BACKGROUND: Curative resection of sigmoid colon and rectal cancer includes "high tie" of the inferior mesenteric artery (IMA). However, IMA ligation compromises blood flow to the anastomosis, which may increase the leakage rate, and it is unclear whether this confers a survival advantage. Accordingly, the IMA may be ligated at a point just below the origin of the left colic artery (LCA) "low tie" combined with lymph node dissection (LND) around the origin of the IMA (low tie with LND). However, no study has investigated the detailed prognostic results between "high tie" and "low tie with LND."

A 30s-year-old male complaining of abdominal distension and pain was referred to our hospital. The support by palliative care team was initiated soon after emergency admission. OBJECTIVE: The majority of data on colonic endoscopic ultrasound (EUS) are limited to malignant lesions in the rectum and diseases of the anal sphincter. The forward-oblique-viewing upper echoendoscope has been mostly applied for staging rectal cancer. A front-viewing echocolonoscope is available but has not been widely used because of limited indications and the expense of buying another instrument. We reported the case of a 55-year-old man with metastatic tumor of the pituitary gland who suffered from symptomatic pituitary apoplexy with subarachnoid hemorrhage. The patient, who had sigmoid colon carcinoma and left parietal metastatic brain tumor, developed severe headache and decrease of right visual acuity. CT showed a pituitary mass with subarachnoid hemorrhage. Transsphenoidal surgery was performed after replacement therapy with corticosteroids. Histological examination revealed metastasis of adenocarcinoma. Pituitary apoplexy is an unusual manifestation of metastatic pituitary tumor.

A case of chylous ascites due to retroperitoneal tumoral compression is reported. Following failure of a medical treatment which consisted of paracentesis and medium-chain triglyceride (MCT) diet, the ascites dried up after installation of a peritoneum-jugular vein shunt valve. This operation is seldom performed in patients with chylous ascites since the medical treatment with paracenteses, MCT diet or even total parenteral nutrition succeeds in drying the effusion in almost 50 percent of the cases, notably those with postoperative lesion.

Tumor-to-tumor metastases are rare occurrences. A 75-yr-old male presented with an enlarging axillary mass. Further investigation revealed an adenocarcinoma of the colon and a colonic polyp. Metastatic malignant melanoma was present within the colonic polyp and in axillary lymph nodes. After curative surgery for rectal or sigmoid cancer, loco-regional recurrence occurs in about 30% of the patients. A 66-year-old woman was diagnosed with sigmoid carcinoma with bilobar unresectable liver metastases. Primary tumor resection was performed. Neoadjuvant chemotherapy was administered to downstage liver disease. Following FOLFOX/panitumumab (4 cycles), disease progression was observed within the liver. Malignant transformation in a mature cystic teratoma (MCT) is rare occurring in 1.8% out of 8000 cases of MCT. The most common histological types are squamous cell carcinoma (SCC) followed by adenocarcinoma and melanoma. Clinically, these tumors are usually asymptomatic but may be discovered accidentally during gynecologic examination due to mass effect. The association between schistosomiasis and colorectal malignancy has long been suggested in the literature, but it is not uniformly accepted. In the Far East, considerable evidence supports an etiological link between Schistosoma japonicum and colorectal cancer. However, the available data regarding the role of Schistosoma mansoni in colorectal carcinogenesis are conflicting and most often do not show causality.
