The emotional brain and sleep: an intimate relationship.

Research findings confirm our own experiences in life where daytime events and especially emotionally stressful events have an impact on sleep quality and well-being. Obviously, daytime emotional stress may have a differentiated effect on sleep by influencing sleep physiology and dream patterns, dream content and the emotion within a dream, although its exact role is still unclear. Other effects that have been found are the exaggerated startle response, decreased dream recall and elevated awakening thresholds from rapid eye movement (REM)-sleep, increased or decreased latency to REM-sleep, inc

Periodic leg movements during sleep and wakefulness in narcolepsy.

The objectives of the study were to measure the prevalence of periodic leg movements during NREM and REM sleep (PLMS) and while awake (PLMW) and to assess the impact of PLMS on nocturnal sleep and daytime functioning in patients with narcolepsy. One hundred and sixty-nine patients with narcolepsy and 116 normal controls matched for age and gender were included. Narcoleptics with high and low PLMS indices were compared to assess the impact of PLMS on sleep and Multiple Sleep Latency Test (MSLT) variables. More narcoleptics than controls had a PLMS index greater than 5 per hour of sleep (67% ver

Instantaneous change in sleep stage with noise of a passing truck.

The transient effects of passing truck noise on sleep polygraphs of 8 men were studied. The percentage of the sleep Stage 2 which changed from Stage 2 to Stage 1, waking or movement time, was calculated. The shallowing of REM sleep was identified when it had changed to other stages, and the shallow epoch percentage was calculated for the REM sleep. The percentage of shallow epochs for Stage 2 significantly increased against that of the control condition after exposure to noise but was not statistically significant for Stage REM. In conclusion, the shallowing or changes in the sleep Stage 2 or

Cerebral blood flow in normal and abnormal sleep and dreaming.

Measurements of regional or local cerebral blood flow (CBF) by the xenon-133 inhalation method and stable xenon computerized tomography CBF (CTCBF) method were made during relaxed wakefulness and different stages of REM and non-REM sleep in normal age-matched volunteers, narcoleptics, and sleep apneics. In the awake state, CBF values were reduced in both narcoleptics and sleep apneics in the brainstem and cerebellar regions. During sleep onset, whether REM or stage I-II, CBF values were paradoxically increased in narcoleptics but decreased severely in sleep apneics, while in normal volunteers

Pontine Waves Accompanied by Short Hippocampal Sharp Wave-Ripples During Non-rapid Eye Movement Sleep.

Ponto-geniculo-occipital or pontine (P) waves have long been recognized as an electrophysiological signature of rapid eye movement (REM) sleep. However, P-waves can be observed not just during REM sleep, but also during non-REM (NREM) sleep. Recent studies have uncovered that P-waves are functionally coupled with hippocampal sharp wave ripples (SWRs) during NREM sleep. However, it remains unclear to what extent P-waves during NREM sleep share their characteristics with P-waves during REM sleep and how the functional coupling to P-waves modulates SWRs. Here, we address these issues by performin

Fluctuations between sleep and wakefulness: wake-like features indicated by increased EEG alpha power during different sleep stages in nightmare disorder.

Although a growing body of research indicates that frequent nightmares are related to impaired sleep regulation, the pathophysiology of nightmare disorder is far from being fully understood. We examined the relative spectral power values for NREM and REM sleep separately in 19 individuals with nightmare disorder and 21 healthy controls, based on polysomnographic recordings of the second nights' laboratory sleep. Nightmare subjects compared to controls exhibited increased relative high alpha (10-14.5Hz) and fronto-central increases in high delta (3-4Hz) power during REM sleep, and a trend of in

Strong and Weak Episodic Memories Are Shaped by Multiple Cycles of NREM Spindles and REM Alpha Bursts.

Prior research has built up the notion that non-rapid eye movement sleep (NREM) and rapid eye movement sleep (REM) are separately important for specific types of memory. However, growing evidence suggests that these sleep stages work in conjunction to jointly shape memory. The sequential hypothesis proposes that the ordered sequence of NREM followed by REM is critical for memory consolidation. Additionally, NREM spindles and REM alpha bursts have been independently implicated in episodic memory, yet it is unknown how these oscillatory events may shape episodic memory together. We hypothesised

Reliability of heart rate variability during stable and disrupted polysomnographic sleep.

Heart rate variability (HRV) is commonly used within sleep and cardiovascular research, yet HRV reliability across various sleep stages remains equivocal. The present study examined the reliability of frequency- and time-domain HRV within stage-2 (N2), slow-wave (SWS), and rapid-eye-movement (REM) sleep during both stable and disrupted sleep. We hypothesized that high-frequency (HF) HRV would be reliable in all three sleep stages, low-frequency (LF) HRV would be reliable during N2 and SWS, and that disrupted sleep via spontaneous cortical arousals would decrease HRV reliability. Twenty-seven p

Sleep changes during a spontaneous manic episode: PSG assessment in a clinical context.

Sleep plays a key role in the pathogenesis and clinical presentation of mood disorders. However, only a few studies have investigated sleep architecture during the manic episodes of Bipolar Disorder (BD) and changes in sleep parameters that follow clinical variations. Twenty-one patients (8 males, 13 females) affected by BD, manic phase, underwent polysomnographic recordings (PSG) at the beginning of the admission in our ward (T0) and after three weeks of hospital treatment (T1). All participants were clinically evaluated using Young Mania Rating Scale (YMRS), Pittsburgh Sleep Quality Index (P

Sleep-related head jerks: toward a new movement disorder.

