensitive process, which allows identificat
