[Anxiety disorders in the elderly: Clinical and therapeutic aspects].

Few studies have been specifically devoted to anxiety disorders in the elderly. Nevertheless, various factors can expose this population to relevant levels of anxiety: development of somatic disorders, bereavements, isolation, changes of life, use of anxiogenic drugs, etc. Prevalence rates weaker than in the general population are usually found, but with high rates, however, for generalized anxiety disorder and agoraphobia. The clinical aspect of these anxious states is often dominated by somatic symptoms and complaints, and some topics seem of special concern: fear of death and disease, displ

Characteristics of compensable disability patients who choose to litigate.

This study tested the hypothesis that personal characteristics, when profiled by performance on MMPI-2 scales related to attention-seeking behavior through somatization, would differ between compensable personal injury claimants who choose to go to litigation and those who choose not to litigate. The authors examined the MMPI-2 profiles and other file data, including type and severity of injury, on 96 patients who litigated and 46 who chose not to litigate. The profiles of the two groups differed significantly overall. The difference was accounted for by the litigating patients' significantly

Psychiatric patients presenting with primarily somatic symptoms: report of two cases.

Two recent cases of psychiatric disorders, presenting primarily with physical symptoms are described, which suggest that both psychotic and psychoneurotic disorders can present with somatic symptoms for which no adequate physical cause could be defined. It is emphasised that such cases require specialist psychiatric evaluation and assessment for the appropriate management to be initiated. Recent studies have stressed the frequency with which psychiatric illness or psychological distress is presented in most African patients as somatic illness, especially in depressive illness, psychotic disord

Neurasthenia and related problems.

Neurasthenia is a diagnostic entity included in ICD-9 which is used in daily clinical practice in China and the Soviet Union, as well as some other Asian countries, so far as I know. Although it was eliminated from the psychiatric nosology in DSM-III of the United States and in several European countries, this disease has not disappeared. In my view, elimination of the category only indicates change of diagnostic concept without definite direction. Whether this change will be accepted by psychiatrists all over the world is still a question. The continued use of this diagnostic term in China is

Social bonds in a healthy elderly sample: characteristics and associated variables.

Social bonds were examined in 256 healthy, independent-living elderly men and women using the Interview Schedule for Social Interaction (ISSI). Multivariate techniques were used to test for relationships between six dimensions of social bonds, as measured by the ISSI, and demographic characteristics as well as scores from the Kellner-Sheffield Symptom Questionnaire, the Jacobs Cognitive Screening Exam, the Halstead Category Test and the Wechsler Memory Scale. The study participants reported involvement in a range of satisfying, supportive, interpersonal relationships. Significant associations

Self-stigma, severity of psychopatology, dissociation, parental style and comorbid personality disorder in patient with neurotic spectrum disorders Part 1: Relationships between self-stigma and clinical, psychosocial and demograph.

OBJECTIVES: Self-stigma represents a process of accepting negative social prejudices with a consequent negative impact on many areas of the patient's life (self-concept, social and work functioning, relationships, cooperation in treatment, quality of life, willingness to strive for something). The study was aimed to examine the level of self-stigma and other significant variables potentially related to self-stigma (personality characteristics, childhood traumatisation, anxiety, depression, personality disorder, dissociation, parental styles, attachment). METHOD: The study was conducted at the

Chronic subjective dizziness is consistent with advancing research on anxiety and somatoform disorders and offers greater insights into the relationships between neuro-otologic illnesses and anxiety. Medically unexplained cutaneous sensory symptoms may represent somatoform dissociation: an empirical study.

OBJECTIVE: We examined the relation between medically unexplained cutaneous symptoms and dissociation [measured by the Dissociative Experiences Scale, or DES; Bernstein EM, Putnam FW. Development, reliability, and validity of a dissociation scale. J Nerv Ment Dis 1986;174:727-35] among participants with no history of primary dermatologic disorders. METHODS: Three hundred sixty consenting participants (44 psychiatric outpatients and 316 nonclinical participants from the community at large) rated the severity of nine medically unexplained cutaneous symptoms (sum of all ratings comprised the Cut

Phobic postural vertigo--a long-term follow-up (5 to 15 years) of 106 patients.

One hundred and six patients diagnosed between 1987 and 1998 to have somatoform phobic postural vertigo were examined in a follow-up study with a self-evaluating questionnaire. The improvement rate after a mean follow-up time of 8.5 years (5 to 15.9 years) was 75% (27% of the patients reported a complete remission). While the majority of these patients experienced improvement or remission during the first year after assessment of diagnosis and a short-term psychotherapeutic approach, some patients also had considerable improvement even after two or more years. There was a negative correlation

Dissociation: An insufficiently recognized major feature of complex posttraumatic stress disorder.

The role of dissociation in (complex) posttraumatic stress disorder (PTSD) has been insufficiently recognized for at least two reasons: the view that dissociation is a peripheral, not a central feature of PTSD, and existing confusion regarding the nature of dissociation. In this conceptual article, the authors address both issues by postulating that traumatization essentially involves some degree of division or dissociation of psychobiological systems that constitute personality. One or more dissociative parts of the personality avoid traumatic memories and perform functions in daily life, whi

[Psychiatric illnesses in general practice].