STUDY OBJECTIVES: Sleep-related head jerks (SRHJ) are often considered as a physiological motor phenomenon, occurring mainly during rapid eye movement (REM) sleep. Their clinical relevance and links with other sleep parameters are unclear. We characterized the clinical and polysomnographic features of patients with excessive SRHJ and compare them with healthy controls and patients with isolated REM sleep behavior disorder (iRBD). METHODS: A total of 30 patients (19 males, 27.5 y.o., 16.0-51.0) with a REM-HJ index >30/h were identified over a period of 5 years. All had a video-polysomnographic

Analysis of rapid eye movement periodicity in narcoleptics based on maximum entropy method.

We examined REM sleep periodicity in typical narcoleptics and patients who had shown signs of a narcoleptic tetrad without HLA-DRB1*1501/DQB1*0602 or DR2 antigens, using spectral analysis based on the maximum entropy method. The REM sleep period of typical narcoleptics showed two peaks, one at 70-90 min and one at 110-130 min at night, and a single peak at around 70-90 min during the daytime. The nocturnal REM sleep period of typical narcoleptics may be composed of several different periods, one of which corresponds to that of their daytime REM sleep.

Rapid Eye Movement Sleep Behavior Disorder and Other Rapid Eye Movement Sleep Parasomnias.

PURPOSE OF REVIEW: The most common rapid eye movement (REM) parasomnia encountered by neurologists is REM sleep behavior disorder (RBD), and nightmares are so frequent that every neurologist should be able to differentiate them from the dream enactment of RBD. Isolated sleep paralysis is relatively common and is often mistaken for other neurologic disorders. This article summarizes the current state of the art in the diagnosis of RBD, discusses the role of specific questionnaires and polysomnography in the diagnosis of RBD, and reviews recent studies on idiopathic RBD as an early feature of a

Sleep in Adults: Parasomnias.

Parasomnias are sleep disorders involving undesirable physical events or experiences occurring during sleep onset, while sleeping, or on arousal. They include non-rapid eye movement (REM) parasomnias (eg, confusional arousals, sleep terrors, sleepwalking) and REM-related parasomnias (eg, nightmare disorder, REM sleep behavior disorder, sleep paralysis). Notable among these is REM sleep behavior disorder, which is closely linked to neurodegenerative conditions such as Parkinson disease. Early detection of this disorder is essential because it often precedes the motor symptoms of neurodegenerati

Multiple Sleep Latency Test findings in Kleine-Levin syndrome.

Multiple Sleep Latency Test (MSLT) findings in a case of Kleine-Levin syndrome are reported for the first time. MSLT data indicate sleepiness as severe as in narcolepsy or obstructive sleep apnea and the occurrence of four sleep onset rapid eye movement (REM) periods, with a greater REM propensity at 2:00 p.m. and 4:00 p.m. than at 10:00 a.m. and 12:00 noon. The replication of such findings might suggest that Kleine-Levin syndrome could be considered a form of periodic REM sleep disinhibition. Therefore, the traditional hypothesis of diencephalic dysfunction may require modification to include

Seizure susceptibility decreases with enhancement of rapid eye movement sleep.

The study examined the effect of enhanced rapid eye movement (REM) sleep duration on the seizure threshold determined by electrical stimulation of the amygdala in rats. The duration of REM sleep was specifically increased by the microinjection of a cholinergic agonist, carbachol, into the pontine reticular formation. This was accompanied by a significant increase in the threshold current required to elicit an afterdischarge in the amygdala. The results suggest that an increase in REM sleep decreases the likelihood of cortical seizure activity, an effect that is manifest even in other stages of

Examining Sleep-Disordered Breathing Events Using Latent Profile Analysis.

The clinical utility of the ratio of the apnea-hypopnea index (AHI) occurring during rapid eye movement (REM) and non-REM (NREM) sleep (AHIREM/AHINREM ratio) has been debated. We investigated the heterogeneity of REM and NREM sleep behaviors to identify unobserved distinct subtypes of sleep-disordered breathing (SDB) and examine their demographic and clinical features. The present study used a sample of 3626 adult patients who underwent diagnostic polysomnography evaluations at the Sleep Disorders Center of King Abdulaziz Medical City in Riyadh, Saudi Arabia. Latent profile analysis was perfor

An EEG sleep study of a bipolar (manic-depressive) patient with a nocturnal switch process.

Fifty-eight all-night polygraphic sleep recordings were obtained on an unmedicated female bioplar (manic-depressive) patient who switched into and out of mania eight times. While depressed she was hypersomniac and exhibited elevated Rapid Eye Movement (REM) sleep time and short REM latency. On four nights, she apparently switched into mania while asleep. The last recorded sleep stage in each case was REM sleep.

Heart rate variability during sleep in patients with vasovagal syncope.

BACKGROUND: There are a few studies showing no significant heart rate variability (HRV) over a 24-hour period in vasovagal syncope (VVS) patients, but no research has examined HRV and its sympathetic and parasympathetic components during rapid eye movement (REM) and non-REM sleep. The authors hypothesized that REM sleep might be a critical state in which VVS patients would show abnormal responses. OBJECTIVES: To analyze the sympathetic and parasympathetic components of HRV during REM and SWS in patients with VVS compared to normal subjects, and in patients with positive HUTT compared to negat
