[Allergic fungal sinusitis: is this rare disease an allergy or infection?].

Allergic Fungal Sinusitis (AFS) is a newly recognized form of benign, non invasive sinusitis the histopathologic features of which are similar to those of allergic bronchopulmonary aspergillosis. AFS is a rare condition. However, because treatment and prognosis vary widely, it is important that this disorder be recognized and differentiated from chronic bacterial sinusitis and other forms of fungal sinusitis. AFS does not discriminate by age although it is primarily found in young adults. AFS patients are usually atopic, often having a history of asthma and nasal polyposis. Many have suffered

[Catheter-based balloon dilatation of the frontal, maxillary, and sphenoid ostia: a new procedure in sinus surgery].

BACKGROUND: Endoscopic sinus surgery is the procedure of choice for treating chronic rhinosinusitis. This paper describes a new minimally invasive method to restore ostium patency without removing tissue: the balloon sinuplasty technique. PATIENTS AND METHODS: Two patients were treated with the new procedure. One patient had her maxillary and sphenoid ostia dilated; the other patient had both frontal ostia, both maxillary ostia, and the right sphenoid ostia dilated. RESULTS: The balloon dilatations of the ostia were achieved successfully without impairing the mucosal circumference. There was

[Significance of medical imaging findings in sphenoid sinus disease to pituitarism].

To explore the relationship between medical imaging finding in sphenoid sinus disease and pituitarism, medical images in sphenoid sinus of 7 patients with pituitarism were analysed. Three cases had mass, and the other 4 cases had inflammatory disease in sphenoid sinus. However, their pituitary images were all normal. Sphenoid sinus disease is significant to pituitarism, but may be overlooked. Radiologists must pay more attention to imaging finding in sphenoid sinus.

Clinical significance of thickened sphenoid sinus mucosa in Rathke's cleft cyst.

BACKGROUND: To determine the clinical significance of thickening of the sphenoid sinus mucosa (TSSM) in Rathke's cleft cyst (RCC). METHODS: We retrospectively reviewed patients with pituitary lesions. A total of 99 patients, who underwent surgery of pituitary lesions between 2008 and 2015, were analyzed to evaluate the clinical significance of TSSM using magnetic resonance imaging. The patients with TSSM were evaluated for their characteristics. RESULTS: Of the 99 operations, 15 patients with paranasal sinusitis, direct invasion to the sellar floor, and/or a history of transsphenoidal surger

Recurrence of olfactory groove meningiomas.

OBJECTIVE: Despite apparent gross total resection, olfactory groove meningiomas have a high rate of late recurrence (average, 23%). In this retrospective study, we confirmed that the sites of these recurrences are the cranial base and paranasal sinuses. We postulated that these recurrences stem from conservative handling of the underlying invaded bone. Therefore, we analyzed patient outcomes according to the radicality of surgical resection. METHODS: Fifteen consecutive patients with a diagnosis of olfactory groove meningioma were treated surgically between 1992 and 2001 (nine new cases, six

IgG4-related disease in the sinonasal cavity accompanied by intranasal structure loss.

IgG4-related disease was recently proposed under the classification of systemic chronic inflammatory disease. In the field of otolaryngology, organ-specific diagnostic criteria have been established for the occurrence of this condition in the salivary glands, but not in the sinonasal cavity. Here we report a case involving a 70-year-old man with IgG4-related disease in the sinonasal cavity. The patient, with the chief complaint of nasal bleeding, first visited a physician. However, the patient experienced recurrent bleeding with intranasal structure loss and was subsequently referred to our ho

Role of craniotomy in the management of pituitary adenomas and sellar/parasellar tumors.

The transphenoidal procedure has become the preferred approach in the surgical management of sellar/parasellar tumors. Nevertheless, specifi … the transphenoidal route, ectatic midline carotid arteries, co-existent intracranial aneurysms and sphenoid sinusitis. We review the data on visual and endocrinological outcomes following the transcranial or transphenoidal approach and provide an argument that, while there appears to be a trend towards greater visual improvement after transcranial surgery for large …

Draf IIB with superior septectomy: finding the "middle ground".

BACKGROUND: Previous studies have demonstrated a high failure rate of endoscopic sinus surgery (ESS) with Draf IIa in patients with diffuse polyposis, asthma, and aspirin-exacerbated respiratory disease. A high percentage of these patients progress to endoscopic modified Lothrop procedure (EMLP). We describe a modification of the Draf IIb with a superior septectomy (IIb+SS), which may provide similar therapeutic benefit as demonstrated by the distribution of sinus irrigations in the sinus cavity with ESS with IIb+SS vs ESS with EMLP. METHODS: ESS with IIb+SS was performed on 6 cadaver heads.

An Analysis of Adverse Event Reporting in Balloon Sinus Procedures.

OBJECTIVE: (1) Become familiar with reported adverse events related to balloon dilation of the paranasal sinus ostia. (2) Understand the sequelae occurring with these events. STUDY DESIGN: Retrospective analysis of a prospective database. SETTING: OpenFDA database. SUBJECTS AND METHODS: The OpenFDA program website of the Food and Drug Administration was queried with the Application Program Interface query system for medical device adverse events involving balloon devices for the dilation of paranasal sinus ostia from January 2006 through December 2014. The raw data were then tabulated, with

Pneumosinus Dilatans: An exploration into the association between Arachnoid Cyst, Meningioma and the pathogenesis of Pneumosinus Dilatans.

OBJECTIVES: To investigate the association between certain intracranial masses (meningioma and arachnoid cyst) and the incidence of Pneumosinus Dilatans (PSD) - including whether the size of the mass correlates with severity of the condition. PATIENTS AND METHODS: A review of the available case reports on PSD was performed. Clinical data was extracted from 111 case reports for analysis. A further case-control study was performed using CT Head datasets to investigate the aetiological relationship between intracranial masses and PSD. Cases included patients with confirmed arachnoid cyst or meni

The treatment of acromegaly with special reference to trans-sphenoidal hypophysectomy.

Experience in the management of 100 cases of acromegaly is described. Three quarters of these had been referred directly to the endocrine clinic at the Middlesex Hospital. The remainder were referred from the Royal Post-graduate Hospital because they were thought unsuitable for yttrium implantation. The patients were studied by clinical assessment of severity, by measurement of basal growth hormone levels on three separate mornings, and by a review of possible complications. Particular attention was paid to diabetes, hypertension, cardiomegaly, respiratory, vascular and skeletal changes as wel
