Which patients undergo a surgical reintervention following a distal radius fracture?

Not all malunited distal radius fractures cause pain or functional impairment, and only few patients require a surgical correction or a salvage procedure. The purpose of this retrospective study was to investigate differences between 38 patients who underwent a reintervention and 65 patients who did not. The only discriminative variable in the current study was age : 46 years in the reintervention group versus 59 years in the non-reintervention group (p < 0.0001). Differences in gender or type of malunion were not significant, and thus not discriminative. It can be concluded that patient age m…

Use of bone graft substitute in the treatment for distal radius fractures in elderly.

INTRODUCTION: Fractures of the distal radius in elderly patients are often associated with metaphyseal defects that can lead to collapse, malunion and therefore decreased function. An alternative approach to simple reduction is to fill the defects with materials that can provide structural support. METHODS: We used synthetic hydroxyapatite (HA) in unstable fractures of the distal radius in thirty-one elderly patients, of which four patients lost to follow-up, leaving twenty-seven patients for this study. All subjects underwent closed reduction with K-wire fixation and HA augmentation. They we

Incidence and clinical outcomes of tendon rupture following distal radius fracture.

PURPOSE: To evaluate the incidence of tendon rupture after nonoperative and operative management of distal radius fractures, report clinical outcomes after tendon repair or transfer, and examine volar plate and dorsal screw prominence as a predictor of tendon rupture. METHODS: We performed a retrospective chart review on patients treated for tendon rupture after distal radius fracture. We evaluated active range of motion, Disabilities of Arm, Shoulder, and Hand score, grip strength, and pain score, and performed radiographic evaluation of volar plate and dorsal screw prominence in both the st…

Dorsal cortical comminution as a predictor of redisplacement of distal radius fractures in children.

BACKGROUND: The purpose of this study was to evaluate the various factors, which could contribute towards redisplacement of distal radius fractures, including comminution of the dorsal cortex of the distal radius, treated in our department. METHODS: In this retrospective study, we evaluated the risk of redisplacement of distal radius fractures in our department and also looked at the probable factors predisposing to this risk. A total of 134 fractures(129 children) were included in the study after exclusions. The variables that were assessed as possible causes of redisplacement were age, gend…

What are the radiological predictors of functional outcome following fractures of the distal radius?

The fracture most commonly treated by orthopaedic surgeons is that of the distal radius. However, as yet there is no consensus on what constitutes an 'acceptable' radiological position before or after treatment. This should be defined as the position that will predict good function in the majority of cases. In this paper we review the radiological indices that can be measured in fractures of the distal radius and try to identify potential predictors of functional outcome. In patients likely to have high functional demands, we recommend that the articular reconstruction be achieved with less th

Finite element prediction of surface strain and fracture strength at the distal radius.

To better understand the mechanisms underlying distal radius fracture we have developed finite element models to predict radius bone strain and fracture strength under loading conditions simulating a fall. This study compares experimental surface strains and fracture loads of the distal radius with specimen-specific finite element models to validate our model-generating algorithm. Five cadaveric forearms were instrumented with strain gage rosettes, loaded non-destructively to 300 N, and subsequently loaded until failure. Finite element models were created from computed tomography data; three s

Complex fracture-dislocations of the proximal ulna and radius in adults: a comprehensive classification.

BACKGROUND: Complex fracture-dislocations of the proximal ulna and radius include multiple anatomic lesions, the management of which is known to be demanding. Although several classifications have been proposed, none appear to be exhaustive, and most of them have neither therapeutic nor prognostic value. The purpose of this study was to design a comprehensive classification that may provide a guide for the operative management of these injuries. MATERIALS AND METHODS: The classification is aimed at identifying definite anatomic lesions, called the "main lesions," the presence of which can aff

A new index for the assessment of distal radius fractures involving the ulnar styloid.

OBJECTIVE: The aim of this retrospective study was to evaluate the relation between a new index we created for the assessment of distal radius fractures involving the ulnar styloid, and the clinical outcome. METHODS: We devised a radiographic separation index (RSI), to evaluate the displacement of the ulnar styloid. We used this index in 44 patients (28 men and 16 women; mean age: 43.2 years; range: 24-64 years) with distal radius fractures involving the the ulnar styloid. In all cases, the distal radius fracture was fixed using a volar locking plate. The styloid fracture was treated conserva…

An economic analysis of outcomes and complications of treating distal radius fractures in the elderly.

