Postemetic rupture of the esophagus: Boerhaave's syndrome.

Postemetic rupture of the esophagus, also known as spontaneous rupture or Boerhaave's syndrome, was first described by Herman Boerhaave in 1724. This is a severe disease that causes high mortality rates and is difficult to diagnose not only because it is rare but also because it is frequently confused with other severe clinical conditions, such as acute myocardial infarction, perforated peptic ulcer, and acute pancreatitis. Herein, we describe three cases of patients with this syndrome. Two underwent primary repair of the rupture, and one underwent esophagectomy followed by reconstruction. The…

Spontaneous splenic rupture in primary cytomegalovirus infection.

INTRODUCTION: Spontaneous splenic rupture is a rare occurrence in primary cytomegalovirus infection. CASES: We report two cases of spontaneous rupture of the spleen associated with primary cytomegalovirus infection in young immunocompetent adults. One patient had iron deficiency anemia, and the other a pyruvate kinase deficiency. Nonoperative management was successful in both cases. DISCUSSION: Nine other cases identified by a search of the medical literature are also reviewed. These cases do not show evidence of any particular risk factor.

Rupture uterus in primigravida: morbidity and mortality.

OBJECTIVE: This paper is aimed to present "Rupture of the uterus (RU) in primigravida "- unscarred cases which are supposed to be extremely rare. MATERIAL AND METHOD: The charts of patients labelled as "Rupture Uterus" for the period 1985 - 2005 AD (2042 - 2061 BS), 20 years were taken out and analysed. RESULT: There were 251 cases of rupture uterus during the study period giving the incidence of 0.09% i.e. 1:1100 among live births in a very busy tertiary maternity hospital of capital, Kathmandu of Nepal. There were 60% spontaneous, 29% scar dehiscence and 11% Iatrogenic/traumatic rupture an…

Dermatomyositis associated with bilateral ureteral spontaneous rupture.

Dermatomyositis associated with genitourinary malignancies is not uncommon. However, only a few cases associated with ureteral stricture have been reported, and ureteral rupture is even rarer. A 56-year-old woman with dermatomyositis who had been treated with prednisolone, cyclophosphamide, and azathioprine for 2 months presented with fever, hematuria, and abdominal pain for 2 days. Radiographic imaging revealed bilateral ureteral spontaneous rupture and intraperitoneal urinoma formation. Urine cytology and endourologic examinations showed no evidence of genitourinary malignancy. Ureteral urin…

[Right atrial rupture after abdominal blunt trauma].

If the cardiac injuries are frequent after closed chest traumatism, the cardiac injuries after abdominal closed traumatism are unusual but serious. We report the case of a right auricular rupture associated with a liver injury after a closed abdominal traumatism. The diagnosis was suspected on the TDM and confirmed by echocardiography. An emergency sternotomy was performed due to sudden haemodynamic instability. The initial clinical signs are often misleading. However the diagnosis must be made quickly and the treatment begun without delay.

Papillary muscle rupture after blunt chest trauma.

We report a case of anterolateral papillary muscle rupture in a 22-year-old man who had blunt chest trauma caused by a car accident. Transesophageal echocardiography revealed severe mitral regurgitation caused by the rupture. He successfully underwent emergency mitral valve replacement and was discharged 9 days after the surgical correction.

Partial rupture of the anterior cruciate ligament.

The anterior cruciate ligament (ACL) consists of two major fiber bundles, namely the anteromedial (AM) and posterolateral (PL) bundles. Although disagreement exists among arthroscopic surgeons about the occurrence of isolated ruptures of the AM or PL bundle, there are reports of partial ruptures of the ACL in the literature. A potential reason for controversy could be that with conventional magnetic resonance imaging, isolated PL ruptures are difficult to diagnose because of the oblique course of this bundle. Another reason could be that isolated ruptures of the AM or PL bundle are difficult t…

[Gastric rupture following cardiopulmonary resuscitation: case report].

BACKGROUND AND OBJECTIVES: Gastric rupture is a rare complication from cardiopulmonary resuscitation (CPR), with a reported incidence of 0.1%. Unawareness of this possible complication during these maneuvers delays its recognition and reduces the patient’s surviving chance. The aim of this report is to describe a case of acute abdomen due to gastric rupture following CPR maneuvers that was promptly diagnosed and treated. CASE REPORT: We report a case of a 76-year-old patient that was admitted to the hospital with pneumonia and also to rule out a possible brain vascular accident (BVA). In the …

Spontaneous common bile duct rupture in pregnancy.

This report describes how a rare condition in pregnancy can present with features resembling more common diagnoses in the unwell obstetric patient. Our patient presented in late pregnancy initially with clinical features similar to mild preeclampsia, with proteinuric hypertension, epigastric pain and mildly deranged liver function tests. She went on to develop signs of acute abdomen associated with a persistent tachycardia and hypotension; there was evidence of fetal compromise. Following resuscitation, emergency caesarean section was performed and evolved into a laparotomy. The diagnosis was

Analysis of 30 breast implant rupture cases.

Breast implants used for augmentation mammoplasty or breast reconstruction could rupture from various causes such as trauma or spontaneous failure. The objectives of this study were to investigate the relationships between the causes of implant rupture and the degree of capsular contracture, and then to evaluate the relative efficacies of specific signs on magnetic resonance imaging (MRI) known to be beneficial for diagnosing the rupture. A retrospective review identified patients with prosthetic implant rupture or impending rupture treated by the senior author. The 30 cases of implant rupture…

Traumatic intrapericardial ascending aortic rupture: CT appearance.

Ascending aortic injury is seen in only 5% of patients who survive long enough to undergo imaging. Emergent aortography is commonly used to confirm and define suspected thoracic rupture. We describe a case of ascending aortic rupture at the root of the aorta following blunt trauma diagnosed with CT imaging.
