Decreased pituitary-gonadal secretion in men with obstructive sleep apnea.

Decreased libido is frequently reported in male patients with obstructive sleep apnea (OSA). The decline in morning serum testosterone levels previously reported in these patients was within the normal adult male range and does not explain the frequent association of OSA and sexual dysfunction. We determined serum LH and testosterone levels every 20 min between 2200-0700 h with simultaneous sleep recordings in 10 men with sleep apnea and in 5 normal men free of any breathing disorder in sleep. The mean levels and area under the curve of LH and testosterone were significantly lower in OSA patie

Morbidity of obstructive sleep apnea in children: facts and theory.

Sleep-disordered breathing (SDB) is a frequent, albeit underdiagnosed, problem in children. If left untreated, SDB may lead to substantial morbidities affecting multiple target organs and systems. This review provides a detailed and current description of the current status of our understanding of SDB-associated morbidity in children, and provides recommendations of future research directions necessary for increasing our knowledge and awareness on the short- and long-term consequences of SDB during childhood.

Facial patterns of obstructive sleep apnea patients using Ricketts' method.

To analyze the facial patterns of obstructive sleep apnea syndrome (OSAS) patients, we took lateral cephalograms of 31 OSAS patients and 26 non-OSAS controls and utilized Ricketts' method. In addition, we measured the hyoid bone position, the length of mandibular plane to H-point, the length of soft palate, and lower pharynx. The facial patterns of OSAS patients were dolico, the hyoid bone was positioned low, the soft palate was longer and the width of the airway was narrower than that of the non-OSAS controls.

Site of airway collapse in obstructive sleep apnea after uvulopalatopharyngoplasty.

The objective of this prospective study was to determine the site and pattern of upper airway collapse by a multiple-catheter technique in subjects demonstrated to have obstructive sleep apnea (OSA) after uvulopalatopharyngoplasty (UPPP). Standard diagnostic nocturnal polysomnography (PSG) was done on all subjects. The PSG recordings included electroencephalogram, electrooculogram, electrocardiogram, chin and leg electromyograms, nasal and oral airflow, and abdominal effort. Polysomnography with a multiport flexible airway Gaeltec catheter was performed in 22 subjects. The Gaeltec flexible air

Evaluation of uvulopalatopharyngoplasty in treatment of obstructive sleep apnea syndrome.

Many methods have been reported to select the proper candidates for uvulopalatopharyngoplasty (UPPP) among obstructive sleep apnea patients. Polysomnography with intraluminal pressure recordings in the upper airway was found useful to predict obstructive site(s) and hence the success of surgery. According to the pressure analysis, 11 cases with high negative pressure at the mesopharynx were selected for UPPP, while UPPP combined with midline partial glossectomy (UPPP-MLG) was done in 18 cases proved to have high negative pressure difference between the mesopharynx and esophagus. A reduction of

[Effectiveness of uvulopalatopharyngoplasty in snoring disorders and obstructive sleep apnea].

We studied the effectiveness of uvulopalatopharyngoplasty under general anesthesia and classical surgery in 26 patients treated surgically for chronic snoring (CS) and obstructive sleep apnea syndrome (OSAS). Our technique is a variation of the Fujita and Quesada technique that involves partial resection of the palate and uvula with endonasal microsurgery. The criteria for inclusion were age 35 to 47 years and desire to do without C-PAP or Bi-PAP. This device, although effective, produced poor physical and psychological tolerance. After a postoperative follow-up of 3 years, good results were f

Treatment of obstructive sleep apnea syndrome by rapid maxillary expansion.

The precise role of maxillary constriction in the pathophysiology of obstructive sleep apnea (OSA) is unclear. However, it is known that subjects with maxillary constriction have increased nasal resistance and resultant mouth-breathing, features typically seen in OSA patients. Maxillary constriction is also associated with alterations in tongue posture which could result in retroglossal airway narrowing, another feature of OSA. Rapid maxillary expansion (RME) is an orthodontic treatment for maxillary constriction which increases the width of the maxilla and reduces nasal resistance. The aim of

Obstructive sleep apnea syndrome in a family with Crouzon's syndrome.

Craniofacial anomalies are recognized causes of obstructive sleep apnea syndrome (OSAS) in children. Current literature is limited due to rarity of cases. Furthermore, the mechanism of upper airway obstruction is not clearly understood. We would like to report a family (father and 2 sons) who are suffering from Crouzon's syndrome. The two brothers (ages 1 and 3) were found to have significant obstructive sleep apnea syndrome (OSAS) with failure to thrive. Nasal continuous positive airway pressure (CPAP) markedly improved their OSAS and resulted in accelerated weight gain. The nasoendoscopy and

[Nasal strips without effect on obstructive sleep apnea and snoring].