The frequency of mental illness is often mistaken for the frequency of those worries and depressed feelings, which very often accompany somatic problems. The general practitioner is aware of these minor psychic disturbances presented together with somatic complaints, but he does not express this in his label of the result of consultation. In general practice somatic problems far prevail. Among the 3% at the most mental illnesses in general practice/family medicine "polymorphous complaints of probably non organic origin" are, together with depression, the disorders most often seen.

[Psychological disorders and environmental pollution].

In the past years Environmental Medicine has established itself in the departments of community medicine. The clients of these health offices as well as those attending other Environmental Outpatient Departments and who suffer from anxiety in connection with environmental pollution show increasingly psychomatic and even psychiatric disturbances. This paper demonstrates, from the psychiatrists as well as the environmental physicians point of view, the relationship between psychological disturbances and environmental pollution in a typical case-history. The differentiation between possibly toxic

Distinguishing child diagnostic types with early memories.

The present study attempts to correct design weaknesses in earlier studies which assessed the use of early memories (EMs) in identifying psychopathology in children. A new EM scoring system, the CEMSS, was developed to provide an objective, comprehensive analysis of the reported EMs. Subjects were 64 boys, SES IV and V, 8 through 12 years old from four diagnostic groups--Delinquent, Hyperactive, Somatic Complaints, and Schizoid. Overall, the CEMSS provided an effective means to distinguish the diagnostic groups, p less than or equal to .01. The results suggest that content variables best disti

Are psychodynamic and psychoanalytic therapies effective?: A review of empirical data.

There is a need for empirical outcome research in psychodynamic and psychoanalytic therapy. However, both the approach of empirically supported therapies (EST) and the procedures of evidence-based medicine (EBM) have severe limitations making randomised controlled trials (RCTs) an absolute standard. After a critical discussion of this approach, the author reviews the empirical evidence for the efficacy of psychodynamic psychotherapy in specific psychiatric disorders. The review aims to identify for which psychiatric disorders RCTs of specific models of psychodynamic psychotherapy are available

A comparison study of body dysmorphic disorder and obsessive-compulsive disorder.

BACKGROUND: Body dysmorphic disorder, a preoccupation with an imagined or slight defect in appearance, is classified as a somatoform disorder, but has been hypothesized to be related to obsessive-compulsive disorder. A reflection of this hypothesis, body dysmorphic disorder is included in the Yale-Brown Obsessive Compulsive Scale symptom checklist, and its transfer to the anxiety disorders' section was considered for DSM-IV. However, the similarities and differences between body dysmorphic disorder and obsessive-compulsive disorder have received little investigation. METHOD: We compared patie

Psychiatric components of a health maintenance facility (HMF) on Space Station.

The operation of a permanently manned space station brings a special challenge to psychiatry due to the combined physiological, emotional, and social stresses which will affect its occupants. Psychiatric diagnostic and therapeutic techniques will be a part of the Space Station's general medical support, known as the Health Maintenance Facility (HMF). The psychiatric component of HMF will promote psychological health throughout the mission and permit treatment, if necessary, of those psychiatric disorders which could impact mission success. This paper will discuss these emotional disorders and

Interactions between the painful disorders and the autonomic nervous system.

he autonomic nervous system (ANS) controls the heart rate, blood pressure, digestion, respiration, pupillary reactivity, sweating, urination, sexual arousal, and regulates the functions of internal organs. This system provides the homeostasis of the cells, tissues, and organs throughout the body and protects against the disturbances imposed by the external and internal stressors. The ANS has three main divisions: The sympathetic nervous system (SNS), the parasympathetic nervous system (PNS), and the enteric nervous system. In general, the SNS and PNS have opposing effects. Each region belongin

Review of Fibromyalgia (FM) Syndrome Treatments.

BACKGROUND: Fibromyalgia (FM) is a disease characterized by widespread musculoskeletal chronic pain that impairs the patient's quality of life and is considered a somatization disorder. The symptoms of the disease also affect the patient mentally, mainly since invisible pain is the only thing that indicates its existence. A typical symptom that characterizes FM patients is the lack of acceptance of the disease since its pathophysiology is not elucidated, hence the deficiencies in its management, or rather, cognitively, the belief that there is no disease to manage. The current paper aims to sh

Abnormal Functional Asymmetry in the Salience and Auditory Networks in First-episode, Drug-naive Somatization Disorder.

Evidence of abnormal functional connectivity (FC) has been implicated in patients with somatization disorder (SD). Although the importance of damage to the functional asymmetry has been established, it remains unclear as to whether abnormal intra- and inter-hemispheric FCs are related to patients with SD. We applied resting-state functional magnetic resonance imaging to first-episode, medication-naive patients with SD (n = 25) and matched healthy controls (HCs) (n = 28). The data were analyzed using parameter of asymmetry (PAS) and support vector machine (SVM). Patients with SD showed signific

[Advice and treatment from the clairvoyant Croiset].

Around 1960, the Dutch clairvoyant Gerard Croiset (1909-1980) was consulted by 'people with symptoms - considered to be unexplained - such as paralysis or neurological disorders'. I searched the archive of the Johan Borgman Fund Foundation for the effect of Croiset's advice and treatment in patients with these symptoms who might have had the diagnosis of conversion disorder. Contrary to my expectations, Croiset treated no patients with conversion disorder. His advice and treatment were successful in patients with poliomyelitis, epilepsy, lumbar disc prolapse and infantile encephalopathy. Four
