Long-term stability of conservative orthodontic treatment in a patient with rheumatoid arthritis and severe condylar resorption.

Rheumatoid arthritis is a chronic inflammatory condition that can result in progressive destruction of the articular surfaces of the joints, including the temporomandibular joint. The purpose of this article is to report the conservative correction of a Class II malocclusion in a woman with rheumatoid arthritis. The patient was 32 years 6 months old at the start of treatment. She had a convex profile and a skeletal Class II jaw-base relationship caused by severe condylar resorption. An anterior open bite of -2.0 mm and an excessive overjet of 10.0 mm were observed. Severe crowding was shown in

Cephalometric analysis of pharyngeal airway space dimensions in Turner syndrome.

Turner syndrome (TS) that is due to a total or partial lack of an X chromosome affects about 1 in 2000 girls. The syndrome is characterized by short stature and gonadal dysgenesis. Its documented craniofacial features include retrognathic jaws, a short mandible, and a large cranial base angle. Our aim was to find out whether the syndrome also has an effect on the pharyngeal airway space. We retrospectively analysed lateral cephalograms of 35 TS subjects whose age ranged from 6.5 to 21 years and of 35 healthy female controls matched for age. On those, we did 7 linear and 10 angular cephalometri

[Pathogenic situations after orthodontic treatment. Defects in skeleton and soft tissues that should be avoided after orthodontic treatment].

The perfect correction of dental alignment anomalies found in dento facial dysmorphosis should not be the only criterium of success in dento facial orthopedics. One must insure, lest severe T.M.J. disorders and sleep apnea problems subsequently occur, that : (1) the condyles and mandibular vertical branches are correctly positioned relative to the basilar apophysis and cervical vertebrae, (2) the hyoid bone must not descend too low relative to the C3-C4 intervertebral disk.

Complications associated with maxillary osteotomies.

There are certain complications involved in maxillary ostetomies; devitalization of teeth, loss of part or all of the osteotomized segment, and relapse are possible problems. The paramount cause of the complications in seven different cases seemed to be the interruption of the blood supply to the operative site. A seven-point guideline is formulated; it integrates preoperative planning, surgical procqedures, and postoperative care.

Long-term results of surgically assisted maxillary protraction vs regular facemask.

OBJECTIVE: To evaluate the short- and long-term treatment results of rapid maxillary expansion (RME) and surgical assistance during maxillary protraction with a facemask (FM). MATERIALS AND METHODS: This study was carried out in 28 patients (12 male, 16 female) with maxillary retrognathism, anterior crossbite, and Class III skeletal and dental malocclusion characteristics. Seventeen patients (9 male, 8 female) with mild maxillary retrognathism were treated by RME and FM. The other 11 patients (8 female, 3 male), who had moderate to severe maxillary retrognathism, were treated with surgically

Orthodontic and orthognathic management of a complex mandibular asymmetry.

The application of a combined surgical-orthodontic procedure for the treatment of major facial deformities (eg, asymmetries) and associated malocclusions has become an increasingly prevalent treatment modality over the past few decades. Development of sound conjoint treatment principles made possible the correction of skeletal and dental dysplasias that were unyielding to either surgical or orthodontic treatment alone. The presence and severity of dentofacial asymmetries has been the subject of many commentaries and investigations. Treatment of facial asymmetry has always been a challenge with

The orthodontic and surgical management of Zimmerman-Laband syndrome.

Zimmermann-Laband syndrome (ZLS) is a rare autosomal disorder characterized by gingival fibromatosis, abnormalities of the nose and/or ears, hypoplasia of the nails or terminal phalanges of the hands and feet, hyperextensibility of joints, hepatosplenomegaly, hirsutism, and intellectual disability. The characteristics of the syndrome are highly variable and complicated. This paper reports a case of ZLS and the clinical manifestations encountered in relation to the current available literature. The comprehensive management of the patient's dentition is described, focusing on the orthodontic and

A case of amelogenesis imperfecta, cleft lip and palate and polycystic kidney disease.

OBJECTIVE: Amelogenesis imperfecta (AI) is a heterogeneous group of genetic disorders characterized by developmental abnormalities of tooth enamel. The AI is also seen as part of multi-organ abnormalities, e.g. with cone-rod dystrophy, hypothalamo-hypophyseal insufficiency and renal failure. The present patient with AI and nephrocalcinosis exhibited a phenotype different from previous cases with renal failure. To highlight the characteristics of this rare case, extensive analysis that included histological, biochemical and genetic examinations was performed. PATIENT: The present Japanese male

[The importance of condyle position in the diagnosis, treatment and prevention of TMD].

When a patient presents with TMJ pain, it could be all too easy for the orthodontists to turn the other way claiming that their occlusion has no connection with their symptoms and that they are suffering from a biopsychosocial disorder. This abundantly illustrated article attempts to demonstrate that patients can appear to display a stable occlusion even though their TMJ situation is unstable. This can give rise to dental abrasion, articular adaptations and pain. Orthodontics allows us to treat patients in pain provided that the treatment plan takes into account the need to restore a stable TM

Interdisciplinary treatment of an adolescent with unilateral cleft lip and palate.

AIM: The present case report describes the importance of interdisciplinary approach and gives an understanding on management of an adolescent with unilateral cleft lip and palate. BACKGROUND: Failure of fusion between medial nasal process and maxillary process or between the palatal process leads to the formation of clefts. Clefts are result of genetic or environmental factors or a combination of both. Common dental problems associated with clefts includes anterior and posterior crossbites, hypodontia, malformation and abnormal eruption pattern. CASE REPORT: A girl, aged 15 years reported wi

A comparison of current prediction imaging programs.

To investigate perceived differences in the ability of current software to simulate the actual outcome of orthognathic surgery, we chose 10 difficult test cases with vertical discrepancies and "retreated" them using the actual surgical changes. Five programs--Dentofacial Planner Plus, Dolphin Imaging, Orthoplan, Quick Ceph Image, and Vistadent--were evaluated, by using both the default result and a refined result created with each program's enhancement tools. Three panels (orthodontists, oral-maxillofacial surgeons, and laypersons) judged the default images and the retouched simulations by ran

Functional rehabilitation using orthognathic surgery, bone transplants and implants after irradiation of malignancy in early childhood.

Functional rehabilitation has been carried out in four young patients by use of orthognathic surgery, bone grafting and implants. All four patients had been successfully treated for malignant tumours in the midface during early childhood by use of full dose irradiation. Retarded growth of the maxilla together with missing root formation of permanent teeth prevented the alveolar process to develop. Bimaxillary osteotomies with inlay bone graft have been made in two cases and maxillary osteotomy le Fort I with interpositional and inlay grafting in two individuals. Implants surgery has been made