PURPOSE: There is a lack of scientific data regarding which treatment provides the best outcome for distal radius fractures (DRFs) in the elderly. Currently, casting is used to treat the majority of these fractures, although open reduction and internal fixation (ORIF) has been used increasingly in recent years. Given the recent emphasis on the wise use of medical resources, we conducted a cost-utility analysis to assess which of 4 common DRF treatments (casting, wire fixation, external fixation, or ORIF) optimizes the cost-to-patient preference ratio. METHODS: We created a decision tree to mo…

[Evolution of the treatment of distal radius fractures in Spain. Is this the right way to go?].

OBJECTIVE: To assess the changes in the treatment of distal radius fractures that have occurred in recent years at our center. To analyze the changes that have occurred and their possible causes. MATERIAL AND METHODS: We analyzed the hospital admission data from our center, both total admissions and those due to distal radius fractures. Surgical activity records show the evolution of the surgical treatment of distal radius fractures throughout time. Using the data from 2 research works done at our center we assessed the radiological and functional results obtained with an external fixator and…

Predictors of acute carpal tunnel syndrome associated with fracture of the distal radius.

PURPOSE: A better understanding of the risk factors for acute carpal tunnel syndrome (CTS) associated with fracture of the distal radius might influence recommendations for prophylactic carpal tunnel release. METHODS: Fifty patients who had release of an acute CTS in association with open reduction and internal fixation (ORIF) of a fracture of the distal radius were identified from orthopedic trauma databases at 2 institutions. Each patient was matched with a control patient (ORIF, but no acute CTS) of the same gender, similar age (+/-4 years), and similar injury mechanism. RESULTS: The prev

Transverse divergent dislocation of the elbow with ipsilateral distal radius fracture in a child.

We report a rare case of an irreducible transverse divergent dislocation of the elbow with an ipsilateral distal radius torus fracture and a fracture of the coronoid process in a 9-year-old male. Closed reduction of the elbow was attempted, but the humeroulnar joint remained dislocated. At surgery, the avulsed anterior band of the medial collateral ligament complex of the elbow was found to be interposed between the medial condyle of the humerus and the olecranon. The dislocation was reduced after relieving the ligament entrapment, which was then repaired. The unstable proximal radioulnar join

A significant age difference between those with and without subsequent distal radius fractures was found (p = 0.01) but not a significant difference between patients with or without osteoporosis medication (p = 0.79), primarily because the case numbers were too small. CONCLUSIONS: A substantial proportion of postmenopausal women hospitalized …

Displaced fractures of the neck of the radius in adults. An excellent long-term outcome.

We have reviewed 20 women and three men aged 22 to 73 years, who had sustained a Mason type-IIb fracture of the neck of the radius 14 to 25 years earlier. There were 19 patients with displacement of the fractures of 2 mm to 4 mm, of whom 13 had been subjected to early mobilisation and six had been treated in plaster for one to four weeks. Of four patients with displacement of 4 mm to 8 mm, three had undergone excision and one an open reduction of the head of radius. A total of 21 patients had no subjective complaints at follow-up, but two had slight impairment and occasional elbow pain. The me

[External fixator by Mitkovic in the treatment of comminuted intraarticular fractures of the distal radius].

INTRODUCTION: Comminuted intraarticular fractures of the distal radius metaphysis are a major challenge for orthopedic surgeons. The aim of this study was to present results of the survey on treatment of these fractures using an external fixator. MATERIAL AND METHODS: 73 patients (30 females and 43 males) with closed comminuted intraarticular fractures of the distal radius, type C AO/ASIF (based on radiography at the moment of injury) were treated by a Mitkovic external fixator and followed-up for at least 2 years. An external fixator and Kirschner wires were used in 43 patients. An external

Combined fracture of the distal radius and scaphoid in children. Report of 2 cases.

The authors report 2 cases of concomitant scaphoid and distal radial fractures, a rare combination of lesions in children. The first case was in a 13-year-old boy who presented a transverse midscaphoid fracture associated with a Salter type II distal radial fracture. The second concerned a 10-year-old boy who presented a bilateral wrist injury. On the left side, a distal forearm fracture with anterior displacement was associated with a transverse midscaphoid fracture. On the right side, there was only a distal forearm fracture. Scaphoid fracture associated with distal radial fracture seems to

[Dislocation of the Ulnar Head following Malunion of Distal Radius Fracture in Childhood].

Four children between the age of 10 and 13 years with distal radius fractures healed with dorsal angulation presented to us in our out-patient department. All had been treated conservatively with an upper-arm cast, all fractures had healed with an average dorsal angulation of 10 degrees. Despite this dorsal angulation, the patients were pain free. Subsequent minor injury resulting in pain to the wrist was now the reason for their presentation. On examination we found a spontaneous palmar dislocation of the ulnar head during supination. This proved to be resistant to conservative therapy. In al…
