Management of malignant carinal involvement using 'side-by-side' method of bilateral self-expandable metallic stents.

Central airway obstruction caused by cancer requires urgent interventional pulmonology. Malignant main carinal involvement is one of the most challenging situations, usually treated by rigid bronchoscopic intervention under general anaesthesia. However, these patients tend to be in poor condition due to underlying malignancy. Therefore, less-invasive strategies are needed. Six patients with lung cancer exhibiting malignant carinal involvement treated using the 'side-by-side' method of bilateral self-expandable metallic stents using fibre-optic bronchoscopy under topical anaesthesia were retros

An integrative literature review to examine the provision of self-management support following transient ischaemic attack.

AIMS AND OBJECTIVES: To identify the types of self-management support that have been provided to persons with transient ischaemic attack and the evidence for the effectiveness of these programmes. BACKGROUND: Self-management is a key element for the effective management of chronic conditions. For persons with transient ischaemic attack, secondary stroke prevention is a recognised standard of care. Best practice guidelines indicate that persons who have experienced transient ischaemic attack should be supported to develop self-management skills. However, it is not clear how best to support the…

The Perceived Medical Condition Self-Management Scale can be applied to patients with chronic kidney disease.

Chronic Kidney Disease (CKD) is a major burden on patients and the health care system. Treatment of CKD requires dedicated involvement from both caretakers and patients. Self-efficacy, also known as perceived competence, contributes to successful maintenance of patient's CKD self-management behaviors such as medication adherence and dietary regulations. Despite a clear association between self-efficacy and improved CKD outcomes, there remains a lack of validated self-report measures of CKD self-efficacy. To address this gap, the Perceived Kidney/Dialysis Self-Management Scale (PKDSMS) was adap…

The Kennedy Krieger Independence Scales-Spina Bifida Version: a measure of executive components of self-management.

PURPOSE/OBJECTIVE: Successful implementation of functional self-care skills depends upon adequate executive functioning; however, many scales assessing adaptive skills do not address the inherent executive burden of these tasks. This omission is especially relevant for individuals with spina bifida, for whom medical self-care tasks impose a significant burden requiring initiation and prospective memory. The Kennedy Krieger Independence Scales-Spina Bifida Version (KKIS-SB) is a caregiver-reported measure designed to address this gap; it assesses skills for managing both typical and spina bifid

Self-care strategies for the management of bowel symptoms following sphincter-saving surgery for rectal cancer.

The primary aim of this article is to identify the self-care strategies that patients use to manage bowel symptoms experienced following sphincter-saving surgery for rectal cancer. Comparisons will be made with self-care strategies used by patients to manage chronic fecal incontinence and the bowel symptoms associated with other chronic bowel diseases, such as irritable bowel syndrome and inflammatory bowel disease. Published studies and conceptual literature from 2000-2010 were the data sources. Three major themes emerged from the literature reflecting the self-care strategies used by patient

Self Management Activation Randomised Trial for Prostatitis (SMART-P): study protocol for a randomised controlled trial.

BACKGROUND: Chronic prostatitis otherwise known as chronic pelvic pain syndrome is a common urological diagnosis that causes many men significant morbidity and has a detrimental effect on their quality of life. Standard treatment with antibiotics and simple analgesia are often ineffective and many patients are managed by the chronic pain services.Cognitive behavioural therapy has been shown to be helpful in the management of many chronic diseases and has recently been proposed as an effective treatment for chronic prostatitis. Furthermore, a self management programme administered to groups of

Self-management strategies used by 'high functioning' individuals with bipolar disorder: from research to clinical practice.

INTRODUCTION: Bipolar disorder (BD) is a complex mental illness that results in substantial costs, both at a personal and societal level. Research into BD has been driven by a strongly medical model conception, with a focus upon pathology and dysfunction. Little research to date has focused upon strategies used to maintain or regain wellness in BD. Here, we present results from a qualitative study of self-management strategies used by a Canadian sample of 'high-functioning' individuals with BD. The aims of the present paper are two-fold: (1) To provide a description of the self-management stra…

House dust mite allergen avoidance and self-management in allergic patients with asthma: randomised controlled trial.

BACKGROUND: The efficacy of bed covers that are impermeable to house dust mites has been disputed. AIM: The aim of the present study was to investigate whether the combination of 'house dust mite impermeable' covers and a self-management plan, based on peak flow values and symptoms, leads to reduced use of inhaled corticosteroids (ICS) than self-management alone. DESIGN OF STUDY: Prospective, randomised, double blind, placebo-controlled trial. SETTING: Primary care in a south-eastern region of the Netherlands. METHOD: Asthma patients aged between 16 and 60 years with a house dust mite alle…

Effective management of intrathoracic anastomotic leak with covered self-expandable metal stents. Report on three cases.

Intrathoracic anastomotic leakage is one of the major complications of esophageal surgery that can lead to high mortality rate. Depending on the clinical presentation, therapeutic options include surgical reintervention and conservative management with or without external drainage or stenting. In this paper, we report a series of three patients, two men and one woman, mean age of 55.6 years (range 34-68 years), having intrathoracic anastomotic leak after esophagogastrectomy treated by insertion of four covered self-expandable metal stents performed exclusively under fluoroscopic view. OBJECTIVE: Although patient activation and eHealth literacy are known to influence self-management in chronic diseases, evidence on whether eHealth literacy mediates this relationship in patients with coronary heart disease (CHD) remains limited. This study explored the associations between patient activation, eHealth literacy, self-efficacy and self-management in patients with CHD and aimed to examine both the direct and indirect associations of patient activation on self-efficacy and self-management through eHealth literacy. A visual reminder chart to improve tuberculosis treatment adherence and self-management in a low-resource setting.

BACKGROUND: Treatment adherence in tuberculosis (TB) remains a significant public health challenge, particularly in low-resource settings where limited health literacy contributes to poor outcomes. METHODS: A single-arm pre-post intervention was conducted with 39 adult pulmonary TB patients attending a regional clinic. Participants used the TB-Minder Chart daily for 2 months to record medication intake and reflect on treatment experiences. Adherenc…
