Parasomnias are undesirable behaviors or experiences during sleep that manifest clinically as abnormal behavior, emotions, and nightmares. We herein report four elderly parasomnia patients who were successfully treated for abnormal nocturnal behaviors, including rapid eye movement (REM) sleep behavior disorders, with Keishikaryukotsuboreito (KRB), a Japanese traditional herbal medicine. KRB resolved nocturnal violent behaviors and sleep walking without any adverse effects. In one patient, occipital dominant spike-wave complexes induced by 3-Hz photic stimulation were reduced after KRB treatmen

STUDY OBJECTIVES: Assess the physiologic and self-reported effects of wind turbine noise (WTN) on sleep.
METHODS: Laboratory sleep study (n = 50 participants: n = 24 living close to wind turbines and n = 26 as a reference group) using polysomnography, electrocardiography, salivary cortisol, and questionnaire endpoints. Three consecutive nights (23:00-07:00): one habituation followed by a randomized quiet Control and an intervention night with synthesized 32 dB LAEq WTN. Noise in WTN nights simulated closed and ajar windows and low and high amplitude modulation depth.
RESULTS: There was a lon

PURPOSE OF REVIEW: Sleep and the chronobiological disease cluster headache are believed to be interconnected. Despite efforts, the precise nature of the relationship remains obscured. A better understanding of this relation may lead to more effective therapeutic regimes for patients suffering from this debilitating disease. This review aims to evaluate the existing literature on the subject of cluster headache and sleep.
LATEST FINDINGS: Several previous studies describe an association between episodic cluster headache and distinct macrostructural sleep phases. This association was not confir

Sleep disturbances are common in neurodegenerative diseases. Disturbed sleep can result in fatigue, irritability, morning headaches, impaired motor and cognitive skills, depression, and daytime somnolence. The major sleep complaints include insomnia, hypersomnia, parasomnia, excessive nocturnal motor activity, circadian sleep-wake rhythm disturbance, and respiratory dysrhythmia. The pathogenetic mechanisms of sleep disturbances may be secondary to direct structural alteration of the sleep-wake generating neurons or from several other indirect mechanisms. At the biochemical level, neurodegenera

Headache and sleep have long been recognised as being interdependent due to specific causative factors. Yet, the precise understanding of the roles played by these factors in this interdependency remains elusive. Many observations have suggested a reciprocal relationship between headache and sleep; however, these hypotheses have only been partially substantiated by robust findings. Being so, additional well-designed clinical and laboratory studies are required to confirm these relationships. Nonetheless, sleep and headache are known to be related in several ways: primary headache such as migra

Restless Legs Syndrome (RLS) is a sensorimotor disorder with diverse clinical manifestations that significantly impact sleep and quality of life, particularly among women. Polysomnography (PSG) plays a key role in evaluating motor features of RLS, such as periodic leg movements during sleep (PLMS). This review examines the effects of pharmacological treatments on PSG parameters and sleep architecture in RLS. Dopamine agonists are the most studied, consistently reducing PLMS but offering limited improvements in sleep structure and carrying risks of augmentation with long-term use. In contrast,

Obstructive sleep apnoea was first brought to prominence by Henri Gastaut, a French epileptologist. Since that time the interface between epilepsy and sleep disorders has received less attention than might be justified, recognizing that sleep deprivation is a poignant provocateur for seizures. Sleep deprivation is often used as a diagnostic procedure during electroencephalography (EEG) when waking EEG has failed to demonstrate abnormality. Patients referred to an outpatient neurological clinic for evaluation of possible seizures in whom sleep disorder was suspected, either due to snoring durin

The pathophysiology of insomnia remains unclear. This sleep neuroimaging study used arterial spin labelling, self-report and polysomnography to compare nighttime relative cerebral perfusion in individuals with insomnia disorder (n = 11) and controls (n = 17). We hypothesised that insomnia would be associated with altered perfusion in regions involved in arousal, sleep drive and information processing. Participants completed two polysomnography and arterial spin labelling scans: a wake scan at habitual bedtime and a sleep attempt. Sleep state was determined using self-report and polysomnography

Breathing is regulated by the behavioural and the automatic systems, the former mainly acting during wakefulness, the latter during sleep. Central alveolar hypoventilation and breathing disorders in Rett syndrome indicate a selective impairment of the two systems.