Recently a nose plaster (NP, Breathe-Right) has been increasingly used in the treatment of obstructive sleep apnoea (OSA) and snoring. Hence, we examined the use of the NP as a treatment of both OSA and snoring without OSA. The NP has a elastic spine that increases the diameter of the nostril and is thought to reduce the degree of OSA and snoring. According to the polysomnographic data two groups were differentiated: Group A (30 patients with OSA, apnoea index > 10/h, 26 men) and Group B (20 snorers, without OSA, 13 men). After the diagnostic polysomnography the efficacy of the NP was measured

A noninvasive technique for detecting obstructive and central sleep apnea.

A new noninvasive method to detect obstructive and central sleep apnea [(OSA) and (CSA)] events is described. Data were collected from ten volunteer subjects with a previous diagnosis of OSA while they were titrated for continuous positive airway pressure (CPAP) therapy. Apneic events were identify by analyzing of estimated airway impedance determined from pressure and airflow signals delivered from CPAP. To enhance performance of this technique, a single-frequency (5 Hz with 0.5 cmH2O peak-to-peak amplitude) probing signal was superimposed on the applied CPAP pressure. The results indicated t

[Severity of obstructive sleep apnea syndrome--correlation with cephalometric parameters].

In 132 patients with polysomnographic records suffering from OSAS a cephalometric analysis according to Hasund supplemented by special OSAS-parameters was performed. The statistical analysis showed a correlation between severity of OSAS and width of posterior airway space, length of the soft palate, hyoid position and posterior growth development of the mandible. A principal component analysis showed four groups: 1) prognathic patients, 2) retrognathic patients with narrow PAS, 3) patients with long soft palate and low standing hyoid, and 4) orthognatic obese subjects. Lateral cephalometry is

Why should we enlarge the pharynx in obstructive sleep apnea?

Patency of the collapsible segment of the pharynx is dependent upon the interplay of anatomic and physiologic factors. The upper airway of the typical patient with obstructive sleep apnea (OSA) is structurally narrowed by fat deposits in the lateral pharyngeal walls. During wakefulness, the upper airway dilator muscles compensate for the structurally reduced pharyngeal size by increasing their activity, thereby pulling the tongue and soft palate forward. As a consequence, the cross-sectional area is relatively preserved, but the shape of the pharynx becomes elliptical along the anteroposterior

Upper airway collapsibility in children with obstructive sleep apnea syndrome.

In adults, the critical nasal pressure (Pcrit) at which the upper airway collapses is higher in patients with the obstructive sleep apnea syndrome (OSAS) than in those with primary snoring. Pediatric OSAS differs clinically from adult OSAS. We therefore compared Pcrit between prepubertal children with OSAS and primary snoring. Pcrit was determined by correlating the maximal inspiratory airflow with the level of positive or negative nasal pressure applied via a nasal mask. As in adults, we found that the maximal inspiratory airflow varied in proportion to the upstream (nasal) rather than the do

Ventilatory responses during wakefulness in children with obstructive sleep apnea.

The pathophysiology of the obstructive sleep apnea syndrome (OSAS) is not fully understood. In children, airway obstruction secondary to tonsilloadenoidal hypertrophy is the leading cause of OSAS. However, not all children with tonsilloadenoidal hypertrophy develop OSAS. Thus, other factors, including abnormalities in ventilatory control, may contribute to the etiology of OSAS. To test this, we performed polysomnography and hypercapnic and hypoxic ventilatory response testing in 20 children and adolescents with OSAS (mean age, 8 +/- 3 [SD] yr) and 19 control subjects. Only two children with OS

Autonomic stress tests in obstructive sleep apnea syndrome and snoring.

Apneic events in the obstructive sleep apnea syndrome (OSAS) are associated with cardiovascular responses mediated through the autonomic nervous system. We examined autonomic cardiovascular responses in 33 patients (median age = 50 years, range = 20-72 years) undergoing polysomnography for suspected OSAS. We examined these responses in the evening and at arousal in the morning. Tests consisted of heart rate responses to Valsalva maneuver, deep breathing and change from lying down to standing. In addition, systolic blood pressure (BP) response to standing and diastolic BP response to handgrip w

The association of normal-pressure hydrocephalus with obstructive sleep apnea.

This report describes a 70-year-old man with obstructive sleep apnea who deteriorated rapidly when nasal continuous positive airway pressure was begun. The patient was found to have normal-pressure hydrocephalus, which was possibly exacerbated by the nasal continuous positive airway pressure. A review of the literature indicates several significant associations between apnea, normal-pressure hydrocephalus, and increased intracranial pressure and suggests that the association of obstructive sleep apnea and hydrocephalus might not be rare. Implications for diagnosis and treatment are discussed.
