Comparative effects of modafinil and amphetamine on daytime sleepiness and cataplexy of narcoleptic dogs.

The effects of modafinil and amphetamine on daytime sleep (polysomnographic recordings) and cataplexy (the food-elicited cataplexy test) were compared using the narcoleptic canine model. Results indicate that both modafinil (5 and 10 mg/kg body weight i.v.) and amphetamine (100 and 200 micrograms/kg i.v.) increase wakefulness and reduce slow-wave sleep in control and narcoleptic dogs. In contrast, the results of cataplexy testing demonstrate that amphetamine (2.5-160 micrograms/kg i.v.), but not modafinil (0.125-8.0 mg/kg i.v.) significantly suppresses canine cataplexy. These results suggest t

Ambulatory 24-hour manometric and pH metric evidence of permanent impairment of clearance capacity in patients with esophageal atresia.

Dysphagia and gastroesophageal reflux (GER) probably caused by structural disorganization of the esophagus occur frequently after repair of tracheoesophageal fistula (TEF), and the extent to which they may improve beyond childhood is not known. The aim of the present study is to assess by combined ambulatory 24-hour manometry and pH-metry the esophageal peristaltic activity and acid clearing capacity in adolescents and adults who had been operated on for TEF at birth. Twenty-two patients, aged 17.1 +/- 4.5 years (mean +/- SD), were examined with combined three-channel manometry and two-channel

A 2-week, polysomnographic, safety study of sodium oxybate in obstructive sleep apnea syndrome.

PURPOSE: Sodium oxybate (SXB) is approved for cataplexy and excessive daytime sleepiness in narcolepsy. Obstructive sleep apnea syndrome (OSAS) affects ∼9-50% of narcoleptics. Effects of 2-week SXB administration on apnea-hypopnea index (AHI), oxygen saturation (SaO(2)), and sleep architecture were investigated in OSAS patients. METHODS: OSAS patients (n = 48) received 2-week SXB or placebo (PBO) treatment with polysomnography at baseline and day 14. The primary outcome measure was change from baseline in mean AHI. Secondary outcomes included changes from baseline in SaO(2), and sleep archite

State-dependent changes in high-frequency oscillations recorded in the rat nucleus accumbens.

Among the local field potentials recorded in the rat nucleus accumbens (NAc) spontaneous high frequency oscillations (HFO) are typically represented by a small peak in the power spectra in the range of 140-180 Hz. These HFO are known to occur in the awake state, but their distribution over the sleep-wake cycle has not been investigated. To address this issue we firstly examined the power of HFO during periods of quiet waking, slow-wave sleep (SWS) and rapid eye movement (REM) sleep. Since general anesthesia resembles certain features of naturally occurring SWS we went on to examine the effect

Sleep and timekeeping changes, and dysregulation of the biological clock in experimental trypanosomiasis.

The rest-activity and body temperature 24 hours cycles, as well as the pattern of spontaneous sleep, were investigated in rats after infection with Trypanosoma brucei brucei. In the infected rats, which were entrained to a 12 hours/12 hours photoperiod, a considerable hypokinesia was detected during the hours of darkness. In most of the infected animals, the body temperature cycle displayed a lower amplitude and an advance of about 3 hours in respect to control rats; in addition, the body temperature rhythm was not significant in some infected rats. The relative proportion of slow wave synchro

Reversibility of para-chlorophenylalanine-induced insomnia by intrahypothalamic microinjection of L-5-hydroxytryptophan.

Para-chlorophenylalanine, a blocker of serotonin biosynthesis by inhibiting tryptophan hydroxylase, induced total insomnia which was accompanied in cat by a permanent discharge of ponto-geniculo-occipital activity. L-5-Hydroxytryptophan microinjection (1-4 micrograms/0.5 microliters) in the anterior hypothalamus 72 h after para-chlorophenylalanine administration, restored both slow wave sleep and paradoxical sleep with variable latencies for each state of sleep. On the contrary, ponto-geniculo-occipital activity was never suppressed. The hypnogenic effects of L-5-hydroxytryptophan were always

Proteostasis failure exacerbates neuronal circuit dysfunction and sleep impairments in Alzheimer's disease.

Failed proteostasis is a well-documented feature of Alzheimer's disease, particularly, reduced protein degradation and clearance. However, the contribution of failed proteostasis to neuronal circuit dysfunction is an emerging concept in neurodegenerative research and will prove critical in understanding cognitive decline. Our objective is to convey Alzheimer's disease progression with the growing evidence for a bidirectional relationship of sleep disruption and proteostasis failure. Proteostasis dysfunction and tauopathy in Alzheimer's disease disrupts neurons that regulate the sleep-wake cycl

Anandamide enhances extracellular levels of adenosine and induces sleep: an in vivo microdialysis study.

STUDY OBJECTIVES: The principal component of marijuana, delta-9-tetrahydrocannabinol increases sleep in humans. Endogenous cannabinoids, such as N-arachidonoylethanolamine (anandamide), also increase sleep. However, the mechanism by which these molecules promote sleep is not known but might involve a sleep-inducing molecule such as adenosine. Microdialysis samples were collected from the basal forebrain in order to detect levels of adenosine before and after injection of anandamide. DESIGN: Rats were implanted for sleep studies, and a cannula was placed in the basal forebrain to collect micro

Effect of acute exposure to loud occupational noise during daytime on the nocturnal sleep architecture, heart rate, and cortisol secretion in healthy volunteers.

