Biceps tendon autograft augmentation for rotator cuff and instability procedures: a narrative review.

Rotator cuff tears (RCT) and instability are the most common surgically treated shoulder pathologies. The concept of augmentation using the long head of the biceps tendon (LHBT) autograft was created to improve the results of surgical treatment of these pathologies, especially in cases of chronic and massive injuries. The popularity of using the LHBT for augmentation is evidenced by the significant number of publications on this topic published in the last 3 years; however, only one systematic review has been published regarding only LHBT augmentation for massive RCTs. Several studies comparin

[Injury pattern in shoulder dislocation in the elderly patient].

PROBLEM: Recurrent shoulder dislocation is reported to be rare in older patients. While injuries of the glenoid labrum and the anterior capsule following primary dislocation in younger patients seem to determine the rate of instability, the pattern of damages in the elderly has rarely been studied. METHODS: 91 patients with an age older than 40 years with the diagnosis of primary (group A, n = 50) or recurrent (group B, n = 41) anterior shoulder dislocation were included in a prospective study. By analysis of the findings in x-rays, MRI, CT-scans and in 36 patients of diagnostic arthroscopy t

The Effect of Vitamin D Deficiency on Post-operative Outcomes Following Rotator Cuff Repair: A Meta-Analysis.

Rotator cuff injuries (RCIs) are common and debilitating injuries, particularly among older adults. Surgical repair, such as arthroscopic rotator cuff repair (RCR), is the preferred treatment, with pre-operative risk factors, including vitamin D deficiency, associated with poor outcomes, but the relationship between vitamin D deficiency and outcomes following RCR remains uncertain. The purpose of this meta-analysis was to investigate the relation between pre-operative vitamin D deficiency and post-operative outcomes following RCR. A systematic review and meta-analysis were conducted to explore…

Retrospective analysis of ultrasound-guided hydrodistension in patients with post-traumatic adhesive capsulitis of the shoulder.

BACKGROUND: Adhesive capsulitis (AC) is a painful shoulder condition characterized by limited motion due to fibrosis and contracture of the joint capsule. Secondary AC frequently occurs after upper limb fractures and immobilization, delaying recovery. METHODS: This retrospective study analyzed 11 patients with post-traumatic AC (< 3 months) following non-displaced humeral fractures treated conservatively with a median immobilization period of 25 days. The cohort included seven men and four women (mean age 52.6 years), five of whom also had rotator cuff injuries. Treatments involved hydrodiste…

Engineering an extracellular matrix-functionalized, load-bearing tendon substitute for effective repair of large-to-massive tendon defects.

A significant clinical challenge in large-to-massive rotator cuff tendon injuries is the need for sustaining high mechanical demands despite limited tissue regeneration, which often results in clinical repair failure with high retear rates and long-term functional deficiencies. To address this, an innovative tendon substitute named "BioTenoForce" is engineered, which uses (i) tendon extracellular matrix (tECM)'s rich biocomplexity for tendon-specific regeneration and (ii) a mechanically robust, slow degradation polyurethane elastomer to mimic native tendon's physical attributes for sustaining

The Hug-up Test: A New, Sensitive Diagnostic Test for Supraspinatus Tears.

BACKGROUND: The supraspinatus tendon is the most commonly affected tendon in rotator cuff tears. Early detection of a supraspinatus tear using an accurate physical examination is, therefore, important. However, the currently used physical tests for detecting supraspinatus tears are poor diagnostic indicators and involve a wide range of sensitivity and specificity values. Therefore, the aim of this study was to establish a new physical test for the diagnosis of supraspinatus tears and evaluate its accuracy in comparison with conventional tests. METHODS: Between November 2012 and January 2014,

Understanding and preventing complications in repairing rotator cuff tears.

Repair of rotator cuff tears is a common procedure. Prior to approaching this surgery, it should be realized that each surgical step can lead to complications, including those related to positioning and anaesthesia. Stiffness, infection and failure of repair are the more frequent complications reported.

MR imaging of delamination tears of the rotator cuff tendons.

OBJECTIVE: The objective was to describe the imaging appearances and location of delamination tears of the rotator cuff tendons on non-contrast conventional MR imaging. SUBJECTS AND METHODS: This study was reviewed and approved by our Institutional Review Board. The reports of 548 consecutive MR examinations of the shoulder were reviewed, looking for mention or description of delamination tears of the rotator cuff. The images of the identified cases were then reviewed by two radiologists to confirm the findings. Correlation with surgical and arthroscopic information was then performed. Delami

Shoulder magnetic resonance imaging.

MRI provides excellent soft tissue contrast and allows for multiplanar imaging in anatomic planes. Because of these advantages MRI has become the study of choice for imaging of shoulder pathology. Some structures, such as the rotator cuff, humeral head contour, and glenoid shape, are evaluated well with conventional MRI. When more sensitive evaluation of the labrum, capsule, articular cartilage, and glenohumeral ligaments is required or when a partial-thickness rotator cuff tear is suspected, magnetic resonance arthrography with intra-articular contrast can be performed. For MR arthrography con

A rare anatomic variant of the superior glenohumeral ligament.

