Managing aflatoxin risk in the peanut supply chain using quantitative metrics.

The Food Safety Objective (FSO) is a concept that specifies the maximum level of hazard that can be tolerated in a food at the time of consumption, guaranteeing an appropriate level of protection for consumer health. It is a tool that provides risk management throughout the food production chain from the field to the consumer. The application of this concept to mycotoxins was performed for the first time for aflatoxins in peanuts, but in a conceptual and theoretical way, without applying quantitative metrics. FSO metrics for mycotoxin management have been applied to aflatoxins in pistachios an…

Anaesthesiologic management of patients at risk of pulmonary aspiration: a Swiss consensus statement.

INTRODUCTION: The perioperative management of patients at risk of pulmonary aspiration varies widely, includes several controversial aspects, and is not regulated by official guidelines. The objective of this study was to achieve peer and expert consensus in the management of patients at risk of aspiration, thereby developing guidance in Switzerland. Controversial aspects to be clarified by expert consensus were previously identified by a broad national survey. METHODS: A structured, three-phase national consensus process was conducted in Switzerland. First, the interprofessional SAFE Survey i

Pharmacological pitfalls in inpatient lung transplant care and management of medication risks.

Pharmacotherapy after lung transplantation is complex and prone to drug-drug interactions, dosing errors, and toxicity. We retrospectively analysed 259 comments from weekly interdisciplinary rounds at the University Hospital Zurich between September 2022 and April 2024 to identify common pharmacological pitfalls in inpatient lung transplant recipients. The cohort included 62 patients (mean age 56.1 years; 40.3% female). Most comments concerned potential risks rather than confirmed harm: 54.4% were classified as potential side effects and 21.6% as potential loss of effectiveness. Interactions w

A standardised risk-stratified approach to the urological management of children with spina bifida.

BACKGROUND: The establishment of a multidisciplinary spina bifida (SB) clinic in 2006 resulted in a review of the literature and an audit of renal outcomes based on then management practices. The audit showed 17% new onset renal scarring over a mean 5.8-year follow-up period. This prompted the development of a local protocol based on risk stratification combining serial ultrasound and non-invasive bladder function assessments, with invasive urodynamic studies reserved for high-risk patients. OBJECTIVE: This study sought to assess the impact of a risk stratified protocol on renal scarring and

[Pain management during pregnancy : A balancing act between benefits and risks].

Pain management during pregnancy poses a considerable challenge in everyday clinical practice. The relief of maternal discomfort must be weighed against the potential risks of using medication (nonsteroidal anti-inflammatory drugs, paracetamol, steroids, opioids, gabapentinoids). The current data on congenital anomalies, complications during pregnancy, neonatal adaptation or withdrawal symptoms, and long-term psychomotor developmental disorders in children exposed in utero must be taken into account. However, refraining from treating chronic maternal pain during pregnancy can also cause proble

Urinary Biomarkers for the Risk Assessment and Management of Heart Failure: Pearls and Pitfalls.

PURPOSE OF REVIEW: We review potential applications and pitfalls of urinary biomarkers in people with heart failure (HF). RECENT FINDINGS: Albuminuria may indicate kidney damage and guide prioritization of medications in HF. Urine sodium is useful in guiding diuretic management. Several experimental urine biomarkers have been evaluated, with the most robust data available for neutrophil gelatinase-associated lipocalin, kidney injury marker 1, insulin-like growth factor-binding protein 7 in conjunction with tissue inhibitor of metalloproteinases-2, angiotensinogen, and N-acetyl-β-D-glucosamini

Thrombocytopenia in Pregnancy: Clinical Challenges, Maternal-Fetal Risks, and Management Strategies.

Thrombocytopenia affects up to 10% of pregnant women and represents the second most common blood disorder during pregnancy. Its causes include immune-mediated thrombocytopenia, hemolysis, elevated liver enzymes and low platelet count (HELLP) syndrome, preeclampsia (PE), and benign pregnant thrombocytopenia. Diagnosis is crucial because the cause dictates the effect on maternal health, pregnancy management, and neonatal outcomes. This narrative review examines the range of thrombocytopenia during pregnancy, primarily focusing on diagnostic evaluation, underlying pathophysiological causes, and d

Frailty and falls in older adults: risk assessment and management for community nurses.