OBJECTIVES: Noise is one of the commonest physical stressors to which industrial workers are exposed. Many workers complain of symptoms associated with a non-specific generalized stress response, including disturbed sleep. However, industrial workers may be exposed to more than one source of stress and it is not possible to completely attribute the disturbed nocturnal sleep and changes in heart rate to the effects of loud noise alone. This study was done to find out whether acute exposure of healthy individuals to loud occupational noise during the daytime would cause changes in their nocturna

[Sleep disturbances in the elderly: pathology, symptoms and treatment].

Although sleep disturbance is common among the elderly, such elderly patients have been considered difficult to treat because the underlying mechanisms are complicated. However, these patients often exhibit adverse effects such as daytime somnolence, poor motor coordination, and an increased risk of falls. This article reviews the pathology, symptoms, and management of sleep disturbances in elderly patients. As a consequence of aging, elderly people exhibit alterations in the sleep architecture and sleep-wake rhythm. Many studies employing polysomnography have demonstrated a shortened total sl

[Hypersomnia and thalamic and brain stem stroke: a study of seven patients].

BACKGROUND AND AIM: Thalamic and brainstem strokes are a cause of organic hypersomnia. In thalamic lesions it has been attributed to disruption of ascending activating impulses from the brainstem reticular formation and to insufficient spindling and slow-wave production, which depends upon activities of reticular thalamic nucleus and thalamocortical neurons, respectively. Reported sleep disorders in brainstem lesions have occasionally been contradictory and that is because of the presence of nearby structures in the brainstem with different functions in sleep-waking cycle. The aim of the study

Modulation of neurofluid fluctuation frequency by baseline carbon dioxide in awake humans: the role of the autonomic nervous system.

INTRODUCTION: Cerebrospinal fluid (CSF) pulsations are linked to hemodynamics, with autonomic mechanisms, suggested to modulate slow-wave induced pulsations. METHOD: To explore autonomic regulation's role in neurofluid flow, independent of sleep and neural activity, we hypothesized that modulating basal CO2 (altering vascular tone, cardiac activity and respiration) would highlight this link. RESULTS: Using resting-state BOLD fMRI in neurofluid regions under different CO2 levels (capnic states), we found: 1) biomechanical modulation does not explain neurofluid dynamic variations across capnia

Obstructive sleep apnea syndrome: a child is not just a small adult.

OBJECTIVES: Pediatric obstructive sleep apnea syndrome (OSAS), like adult OSAS, is characterized by intermittent upper airway collapse during sleep and is associated with anatomic and neuromuscular factors. However, the clinical manifestations, diagnostic criteria, and polysomnographic findings of OSAS in children are likely to be different from those in adults. The purpose of this study was to identify the characteristics that distinguish the clinical manifestations and polysomnographic findings of OSAS in children from those in adults. METHODS: The study population consisted of 34 children

Clinical features and genetic characteristics of two Chinese pedigrees with fatal family insomnia.

Background: Fatal familial insomnia (FFI) is a rare autosomal-dominant inherited prion disease characterized clinically by severe sleep disorder, motor signs, dysautonomia and abnormal behaviour. FFI is caused by a missense mutation at codon 178 of the prion protein gene (PRNP). Our study is aimed to explore typical clinical and genetic features of two Chinese pedigrees with FFI and review the related literatures. Methods: Two FFI cases with family histories were recruited in our study. The main clinical features, genetic features and possible pathophysiologic mechanisms of these two FFI cases

Periodic limb movements do not predict CPAP adherence: The role of sleep fragmentation and disease severity.

BACKGROUND: Periodic limb movements during sleep (PLMS) are common in patients with obstructive sleep apnea (OSA), yet their impact on continuous positive airway pressure (CPAP) adherence remains unclear. This study evaluated whether elevated periodic limb movement index (PLMI) influences CPAP compliance and sleep architecture in OSA patients. METHODS: We retrospectively analyzed polysomnographic and CPAP adherence data from 150 adults diagnosed with OSA. Participants were stratified into two groups: PLMI ≥15 (elevated) and PLMI <15 (normal). CPAP compliance was assessed by percentage of days

Effect of prolonged sedation with dexmedetomidine, midazolam, propofol, and sevoflurane on sleep homeostasis in rats.

BACKGROUND: Sleep disruption is a common occurrence during medical care and is detrimental to patient recovery. Long-term sedation in the critical care setting is a modifiable factor that affects sleep, but the impact of different sedative-hypnotics on sleep homeostasis is not clear. METHODS: We conducted a systematic comparison of the effects of prolonged sedation (8 h) with i.v. and inhalational agents on sleep homeostasis. Adult Sprague-Dawley rats (n=10) received dexmedetomidine or midazolam on separate days. Another group (n=9) received propofol or sevoflurane on separate days. A third g

Evaluation of upper airway obstruction in snoring patients using digital video stroboscopy.

Stroboscopy is routinely used in voice disorder clinics but its use in studying the mechanisms of upper airway obstruction in patients who snore has not yet been described. This study combines the use of stroboscopy during sleep nasendoscopy to evaluate the oscillations and vibrations observed during snoring in slow motion. In addition, we utilised the multi-dimensional voice programme simultaneously to study some of the acoustic parameters of snoring whilst visualising the dynamics of the upper airway. Forty-five patients with primary snoring or mild obstructive sleep apnoea were recruited at