The attachment of the superior glenohumeral ligament (SGHL) to the upper pole of the glenoid is variable and 3 types have been described. We report an anatomic variant of SGHL attachment to the upper pole of the glenoid that has not heretofore been reported in the literature. In this case, the SGHL overrode the biceps origin, continued to the superior labrum posteriorly, and had no attachment to the middle glenohumeral ligament or the anterior labrum. This variant was detected during routine arthroscopic examination undertaken before surgery on a rotator cuff tear.

Age-related prevalence of rotator cuff tears in asymptomatic shoulders.

To determine the prevalence of rotator cuff tears in asymptomatic shoulders we conducted a prospective clinical and ultrasonographic study of 411 volunteers. We anticipated an age-dependent outcome and divided the patients into 4 age-groups. Overall, we found evidence of a rotator cuff tear in 23% of the patients. In group 1 (aged 50 to 59 years), 13% (22 of 167) of the patients had tears; in group 2 (aged 60 to 69 years), 20% (22 of 108) of the patients had tears; in group 3 (aged 70 to 79 years), 31% (27 of 87) of the patients had tears; and in group 4 (age > 80 years), 51% (25 of 49) of the

[Complex trauma of the shoulder girdle].

Complex injuries of the shoulder require differentiated management. Interestingly there is a relative lag of warranted information on this subject. Generally the different lesions are discussed separately. However it is not acceptable to simply add the standard diagnostic and therapeutic measures, because major tactic, strategic and rehabilitation incompatibilities could occur. Diagnostic and therapeutic difficulties are frequent for the following injuries: complex proximal humerus dislocation fractures, dislocation fractures with associated lesions of the rotator cuff, posterior dislocation f

Anterior dislocation of the shoulder in elderly patients.

Of 545 consecutive patients with anterior shoulder dislocations, 108 (20%) were aged 60 years or more at the time of injury. We reviewed and radiographed 95 of these elderly patients after a mean follow-up of 7.1 years. Axillary nerve injuries were seen in 9.3% of the 108 patients, but all recovered completely in 3 to 12 months. There were single or multiple recurrences of dislocation in 21 patients (22.1%), but within this group age had no influence on the tendency to redislocate. Tears of the rotator-cuff were diagnosed by imaging studies or clinically in 58 patients (61%), including all who

[Overuse injury syndromes in the shoulder area].

The shoulder is the most movable and the most unstable joint in a human body. The glenohumeral joint is particularly vulnerable because overhead activities put tremendous stress on its static stabilizers (ligamentous labral complex) and the dynamic stabilizers (rotator cuff muscles). Overhead activities may stretch or injure the static stabilizers. Minor aberrations in mechanisms controlling stability have a significant and cumulative effect on the shoulder biomechanics, and increase the risk of injury. Impingement syndrome and glenohumeral instability are predominant injuries affecting the sh

Shoulder pathoanatomy in marathon kayakers.

OBJECTIVES: To determine the prevalence of soft and hard tissue abnormalities and their interrelations in the shoulders of marathon kayakers … clavicular spurs were found to correlate highly with supraspinatus muscle pathology. CONCLUSIONS: Rotator cuff injuries make up a large portion of the injuries seen in marathon kayakers, about twice the number reported for sprint kayakers. These injuries are the result of secondary impingement factors associated with overuse, possibly specific to kayakers, and not th …

Mitochondrial transfer and transplantation in tendon, ligament, and enthesis repair: current evidence, mechanistic rationale, and regenerative opportunities.

Tendon, ligament, and enthesis injuries often heal through fibrovascular scar rather than restoration of native matrix organization, mechanical function, and graded interface architecture. Mitochondrial dysfunction has emerged as a recurrent feature of these repair failures, linking impaired bioenergetics, oxidative stress, persistent inflammation, cell death, and defective matrix remodeling. In parallel, mitochondria are increasingly recognized as transferable organelles that can move between cells or be delivered therapeutically as isolated mitochondria, mitochondria-rich extracellular vesic

Comparative Analysis of Various Rotator Cuff Stretching Techniques: Efficacy and Recommendations for Gym Enthusiasts.

Shoulder pain is a common complaint among gym-going individuals, particularly those engaged in upper limb workouts. The rotator cuff, comprising four muscles, plays a crucial role in stabilizing the shoulder joint during movements and supporting its mobility. Imbalances or weaknesses in these muscles can lead to shoulder injuries, affecting performance and overall well-being. The main aim of this review is to explore the benefit of one of the approaches in preventing shoulder pain and improving performance among gym-going individuals. Specific rotator cuff stretching exercises target the entir