Frailty and falls are highly prevalent among community-dwelling older adults and are linked through a bidirectional relationship that increases the risk of injury, hospitalisation, functional decline and mortality. Community nurses play a central role in early identification and prevention. This article examines the relationship between frailty and falls, and outlines practical approaches to risk assessment and management in community nursing practice. The authors present a clinically focused synthesis of contemporary evidence and national guidance relating to frailty and falls in community-dw

Defining and managing high-risk acute myeloid leukemia (AML) in 2026.

Acute myeloid leukemia (AML) remains a highly heterogeneous malignancy in which outcomes are particularly poor for patients classified as having high-risk disease. Traditionally, high-risk AML has been defined by adverse baseline genetic features, including complex cytogenetics, TP53 alterations, and mutations associated with secondary or therapy-related disease. However, this static, genetics-centered definition is increasingly insufficient in the modern therapeutic era. Emerging evidence supports a more dynamic and context-dependent model in which risk is shaped not only by molecular archite

[Meningioma Management in the Era of Molecular Classification:From Risk Stratification to Personalized Treatment].

Meningiomas have traditionally been classified and treated according to histopathological grade and extent of resection. However, recent advances in molecular biology have revealed that histology alone does not fully explain the clinical behavior of these tumors. The 2021 World Health Organization classification introduced molecular criteria for meningioma grading, including CDKN2A/B homozygous deletion and TERT promoter mutation as defining features of grade 3 disease. In parallel, large-scale multi-omics studies integrating genomic, epigenomic, and transcriptomic data have established biolog

A multimodal lifestyle intervention management system for people at risk of cognitive decline.

Dementia stands as the primary reason for disability among older adults. Research indicates that dementia's risk factors are a blend of pathological, lifestyle, and genetic elements, with nearly 40% of the potential risk factors being modifiable. In the absence of a definitive cure, the foremost strategies for managing dementia involve its early detection and measures to decelerate its progression. In this regard, lifestyle multimodal preventive strategies have emerged, guiding precision prevention of dementia. Based on the LETHE clinical trial, a digitized version of the FINGER protocol, we p

Gastrointestinal toxicities associated with immune checkpoint inhibitors therapy: risks and management.

Immune checkpoint inhibitors (ICIs) have greatly improved cancer treatment by boosting the immune system's ability to target tumors. However, they can also cause serious side effects, particularly in the digestive system. These include immune-related diarrhea, inflammation of the intestines and, less commonly, inflammation of the stomach or esophagus. This review underscores the importance of early detection, accurate diagnosis, and timely treatment to improve patient outcomes. It also highlights the need for further research to develop strategies to reduce gastrointestinal toxicities and enha

Risks Associated With Extent of Surgical Management for Benign, Non-Toxic Goiter.

OBJECTIVES: Surgery for benign, non-toxic thyroid goiter remains the most common indication for thyroidectomy worldwide, yet the optimal surgical extent remains controversial. While total thyroidectomy is assumed to have greater risks than lobectomy, the difference in complications has not been quantified. The objectives of this study are to (1) compare the risk of surgical complications and (2) quantify the magnitude of this risk difference. METHODS: This is a retrospective analysis utilizing the National Surgical Quality Improvement Program (NSQIP) Thyroid-Specific Dataset harmonized with t

Balancing Risks in Obstetrics: Anesthesia Management in Facioscapulohumeral Muscular Dystrophy with Scoliosis.

Facioscapulohumeral muscular dystrophy (FSHD), the third most common form of muscular dystrophy, is associated with a normal life expectancy, resulting in more pregnant women presenting with the condition. Pregnancy often exacerbates muscle weakness in patients with FSHD, posing unique perioperative challenges. Careful evaluation of systemic involvement is essential to minimize maternal and neonatal risks. Perioperative management is often challenging because both general and neuraxial anesthesia carry significant risks. Spinal deformities further complicate anesthesia management, requiring tr

Risk stratification and management of arrhythmias in patients with Wolff-Parkinson-White syndrome.

Wolff-Parkinson-White (WPW) syndrome is a cardiac disorder characterized by an accessory pathway known as the bundle of Kent, which bypasses the atrioventricular node and predisposes patients to tachyarrhythmias, including supraventricular tachycardia and atrial fibrillation. The prevalence of WPW syndrome ranges from 0.1% to 0.5% globally, with a higher incidence in males and diagnosis typically occurring between ages 10 and 30. Although many individuals remain asymptomatic, WPW presents a risk of life-threatening arrhythmias, including sudden cardiac death, particularly when atrial fibrillat