Altered vigilance performance has been documented in patients with sleep-related breathing disorders (SRBDs). Sleep fragmentation, sleepiness, respiratory disturbances and nocturnal hypoxaemia have been suggested as the pathogenesis of these deficits, yet it remains difficult to find a good correlation between performance deficits and the above factors. In the present study, which performance measure better characterised SRBD patients and the main factors implicated in these disturbances were examined. The study group consisted of 152 patients and 45 controls, all examined using a performance

Narcolepsy-cataplexy, a disorder of excessive sleepiness and abnormalities of rapid eye movement (REM) sleep, results from deficiency of the hypothalamic orexin (hypocretin) neuropeptides. Modafinil, an atypical wakefulness-promoting agent with an unknown mechanism of action, is used to treat hypersomnolence in these patients. Fos protein immunohistochemistry has previously demonstrated that orexin neurons are activated after modafinil administration, and it has been hypothesized that the wakefulness-promoting properties of modafinil might therefore be mediated by the neuropeptide. Here we tes

The loss of orexin neurons in humans is associated with the sleep disorder narcolepsy, which is characterized by excessive daytime sleepiness and cataplexy. Mice lacking orexin peptides, orexin neurons, or orexin receptors recapitulate human narcolepsy phenotypes, further highlighting a critical role for orexin signaling in the maintenance of wakefulness. Despite the known role of orexin neurons in narcolepsy, the precise neural mechanisms downstream of these neurons remain unknown. We found that targeted restoration of orexin receptor expression in the dorsal raphe (DR) and in the locus coeru

Narcolepsy, a disorder of excessive daytime sleepiness that affects more than 125,000 people in the United States, is technically defined as a daytime mean sleep latency (time elapsed before falling asleep) of less than 5 minutes in conjunction with verification of rapid eye movement sleep in at least two of five daytime nap periods. Cataplexy, hypnagogic hallucinations, and sleep paralysis are frequently associated with narcolepsy. Currently, overnight polysomnography and multiple sleep latency testing in a sleep disorders laboratory are used to diagnose narcolepsy. Standard pharmacologic the

OBJECTIVE: Sleep disturbances are common in adults with attention-deficit/hyperactivity disorder (ADHD), yet little is known about how pharmacotherapy and psychiatric comorbidities influence sleep over time in this population. This study examines longitudinal associations between medication use and sleep outcomes in adults with ADHD.
METHODS: A prospective observational study was conducted with 149 adults diagnosed with ADHD attending a specialist clinic in Ireland. Participants were assessed at five time points over 18 months. Sleep outcomes were measured using the Pittsburgh Sleep Quality I

Wakefulness, rapid eye movement (REM) and non-REM sleep are not always mutually exclusive conditions, as commonly assumed. In some cases, the declaration of any state may be incomplete and states can fluctuate rapidly, resulting in peculiar behavioural syndromes such as narcolepsy, REM sleep behaviour disorder and status dissociatus. We briefly introduce this topic and discuss three suggestive clinical cases.

Despite evidence that sleep-disorders alter sleep-stage dynamics, only a limited amount of these parameters are included and interpreted in clinical practice, mainly due to unintuitive methodologies or lacking normative values. Leveraging the matrix of sleep-stage transition proportions, we propose (i) a general framework to quantify sleep-dynamics, (ii) several novel markers of their alterations, and (iii) demonstrate our approach using obstructive sleep apnea (OSA), one of the most prevalent sleep-disorder and a significant risk factor. Using causal inference techniques, we address confoundi

UNLABELLED: A COMMON DISORDER: Sleep is one of the most often altered functions in elderly people. Obviously, insomnia is one of the main complain, inducing benzodiazepine (BSD) abuse, but we must keep in mind that sleep apnoea syndrome (SAS) and restless legs syndrome (RLS) are also frequent in this group of age. After exclusion of the various age-related conditions that could induce sleep disorders, we must focalise on primary and secondary sleep disorders. As an introduction, methods of sleep analysis are described and qualitative and quantitative sleep variables are given. PHYSIOLOGICAL SL

Selective and progressive death of neurons is a characteristic feature of the process of neurodegeneration and leads to corresponding neuronal dysfunctions. Neurodegenerative diseases represent a heterogeneous group of clinically distinct disorders with similar molecular mechanisms of pathogenesis. They are based on the processes of abnormal aggregation of proteins, the formation of fibrillary insoluble structures and their deposition in the form of histopathological inclusions in the tissues of the nervous system. Disturbance of homeostatic functions that regulate neuronal ion and energy meta
